Abstract:Purpose:To evaluate the techniques and clinical applications of virtual endoscopy based on spiral CT scan (CTVE) in bladder tumors. Methods:Thirty five patients suffered from bladder tumors were undergone volume scanning using spiral CT. Virtual endoscopy images were generated from volumetric data using software and the bladders were observed from multi directions by Fly Through software. All cases were compared with the diagnoses of fiberoptic cystoscopy. Results:Thirty four cases of bladder tumors were presented clearly on CTVE scans including 2 cases missed on fiberoptic cystoscopy and the diagnoses were confirmed by operation or pathology except for 1 case of bladder tumor (diameter less than 0.5 cm) missed. Conclusions:CTVE has proved to be a non-invasive and reliable methods in the diagnosis of bladder tumor, which can be served as a supplement to fiberoptic cystoscopy.

Abstract:Objective To study the technique and clinical applications of the virtual cystoscopy bassed on spiral CT scan.Methods Spiral CT scans were done in 15 patients with clinically confirmed bladder tumor. The images of the vesical inner walls were reconstructed by the redealing function of the computer with spiral CT scandatasets.Results Bladder tumors were observed in all the 15 patients including 2 missed on fiberoptic cystoscopy.Conclusions Virtual cystoscopy bassed on spiral CT scan is an efficacious and noninvasive technique,which can be served as a supplement to fiberoptic cystoscopy.

Abstract:Objective: To develop a non-invasive,effective,and convenient virtual cystoscopy system so as to realize three-dimensional visualization and observation for the bladder lumen of patient with bladder cancer(BC).Method:Based on the magnetic resonance imaging(MRI)datasets and medical imaging interaction toolkit(MITK)libraries to establish a database for image information of patients.And based on the arithmetic of Fast Marching Segmentation and arithmetic of live-wire to design semi-automatic partition method of image so as to extract region of interest(ROI),and realize three-dimension(3D)reconstruction and other functions,and obtain 3D mode of bladder lumen pre reconstruction.Using virtual endoscope function to carry out multi-angle observation and realize the detection for lesion of bladder lumen.Results: The 3D visualization and observation of bladder lumen can obtain importantly diagnosis-related information such as tumor locations,amount and size,and can realize preoperative detection of BC.Conclusion: The development of the virtual cystoscopy system,the non-invasive measurement of visualization BC and 3D visualization observation can provide new assistant mean for clinically early screening of susceptible population and postoperative reexamination of patients.
Microfluidic Assaying of Circulating Tumor Cells and Its Application in Risk Stratification of Urothelial Bladder Cancer

Abstract:Bladder cancer is characterized by its frequent recurrence and progression. Effective treatment strategies need to be based on an accurate risk stratification, in which muscle invasiveness and tumor grade represent the two most important factors. Traditional imaging techniques provide preliminary information about muscle invasiveness but are lacking in terms of accuracy. Although as the gold standard, pathological biopsy is only available after the surgery and cannot be performed longitudinally for long-term surveillance. In this work, we developed a microfluidic approach that interrogates circulating tumor cells (CTCs) in the peripheral blood of bladder cancer patients to reflect the risk stratification of the disease. In a cohort of 48 bladder cancer patients comprising 33 non-muscle invasive bladder cancer (NMIBC) cases and 15 muscle invasive bladder cancer (MIBC) cases, the CTC count was found to be considerably higher in the MIBC group compared with the NMIBC group (4.67 vs. 1.88 CTCs/3 mL, P=0.019), and was significantly higher in high-grade bladder cancer patients verses low-grade bladder cancer patients (3.69 vs. 1.18 CTCs/3mL, P=0.024). This microfluidic assay of CTCs is believed to be a promising complementary tool for the risk stratification of bladder cancer.

Abstract:Computer-aided detection(CAD) for virtual colonoscopy(VC) is a new technique that utilizes the difference in morphology and other features between polyps and normal tissues to detect colonic polyps automatically.Compared with physicians' performance using VC directly,CAD-based system can provide objective and consistent results,facilitate interpretation process,and be anticipated a promising mass screening for colorectal cancer in clinics.With the development of VC in past several years,a fundamental framework for CAD has been established,aiming to improve the sensitivity and specificity of detection for clinical acceptance.This paper addresses the key techniques and clinical performance of CAD-based systems,and discusses issues remained and future development of the technique.

