Changes of hemodynamics during induction of intravenous anesthesia in children with ventricular septal defect

Abstract:Objective To observe the hemodynamic variation during induction of intravenous anesthesia in pediatric patients with ventricular septal defect(VSD)and to explore whether or not the induction need improving.Methods Fifty pediatric patients aged above 3months,American Society of Anesthesiologists(ASA) Ⅰ-Ⅱ,undergoing elective operation were enrolled in the study.According to cardiac anatomy,they were divided into VSD group and non-VSD group(n=25).Anesthesia was induced with intravenous injection of etomidate 0.2-0.3mg/kg,rocuronium 0.6mg/kg and sufentanil 2μg/kg in VSD group.Sufentanil was replaced by fentanil 2μg/kg in non-VSD group.Hemodynamic variables,including heart rate(HR),mean arterial pressure(MAP)and cardiac output(CO)were recorded when induction and tracheal intubation were performed,and 2and 5min after intubation.Results In VSD group,HR at 2and 5min after intubation was significantly lower than that at the beginning of induction and intubation(all P<0.05);HR at 5min after intubation was significantly lower than that at 2min after intubation(P<0.05);MAP at 2and 5min after intubation was significantly lower than that at the beginning of induction(both P<0.05).In non-VSD group,both HR and MAP at the beginning of intubation and 2and 5min after intubation were significantly higher than those at the beginning of induction(all P<0.05);HR at 2min after intubation and MAP at 2and 5min after intubation were significantly higher than those at the beginning of intubation(both P<0.05).In VSD group,the level of CO at the beginning of intubation and 2and 5min after intubation was significantly lower than that at the beginning of anesthesia induction(all P<0.05);the level of CO at 5min after intubation was significantly lower than that at the beginning of intubation and 2min after intubation(both P<0.05).In non-VSD group,the level of CO at 5min after intubation was significantly lower than that at the beginning of induction(P<0.05).Conclusion Anesthesia induction in children with ventricular septal defect might require further improvement in order to maintain a more stable hemodynamic induction period.
