Abstract:Background: Diffuse uterine leiomyomatosis is a benign disease with a low incidence rate, mainly manifested as menorrhagia and infertility. Due to its clinical manifestations similar to multiple uterine myomas or adenomyosis, it is often misdiagnosed in clinical practice. Research has found that they can be distinguished through histopathological examination. According to existing reported cases, Diffuse uterine leiomyomatosis mainly occurs during the child-bearing period and has a significant impact on fertility. Hysterectomy is recognized as a radical cure, but uterine-sparing treatment methods need to be continuously explored. Case: A 41-year-old woman presented with recurrent menorrhagia 5 years after myomectomy. B-ultrasound examination indicated that the uterus was enlarged and the myometrium was full of hypoechoic, Considering the patient's desire to preserve the uterus, we performed laparotomy myomectomy again. Result: Hysteroscopic myomectomy has been shown to have a certain therapeutic effect on infertile patients in the early stage of DUL. Laparotomy myomectomy can remove a wider range of lesions and can be combined with gonadotropin releasing hormone agonists or antagonists for pretreatment. Uterine artery embolization is also a commonly administered therapy. High-intensity focused ultrasound ablation has been used with significant success in reducing uterine volume but requires further evaluation for its impact on fertility. Conclusion: A DUL patient underwent extensive myomectomy. This atudy further discussed uterine-sparing treatments, and reviewed existing research reports.

Abstract:Objective:To investigate the clinical,histopathological,radiological features of smooth muscle tumor with uncertain malignant potential and the appropriate treatments.Methods:12 Cases diagnosed as smooth muscle tumor with uncertain malignant potential from January,2000 to December,2005 were chosen to be analyzed.Follow-up was performed.Results:Smooth muscle tumor with uncertain malignant potential accounted for 0.88% of all uterine smooth muscle tumors.The mean age of onset was 42.67 years with 11 premenopausal women.The mean size of the tumor was 8.41cm,5 patients with tumor resistance index less than 0.4.2 patients got serum CA125 more than 35U/ml.1 premenopausal patient presented pelvic recurrence within 0.5 years after leiomyoma resection,and another 1 patient got pelvic leiomyosarcoma 3 years after hysterectomy and bisalpingo-oopharectomy.Conclusions:Ultrasound scan and serum CA125 examination may not help to make the diagnosis of smooth muscle tumor with uncertain malignant potential before operation.Pathologic examination is the definite diagnosis.There is no consolidated treatment yet and the close follow-up is recommended.
Primary Ovarian Leiomyoma: A common tumor in rare location and report of eight cases

Abstract:Abstract Background: Ovarian Leiomyoma accounts only for 0.5 to 1% of all benign ovarian tumors. Here we present a series of eight patients over a six-year period(2012-2018). The clinico-pathological features, diagnosis and management were discussed. Methods: We had experienced eight cases of ovarian leiomyoma.The clinical features,pathologic findings,diagnosis, treatment were reviewed. Results: The mean age of these patients was 38.5 years. The majority of these patients may be asymptomatic and usually diagnosed incidentally during pelvic examination or pathologic examination after surgery. Six cases presented only with pelvic mass sized from 2 to 20 cm and even some patients persisted for more than twenty years while the other two were diagnosed due to four years of primary infertile or three months of irregular vagina bleeding. Three patients coincident with uterine leiomyoma while the others not. Three had ovarian leiomyoma degenerated and one case had a evidence of atypical 4/10. Conclusions : ovarian leiomyoma was very rare but it should be considered in the diagnosis of pelvic or ovarian solid masses. Magnetic resonance imaging was provital in its different diagnose while fast frozen pathology during operation was very necessary for its surgical decision. A proper surgical decision should be made according to patients age.

