Periodontal Tissues and Root Reactions after Rapid Orthodontic Extrusion of Subgingivally Fractured Incisors

Abstract:Objective:To evaluate the changes of gingiva,alveolar and root in subgingivally fractured incisors after rapid orthodontic extrusion.Method:18 patients with fractured incisors extended 2~5 mm below the gingival line were selected.Two weeks after root canal therapy,the subgingival fragment was lifted up and the fracture line was brought above the level of the gingival line by means of edgewise fixed appliance.Once the extrusion had been completed,the tooth was stabilized and held for 6 months.Radiographs were taken respectively before extrusion and after stabilization using the long cone and right angle technique and the Digora Optime intraoral digital imaging system.Changes of the length of gingiva,alveolar and root were measured and compared with the distance of the tooth extrusion.Result:The gingiva and alveolar moved in the same direction in which the teeth were extruded.The height of alveolar on the mesial and distal sides increased about 33% and the width of gingiva on the labial surface increased about 36% as far as the teeth were extruded.And there is no significant change in root length.Conclusion:Under the continuous and proper tooth axial extrusion force,the fractured incisor moved rapidly while coronal migration of the periodontal tissues was less pronounced.And if combined with crown lengthening surgery,both esthetic gingival line coordinated with the adjacent teeth was achieved and biological width was reestablished.