Abstract:Objective To compare the CT-based virtual colonoscopy and optical colonoscopy in a clinical setting.Methods Totally 20 patients were recruited and were subjected to optical colonoscopy and the polyp findings were recorded,and these patients were then immediately received thin-slice abdominal CT scanning and the data were acquired.Then the polyps in the virtual colonoscopy system were identified by both an experienced radiologist and a gastroenterologist.Results It was indicated that the virtual colonoscopy and optical colonoscopy had a similar sensitivity and specificity for polyps larger than 5 mm.Meanwhile,virtual colonoscopy needed a shorter examination time and the patients had a better patient experience than optical colonoscopy.Conclusion Virtual colonoscopy as a new screening method of colon lesions has a great advantage in detecting lesions larger than 5 cm,but not for a smaller one.

Abstract:OBJECTIVE:To evaluate spiral CT urography (SCTU) and CT virtual endoscopy (CTVE) in detecting urologic diseases.METHODS:SCTU was performed in 46 patients with urological diseases including renal neoplasms (2), paropelvic cysts (2), ureteral calculi (6), ureteral stenosis (4), ureteral neoplasms (2), double kidneys and ureter malformation (1), bladder neoplasms (28) and bladder endometreosis (1). The 6 patients with ureteral diseases and 29 patients with bladder diseases underwent CTVE based on spiral CT scan. All CTVE findings were compared with those of B-mode ultrosonography, intravenous urography (IVU), retrograde pyelography (RGP), conventional CT or cystoscopy.RESULTS:All upper urinary tract diseases and bladder diseases (28 cases) were detected by SCTU and CTVE scans and they were confirmed operatively or pathologically except one case of bladder neoplasm (diameter less than 5 mm) was missed.CONCLUSION:SCTU and CTVE have proved to be non-invasive and reliable in the diagnosis of urological diseases and are superior to IVU or conventional CT. CTVE can serve as a supplementary method to fiberoptic cystoscopy or ureteroscopy.

Abstract:Abstract Purpose Vacuum-assisted breast biopsy (VABB) offers high accuracy in the diagnosis of breast tumors but makes the location of the original lesion difficult to identify when the subsequent breast-conserving surgery (BCS) is to be carried out. The aim of this study was to describe an effective technique to identify the exact location of the VABB cavity and to make breast-conserving resection more precise. Methods In this study, a balloon urinary catheter was inserted through the biopsy track; thus, the inflated balloon and catheter perfectly reconstructed the VABB cavity and biopsy track. Then breast-conserving resection with the guidance of the catheter was performed. Six patients were enrolled and assessed for their surgical findings, pathological results, surgical margins, and postoperative complications. Results Residual tumors were confirmed in 4 out of 6 patients after VABB. All the patients gained negative margins and underwent successful breast-conserving surgery. None of them required re-excisions. Satisfactory cosmetic results were observed after the catheter-directed BCS. Conclusions The results of this study show the effectiveness of urinary catheter-guided breast-conserving surgery after VABB. The procedure is simple and inexpensive. This new technique enabled precise breast-conserving resection of the targeted lesion and potentially improve successful rate of breast-conserving surgery.

Abstract:Abstract. The aim of the study was to evaluate the diagnostic potential of the half-Fourier acquired single-shot turbo spin echo (HASTE) sequence for MRI cystoscopy compared with CT cystoscopy and the gold standard conventional cystoscopy. The MRI- and CT cystoscopy was performed in 29 patients with bladder tumors documented at cystoscopy and a control group using a 1.5-T unit (6-mm slice thickness) and a helical CT (3-mm collimation, pitch 1) after filling the bladder with air. Axial MRI and CT images were transferred to a workstation for application of virtual cystoscopy and compared with routine cystoscopy. Axial images and virtual cystoscopies were read by three readers for size and location of tumors. Forty-seven tumors were diagnosed at cystoscopy (12<1 cm, 35≥1 cm). The sensitivity for detection of tumors smaller than 1 cm was 88.9% at MRI cystoscopy and 100% for tumors of 1 cm or larger. These results were not statistically different from the other modalities. Three tumors smaller than 1 cm were not detected at CT cystoscopy, four not all identical at MRI cystoscopy. One tumor with a wall thickening was detected on axial CT and MR images and CT cystoscopy by all observers, but only by two at MRI cystoscopy. The MRI cystoscopy is a diagnostic modality with results comparable to conventional and CT cystoscopy.