Abstract:Leiomyomas are smooth muscle tumors of the uterus and are the most common uterine neoplasm. Although leiomyomas are usually asymptomatic, they can manifest with symptoms such as pain or uterine bleeding. Leiomyomas are classified on the basis of their anatomic location and morphology. Localization of leiomyomas relative to the endometrium, myometrium, and uterine serosa with use of the International Federation of Gynecology and Obstetrics (FIGO) classification system is helpful for guiding management in symptomatic patients. The FIGO system is a practical and universally accepted approach for classifying leiomyomas to guide radiologists and clinicians in deciding management. The MRI appearance of conventional leiomyomas is related to their tissue contents of smooth muscle and fibrous tissue and is well established. The MRI features of some leiomyoma subtypes and forms of degeneration also have been described. Other smooth muscle tumors of the uterus recognized in the 2020 World Health Organization classification system include intravenous leiomyomatosis, smooth muscle tumors of uncertain malignant potential, and metastasizing leiomyoma. At the far end of the spectrum are leiomyosarcomas, which are frankly malignant and therefore must be managed accordingly. Although MRI features that suggest a diagnosis of leiomyosarcoma have been proposed, these features overlap with those of some leiomyoma subtypes and degeneration. © RSNA, 2023 See the invited commentary by Fennessy and Gargiulo in this issue. Abstract:Objective:To investigate pathological characteristics and related factors of endometrium in patients with leiomyomas.Methods:The endometrium of 479 patients with uterine leiomyomas from 2002 to 2004 was studied.Results:97 of 479 patients presented irregular vaginal bleeding.220 patients had single myoma and 259 patients had multiple myomas.Degeneration of myoma occurred in 70 patients.67 patients belonged to cellular leiomyoma.109 cases presented endometrial lesions including 42 endometrial polyp(8.76%),41 simple hyperplasia(8.56%),12 complex hyperplasia(2.51%),3 atypical hyperplasia(0.63%),and 11 endometrial carcinoma(2.29%).Conclusions:Uterine leiomyomas may complicate with endometrial lesions and get different endometrial pathological characteristics.However,irregular vaginal bleeding and menopause are related with endometrial lesions and endometrium carcinoma,not the amount of myoma and degeneration of myoma.

Abstract:Purpose To explore the clinicopathological, immunohistochemical and ultrastructural features of smooth muscle tumors in abdominal soft tissue and their differential diagnosis. Methods Thirty-four cases of smooth muscle tumors from the abdominal cavity or retroperitoneum were studied. Results The tumors occurred in 8 men and 26 women, with age ranged from 26 to 77 years (mean 47.6 years), in which twenty-eight cases arose from the retroperitoneum, 4 from mesentery and 2 from abdominal cavity. The size of tumors ranged from 5 to 18 cm (mean 9.86 cm) in diameter and the tumor cell components varied, mainly spindle cells (32 cases), in which one case was accompanied with reactive giant cells and one case displayed area of high grade pleomorphic sarcoma (dedifferentiated leiomyosarcoma area), epithelioid cells (one cases), and mixed cells (one case). Tumors expressed desmin(21/22),α-SMA(21/21), MSA(21/21), CD117(1/21), CD34(1/21), S-100(3/21), and PGP9.5(2/21). Electron microscopically, continuous basal lamina, regular caveolae, and dense plaque were observed. Four cases were leiomyomas, one case was borderline and 29 cases were leiomyosarcoma. Follow-up of 2 cases of leiomyomas showed long-term survival of 5 years and 10 years, In the leiomyosarcomas group, follow-up information was available in 13 cases, in which 11 cases developed recurrence, metastasis or death within 3 years, one cases died of disease in 5 years, and only one cases survived for 10 years without tumor. Conclusions The smooth muscle tumor of abdominal soft tissue mainly occurs in retroperitoneum. Most cases are female and malignant. The features of tumor coagulative necrosis, mitoses ≥4 per 50 high-power fields and obvious nuclear atypia indicate malignancy. Differential diagnosis includes gastrointestinal stromal tumors, malignant nerve sheath tumors and others.

Abstract:Objective To explore the clinical imaging and pathological features of cotyledonoid dissecting leiomyoma of the uterus.Method The clinical data of one patient with cotyledonoid dissecting leiomyoma of the uterus was retrospectively analyzed.Results Irregular mass was detected incidentally in a 32-year-old woman with a provisional diagnosis of sarcoma.A multinodular fungating tumor adhering to the left wall of the uterus and extending into the broad ligament was discovered,which was composed of fascicles and nodules of smooth muscle cells with prominent peri-nodular hydropic degeneration.Coagulative necrosis,mitoses and nuclear atypia were absent.The patient underwent total hysterectomy and left salpingo-oophorectomy,no recurrence was found during the follow-up of 20 months.Conclusions Cotyledonoid dissecting leiomyoma of the uterus is a rare benign tumor with a bizarre gross appearance confused with malignant neoplasm.Increasing awareness of the disease can help us to avoid wrong treatment.

Abstract:Uterine smooth muscle tumors are a heterogeneous group of mesenchymal neoplasms with multiple histologic variants and distinct biological behaviors. Pathologic classification (benign, uncertain malignant potential, malignant) relies on the evaluation of mitotic index, necrosis, and degree of cytologic atypia, with different thresholds based on each subtype. Immunohistochemistry and other ancillary studies may be necessary to establish the diagnosis in a subset of cases, given the morphologic overlap with other mesenchymal neoplasms, including low-grade and high-grade endometrial stromal tumors, inflammatory myofibroblastic tumors, and PEComa. Recent advances in molecular diagnostics have refined the classification of smooth muscle tumors, but most cases are diagnosed purely on histologic grounds.

