Assessment of diffusion-weighted MR imaging for pelvic lymph node in patients with prostate cancer:a preliminary study

Abstract:Objective To assess the diagnostic value of diffusion-weighted imaging (DWI) for lymph node in patients with prostate cancer from that of benign prostate hyperplasia. Methods The diffusion-weighted images (b=300 s/mm2) of 15 patients with prostate cancer (58 lymph nodes) and 15 patients with benign prostate hyperplasia (30 lymph nodes) were retrospectively analyzed, with apparent diffusion coefficient (ADC) of all the lymph nodes measured and the ADC between patients of cancer and hyperplasia compared. Results The ADC of lymph nodes was significantly greater in patients with benign prostate hyperplasia [(2.282±0.573)×10-3 mm2/s] than that with prostate cancer [(1.607±0.256)×10-3 mm2/s)](P=0.000). The ADC between lymph nodes with short axis smaller than 1.0 cm [(1.602±0.267)×10-3 mm2/s] and that with short axis greater than 1.0 cm [(1.610±0.254)×10-3 mm2/s] in patients of prostate cancer showed no statistical significance (P=0.459). The ADC of lymph nodes with short axis smaller than 1.0 cm [(1.602±0.267)×10-3 mm2/s] in patients with prostate cancer was smaller than that in patients with prostate hyperplasia [(2.282±0.573)×10-3 mm2/s)(P=0.003). The ADC of lymph nodes with ratio of short axis to long axis smaller than 0.7 [(1.631±0.205)×10-3 mm2/s) was higher than that of the ratio greater than 0.7 [(1.594±0.283)×10-3 mm2/s) in patients of prostate cancer (P=0.039). Conclusion DWI may be useful in discriminating metastatic lymph nodes of patients with prostate cancer.