A Review on Virtual Cystoscopy Techniques

Abstract:Objective:Bladder cancer is the ninth cause of cancer deaths and has high recurrence rate after resection of the tumors. Cystoscopy is the current most accurate method for investigating the bladder abnormalities. However,it is expensive,uncomfortable and invasive. It is possible to induce bleeding,urinary-tract infection and even puncture of bladder. Advances in medical imaging and computer technologies make virtual cystoscopy a potential alternative. Methods:Computed tomography(CT) and magnetic resonance imaging(MRI) are the preferred imaging modalities for virtual cystoscopy to get clear structural images or(and) functional images of the bladder. The boundary of bladder is segmented manually or automatically,and then the bladder is reconstructed and displayed by surface rendering. Useful features are extracted from the images and expressed on the reconstructed bladder to provide more valuable diagnosis information for doctors. Results:Comparing with conventional cystoscopy,virtual cystoscopy is noninvasive,more convenient,flexible and can provide more useful diagnosis information as well. Conclusions:Virtual cystoscopy is a promising method of detection and reexamination of bladder cancer. So far,more researches are needed for the virtual cystoscopy before clinical application. It clinical and commercial value are under investigation.
A New Partial Volume Segmentation Approach to Extract Bladder Wall for Computer Aided Detection in Virtual Cystoscopy

Abstract:We propose a new partial volume (PV) segmentation scheme to extract bladder wall for computer aided detection (CAD) of bladder lesions using multispectral MR images. Compared with CT images, MR images provide not only a better tissue contrast between bladder wall and bladder lumen, but also the multispectral information. As multispectral images are spatially registered over three-dimensional space, information extracted from them is more valuable than that extracted from each image individually. Furthermore, the intrinsic TI and TZ contrast of the urine against the bladder wall eliminates the invasive air insufflation procedure. Because the earliest stages of bladder lesion growth tend to develop gradually and migrate slowly from the mucosa into the bladder wall, our proposed PV algorithm quantifies images as percentages of tissues inside each voxel. It preserves both morphology and texture information and provides tissue growth tendency in addition to the anatomical structure. Our CAD system utilizes a multi-scan protocol on dual (full and empty of urine) states of the bladder to extract both geometrical and texture information. Moreover, multi-scan of transverse and coronal MR images eliminates motion artifacts. Experimental results indicate that the presented scheme is feasible towards mass screening and lesion detection for virtual cystoscopy (VC).
The Clinical Application of CT Virtual Colography

Abstract:Objective To investigate the value of virtual colonoscopy in the diagnosis of malignant and benign lesions in the colon. Methods CT colography was performed in 37 patients. Patients included 20 cases colon polyps, 6 cases colon cancer, 4 cases diverticulum, 5 cases ulcerative colitis and 2 cases other benign lesions. The colon lesions were evaluated with 2D and 3D images, depending on individual software. Results were compared with colonoscopy for each case. Results 17 cases colon polyps, 6 cases colon cancer, 4 cases diverticulum, 2 cases ulcerative colitis were detected by CTC. Conclusion CT virtual colography is a promising technique for the detection of colorectal lesions. This procedure is minimal invasive and fast, causes mild patient discomfort.
[Clinical Application of CT-guided Preoperative Pulmonary Nodule Localization Technique].

Abstract:BACKGROUND AND OBJECTIVE:It's difficult to localize the accurate position for some pulmonary nodules in video-assisted thoracoscopic surgery (VATS) wedge resection. The aim of this study is to retrospectively analyze the clinical significance of CT-guided preoperative pulmonary nodule localization technique.METHODS:Between Jan 2010 and Apr 2011, 20 patients of the First Affiliated Hospital of Medical School of Zhejiang University underwent preoperative pulmonary nodule localization technique before performing VATS wedge resection of the pulmonary nodule. Diameter of the lesion ranges from 0.5 cm to 2 cm (average 9.8 cm±5.3 cm). It was evaluated with the success rate in localization technique, rate of localization related complications, and rate of transferring thoracotomy.RESULTS:Eighteen patients underwent successful CT-guided Hookwire localization, with the average time of 14.5 minutes. There was no serious complications.CONCLUSION:CT-guided preoperative pulmonary nodule localization is a promising technique for small solitary pulmonary nodules. It could play an important role in accurate localization of small pulmonary nodules, and it is a safe technique with less postoperative complications.
Utilisation of virtual non-contrast images and virtual mono-energetic images acquired from dual-layer spectral CT for renal cell carcinoma: image quality and radiation dose