Highly Cellular Leiomyoma of Uterus: a Comparative Morphologic and Immunohistochemical Study of Endometrial Stromal Tumors

Abstract:OBJECTIVE:To study the morphological characteristics and immunophenotype of highly cellular leiomyoma (HCL) of uterus, compared with that of uterine endometrial stromal tumors (EST).METHODS:HE and immuno-stained sections EnVision method from 20 cases of HCL, 21 cases of EST and 1 case of stromomyoma were reviewed. Monoclonal antibodies against h-caldesmon, calponin, CD10, desmin and smooth muscle actin (SMA) were used for immunohistochemistry studies.RESULTS:On microscopic examination, HCL were densely cellular and composed of cells that ranged from spindle-shaped to round with scanty cytoplasm. A focal fascicular pattern was present in all cases. Blood vessels with large, thick muscular walls were a conspicuous feature of the majority of tumors. Cleft-like spaces were present in 9 tumors and 15 cases exhibited irregular focal extensions into the adjacent myometrium. ESTs were composed of cells that resembled endometrial stromal cells of proliferative endometrium. These cases included a significant component of delicate blood vessels similar to spiral arterioles. All 20 low grade endometrial stromal sarcoma cases had infiltrative growth to adjacent myometrium. Immunoreactivities of HCL for h-caldesmon, calponin, CD10, Desmin and SMA were 80.0% (16/20), 100% (20/20), 0 (0/20), 95.0% (19/20) and 100% (20/20), respectively, whereas the positive rates of EST were 4.7% (1/21), 23.8% (5/21), 66.7% (14/21), 23.8% (5/21) and 19.0% (4/21), respectively (P = 0.001).CONCLUSIONS:Highly cellular leiomyomas have distinct morphologic features. H-caldesmon, calponin, CD10, desmin and SMA are helpful in the differential diagnosis of HCL and EST.

Abstract:Objective To learn the expression and significance of small ubiquitin-like modifier (SUMO)-1 and P53 protein in uterine leiomyosarcoma.Methods From March 2006 to March 2012,20 cases of uterine leiomyosarcoma and 29 cases of uterine leiomyoma were collected.Immunohistochemical Envision method was used to detecte the expression level of SUMO-1 and P53 in 49 cases with uterine sarcoma,and prognosis was followed up by telephone in 20 cases of uterine leiomyosarcoma.Results The positive expression rates of SUMO-1 and P53 in uterine leiomyosarcoma were 85％ and 30％,respectively;the positive expression rates of SUMO-1 and P53 in uterine leiomyoma were 52％ and 0％,respectively;the positive expression rate of SUMO-1 in uterine leiomyosarcoma was significantly higher than that in uterine leiomyoma (P＜0.01);the positive expression rate of P53 in uterine leiomyosarcoma was significantly higher than that in uterine leiomyoma (P＜0.01);the expressions of SUMO-1 and P53 protein in uterine leiomyosarcoma were not associated with prognosis (P＞0.05).Conclusion P53 may be implicated in the pathogenesis of uterine leiomyosarcoma and is useful in differentiating uterine leiomyosarcoma from uterine leiomyoma.SUMO-1 could be closely related to the occurrence of uterine leiomyosarcoma.

Abstract:Uterine leiomyomas, also called uterine fibroids or myomas, represent one of the most common benign tumour types in women of a fertile age. Leiomyomas arise due to transformation of the layer of smooth muscle cells of corpus uteri - the myometrium. Despite frequent occurrence of this disease, the molecular mechanisms behind the origin and development of leiomyomas are still relatively unknown. Most predisposed are obese women and women of African origin. In more than half of cases, leiomyomas remain asymptomatic. Genetic factors also have an important impact on the development of these hormone-dependent tumours. However, the clinical and molecular characteristics of familiar and sporadic leiomyomas can widely differ. The main reason is the heterogeneity of this disease and the abundance of factors which can underlie their tumourigenesis. Clinical diagnosis of uterine leiomyomas without surgical interference can be hindered in the case of small, mostly submucosal leiomyomas or if it is necessary to avoid potential malignancy of tumour. Also, medical treatment of uterine leiomyomas cannot be nowadays considered sufficient with many medical agents still being tested only within clinical research. The main goal of this article is to summarise known facts about the aetiology of leiomyomas, risk factors that contribute to their development, known molecular-genetic aberrations connected with the presence of leiomyomas as well as the possibilities of their diagnosis and treatment.
Myxoid leiomyoma of cervix