Abstract:Abstract Background Renal cell carcinoma (RCC) is the most common renal malignant tumour. We evaluated the potential value and dose reduction of virtual non-contrast (VNC) images and virtual monoenergetic images (VMIs) from dual-layer spectral CT (DL-CT) in the diagnosis of RCC. Results Sixty-two patients with pathologically confirmed RCC who underwent contrast-enhanced DL-CT were retrospectively analysed. For the comparison between true non-contrast (TNC) and VNC images of the excretory phase, the attenuation, image noise, signal-to-noise ratio (SNR) and subjective image quality of tumours and different abdominal organs and tissues were evaluated. To compare corticomedullary phase images and low keV VMIs (40 to 100 keV) from the nephrographic phase, the attenuation, image noise, SNR and subjective lesion visibility of the tumours and renal arteries were evaluated. For the tumours, significant differences were not observed in attenuation, noise or SNR between TNC and VNC images ( p > 0.05). For the abdominal organs and tissues, except for fat, the difference in attenuation was 100% within 15 HU and 96.78% within 10 HU. The subjective image quality of TNC and VNC images was equivalent ( p > 0.05). The attenuation of lesions in 40 keV VMIs and renal arteries in 60 keV VMIs were similar to those in the corticomedullary images ( p > 0.05). The subjective lesion visibility in low keV VMIs is slightly lower than that in the corticomedullary images ( p < 0.05). Using VNC and VMIs instead of TNC and corticomedullary phase images could decrease the radiation dose by 50.5%. Conclusion VNC images and VMIs acquired from DL-CT can maintain good image quality and decrease the radiation dose for diagnosis of RCC.

CT Virtual Endoscopy: an Initial Evaluation of Its Clinical Applications

Abstract:Objective To evaluate CT virtual endoscopy (CTVE) in clinical practice.Materials and Methods CTVE was performed in 113 cases (86 patients and 27 normal volunteers), with the scanning sites of tracheobronchial tree (n=39), alimentary tract (n=33), larynx (n=13), cardiovascular (n=12), ear (n=6), billiary tract (n=4), bladder and ureter (n=3) and paranasal sinuses (n=3). The diagnosis of 72 patients were proved by pathology.Results The appearances of normal anatomy and varied lesions seen on CTVE images were similar to those seen with fiberoptic endoscopy. CTVE could demonstrate the stenosis and occlusion of the lumen, protrusive lesion over 5mm, and ulcer over 14mm in size. CTVE showed certain ability in localizing and characterizing lesions.Conclusion As a noninvasive diagnostic technique, CTVE can be clinically served as a supplement of, and partly replace, the fiberoptic endoscopy.
[Evaluating the Clinical Applications of CTVL in Laryngeal Carcinomas].

Abstract:OBJECTIVE To evaluate the clinical applications of computer tomography virtual laryngoscopy (CTVL) in laryngeal carcinoma. METHOD 34 cases of laryngeal carcinomas and 5 cases of normal larynx were scanned by spiral CT and analyzed by computers. The virtual endoscopy findings of all patients were compared with those of fiber optical laryngoscopy and surgical findings. All the carcinomas were pathologically proved. RESULT The visualization of normal anatomical structure in larynx on CTVL is the same as that of fiber optic laryngoscopy. Tumor local extension showed in CT virtual endoscopy is superior to that of fiber optic laryngoscopy. Images of early laryngeal carcinomas showed in CT virtual endoscopy is not so good as those of fiber optic laryngoscopy. CONCLUSION CT virtual laryngoscopy is a good complement to fiber optic laryngoscopy.
Achieving sub-millisievert CT colonography for accurate colorectal tumor detection using smart examination protocols: a prospective self-controlled study