Abstract:Leiomyoma, a benign smooth muscle tumour of uterus, is the most common pelvic tumour which occurs in women of reproductive age group. Uterine leiomyomas are frequently seen in women who are older than 30 years of age and they are very rarely seen in women who are below the age of 18 years. They tend to regress after menopause. Incidence is 4% in women who are below 30 years of age. However , lifetime risk of fibroids in women who are over the age of 45 years is more than 60%, with an incidence which is higher in blacks than in whites Cervical leiomyomas constitute 1-2% of the total leiomyoma cases and they rarely occur. There are three types of cervical leiomyomas, namely; interstitial, supravaginal, and polypoidal. Supravaginal leiomyoma is the commonest type. Large cervical fibroids present with abdominal masses, incarcerated procidentia, uterine inversion, cervical malignancy, etc. The prevalence of fibroids during pregnancy, irrespective of site, has been reported to be 1-4%. Degeneration in leiomyomas have been described in 65% of the cases. Myxoid leiomyoma is characterized by absence of mito tic activity and the presence of myogenic phenotype. The higher end of prevalence for a myxoid degeneration has been reported in upto 50% of all degenerations in uterus, but myxoid leiomyoma has been rarely described in cervix. Cause of a myxoid change in pregnancy is unknown.
Experience of Surgical Treatment with Leiomyosarcoma in Abdominal Soft Tissue:11 Cases Report

Abstract:Objective To explore the clinical manifestation,surgical treatment,pathological characteristics and prognosis of smooth muscle sarcoma in abdominal soft tissue.Methods Eleven cases of leiomyosarcoma in abdominal soft tissue from Jul 2009 to Mar 2012 were analyzed retrospectively.All cases were diagnosed with pathology.Results The sarcoma was found in 3 men and 8 women,aged 34-66 years(median age: 54 years).Abdominal pain was the main symptom and abdominal mass the main sign.The rate of complete tumor resection was 72.7% while the concomitant resection of involved adjacent organs was 63.6%.In this study,3 cases had recurrence.Conclusions The smooth muscle sarcoma of abdominal soft tissue is mostly in retroperitoneum and in females.The radical resection is the best way of treatment.Preoperative imaging and the multidisciplinary collaboration play important roles in the surgical treatment.
The clinical pathological features of adenomatoid tumors in the female reproductive system

Abstract:Objective:To investigate the clinical and pathological features of the female reproductive system adenomatoid tumors and to study the difference between the adenomatoid and the other diseases.Method:Clinical and macroscopic features in 73 cases with adenomatoid were observed.Results:There were 72 cases in uterus,1 in fallopian tubes and 1 case in both the uterus and the ovary.The incidence of adenomatoid tumours in uterus was 2.4%,and nearly 75% of the cases were incorporated with leiomyomas or adenomyosis.72 cases were in nodular pattern,and 1 in infused pattern.Grossly,the diameter of the 72 uterus tumors rang from 1~5cm.Most of them were located in the subserosa or near by the subserosa of the uterine and the rest located in the myometrium.The cut surface presented a nodular pattern with grayish white or yellowish in color,without a clear-cut margin from the surroundings.Microscopically,the tumor consisted of various gland-like structure or luminal spaces lined with flat,cuboidal or low columnar cells.Immunohistochemical staining showed the tumor cells positive for cytokeratin.Conclusion:Adenomatoid tumors of the female genital tract is mesothelial in origin and uterus is considered as the most common site of occurance.The adenomatoid must be included in the differentiated category of uterus nodule lesions.Immunohistochemical phenotypes can be used as an important evidence for differential diagnosis.The biological behavior of adenomatoid tumors is benign and with a good prognosis.
["Benign metastasizing leiomyoma"? A case report without evidence for unbalanced cytogenetic aberrations]