Abstract:Purpose: To assess the feasibility of combining Auto-kVp selection technique, higher preset ASIR-V and noise index (NI) to realize individualized sub-mSv CT colonography (CTC) for accurate colorectal tumor detection and localization. Methods: Ninety patients with suspected colorectal cancer (CRC) were prospectively enrolled to undergo standard dose CTC (SDCTC) in the prone and ultra-low dose CTC (ULDCTC) in the supine position. SDCTC used 120 kVp, preset ASIR-V of 30%, SmartmA for a NI of 13; ULDCTC used Auto-kVp selection technique with 80 or 100 kVp, preset ASIR-V of 60%, SmartmA for a NI of 13 for 80 kVp, and NI of 15 for 100 kVp. The effective dose (ED), image quality [signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) of colorectal neoplasms] between the two protocols were compared and the accuracies of tumor locations were evaluated for CTC in comparison with the surgery results. Results: The mean ED of the ULDCTC-80 kVp subgroup was 0.70 mSv, 71.43% lower than the 2.45 mSv for the 120 kVp group, while that of the ULDCTC-100 kVp subgroup was 0.98 mSv, 73.00% lower than the 3.63 mSv for the 120 kVp group (P < 0.001). The tumor SNR and CNR of the ULDCTC were higher than those of SDCTC (P < 0.05), while there was no difference in the subjective image quality between them with good inter-observer agreement (Kappa: 0.805-0.923). Both SDCTC and ULDCTC groups had high detection rate of colorectal tumors, along with good consistency in determining tumor location compared with surgery reports (Kappa: 0.718-0.989). Conclusion: The combination of Auto-kVp selection, higher preset ASIR-V and NI achieves individualized sub-mSv CTC with good performance in detecting and locating CRC with surgery and consistent results between SDCTC and ULDCTC.
Postprocessing techniques of CT colonography in detection of colorectal carcinoma.

Abstract:AIM: To evaluate the value of postprocessing techniques of CT colonography, including multiplanar reformation (MPR), virtual colonoscopy (VC), shaded surface display (SSD) and Raysum, in detection of colorectal carcinomas. METHODS: Sixty-four patients with colorectal carcinoma underwent volume scanning with spiral CT. MPR, VC, SSD and Raysum images were obtained by using four kinds of postprocessing techniques in workstation. The results were comparatively analyzed according to circumferential extent, lesion length and pathology pattern of colorectal carcinomas. All diagnoses were proved pathologically and surgically. RESULTS: The accuracy of circumferential extent of colorectal carcinoma determined by MPR, VC, SSD and Raysum was 100.0%, 82.8%, 79.7% and 79.7%, respectively. There was a significant statistical difference between MPR and VC. The consistent rate of lesion length was 89.1%, 76.6%, 95.3% and 100.0%, respectively. There was a statistical difference between VC and SSD. The accuracy of discriminating pathology pattern was 81.3%, 92.2%, 71.9% and 71.9%, respectively. There was a statistical difference between VC and SSD. MPR could determine accurately the circumference of colorectal carcinoma, Raysum could determine the length of lesion more precisely than SSD, VC was helpful in discriminating pathology patterns. CONCLUSION: MPR, VC, SSD and Raysum have advantage and disadvantage in detection of colorectal carcinoma, use of these methods in combination can disclose the lesion more accurately.
MP43-18 DYNAMIC REAL-TIME MICROSCOPY OF BLADDER NEOPLASMS USING CONFOCAL LASER ENDOMICROSCOPY

Abstract:INTRODUCTION AND OBJECTIVES: Confocal laser endomicroscopy (CLE) enables dynamic imaging of the cellular structures below the mucosal surface and holds promise in providing real time optical diagnosis and grading of urothelial carcinoma. We presented our initial experience using CLE for the diagnosis of urothelial carcinoma of bladder. METHODS: 21 patients scheduled to undergo transurethral resection of bladder tumors were recruited. One urologist who had already participated in a computer-based training course performed all the CLEs. After standard white light cystoscopy (WLC), fluorescein was administered intravesically as a contrast dye. A 2.6 mm probe based CLE (Cellvizio, Mauna Kea Technologies) was passed through a 26Fr resectoscope to image normal and abnormal appearing areas. The images were collected with 488 nm excitation at 8 to 12 frames per second. The endomicroscopic images were compared with standard hematoxylin and eosin analysis of transurethral resection of bladder tumor specimens. RESULTS: Of the 21 recruited patients, 1 had no tumor but inflammation, 2 had carcinoma in situ ( CIS), 7 had low grade tumors, 10 had high grade tumors and 1 had a bladder liomyoma. The overall diagnostic accuracy is 80.9% (17/21) combining CLE+ WLC. 1 inflammation was regarded as CIS, 1 low grade tumor was regarded as high grade and 2 high grade tumors were regarded as low grade. CONCLUSIONS: CLE is an adoptable technology for novice CLE observers after a training session. CLE demonstrated clear differences between normal mucosa and malignant lesions. However, real-time differentiation between low and high grade tumors, inflammation and cancer, can still be challenging. Figure. No caption available. Table. No title available.