Abstract:There are rare cases of histologically benign appearing uterine leiomyomas with subsequent development of multifocal extrauterine smooth muscle tumors, most often located in the lung. It remains unclear whether this evolves from a morphological innocent appearing low grade sarcoma or from proliferation of multifocal but autochtonous cellular foci. Frequently, and in part recurrent karyotypic abnormalities were described for leiomyomas and leiomyosarcomas. Thus, we looked for imbalanced genetic aberrations in a case of a "benign metastasizing leiomyoma" by means of comparative genomic hybridisation (CGH). A 46-year-old female developed multiple bilateral lung nodules 4 years after hysterectomy. Fine needle biopsy of one nodule and subsequent resection of three lung nodules were performed. These and the hysterectomy specimen were investigated by histology, immunohistochemistry and CGH. Revision of the hysterectomy specimen revealed multiple leiomyomas without any evidence for malignancy. Lung nodules were composed of benign appearing smooth muscle cells with epithelial lined cleft-like spaces. Leiomyomata of the uterus and the lung showed a reactivity against actin, desmin, estrogen- and progesteron receptor antigens. DNA analysis by CGH revealed a normal karyotype without evidence for an imbalanced loss or gain of DNA. Recurrent cytogenetic alterations are common in uterine leiomyomas, most often del (7)(q11.2-22q31-32) and t(12;14)(q14-15; q23-24). Leiomyosarcomas display diverse karyotypic abnormalities, most often involving chromosomes 1, 7, 13 and 14. Thus, the missing karyotypic imbalance in the presented case favors a pathogenesis which is different from usual leiomyomas as well as leiomyosarcomas.

Abstract:Introduction. The current trend of shifting the time of delivery to an older age requires the improvement of methods, algorithms for diagnosis, and choice of management for organ-preserving treatment of uterine fibroids. There are few reports in which data obtained using preoperative imaging methods are compared with the results of histological examination of leiomyomata. The aim of the study was to investigate the features of morphotype and clinical course of the disease in patients with common and cellular uterine leiomyoma. Materials and methods. We divided 80 patients with uterine leiomyoma into 2 groups after comprehensive clinical and morphological examination and treatment. Group 1 involved 50 patients with conventional uterine leiomyoma; group 2 included 30 patients with cellular uterine leiomyoma. All patients had a patient-specific personalized three-dimensional mathematical and graphical model of uterine fibroids built before surgery. Leiomyomata were subjected to a standard pathological examination with hematoxylin and eosin staining, immunohistochemical study with Ki-67 antibody, and Van Gieson picrofucsin to assess fibrosis. Results. The frequency of dysmenorrhea, menorrhagia, primary infertility, and endometrial hyperplastic processes significantly exceeded (p<0.05) those in the group of patients with cellular myoma. In 64.6% of patients of group 2, the indications for surgery were location of myomatous nodes and their enlargement; in group 1, the percentage of such patients was 26.0% (p<0.05). Large uterine fibroids and multiple locations of nodes were detected in more than 50% of patients in group 2, while those in patients of group 1 were determined only in 14%. In patients with conventional leiomyoma, the Ki-67 index was 0–1–2%, whereas in patients with cellular leiomyoma it was 3–4–5%. In group 1, the fibrosis area was 20–40%; in group 2, this indicator reached up to 10%. Conclusion. Uterine cell myoma is characterized by rapid growth, large node sizes, a higher frequency of both submucosal location and infertility, a combination with hyperplastic endometrial processes, uterine bleeding, pronounced pain syndrome, and a younger age. Abstract:Abstract Background: Benign metastasizing leiomyoma (BML) is a rare disease that is usually seen in females of reproductive or premenopausal age with a previous surgery history of uterine myomectomy or hysterectomy. The most common sites of metastases are pulmonary metastases, and other sites include heart, bones, liver, lymph nodes, skin, bladder, esophagus, skeletal muscles, and central nervous system. Here we report a case of 50-year-old women with history of hysterectomy whom was first suspect uterine sarcoma but finally confirmed BML with lung and lymph node metastases at the same time and discuss the treatment and prognosis of BML. Case presentation: A 50-year-old women who have a history of abdominal total hysterectomy presented with mild but persistent abdominal pain for over 3 months. She was suspected uterine sarcoma before surgery and laparoscopic extensive debulking surgery including bilateral oophorectomy, pelvic and para-aortic lymph node dissection to the level of the left renal vein, and right inguinal lymph nodes dissection trans-cutaneously were done. Pathology after surgery confirmed benign leiomyoma and she was diagnosed BML. No medicine was given after the surgery and now the follow up is of no significance. Conclusions: Benign metastasizing leiomyoma (BML) is a rare disorder in which histologically benign smooth muscle tumor metastasize to extra-uterine sites. Metastases are commonly seen in the lung, liver, lymph nodes, skin, bladder, esophagus, skeletal muscles, et al. BML is usually misdiagnosed as malignant tumor before surgery until pathology confirmed the benign nature. The treatment is controversial and undetermined. The prognosis is usually favorable because of its benign nature.
