Abstract:Objective To evaluate the diagnostic value of CT perfusion in breast cancer by clinical epidemiological method of ROC(receiver operating characteristic curve)analysis. Materials and Method 81 cases with breast mass found by health examination or mammography received MSCT(multislice spiral CT) perfusion, then hemodynamic parameters (blood flow,BF; mean transit time, MTT; blood volume, BV) were calculated by deconvolution arithmetic. According to the pathologic results, two groups of benign and malignant were classified, Statistical test was performed between them. Then ROC character of BF, MTT, BV was compared, and the diagnostic value of the hemodynamic parameters were confirmed. Result In the malignant group, BF was (0.735±0.440) ml·min-1·ml-1, MTT was (22.771±7.647)s, and BV was 0.234±0.082. While in the benign group, the BF was (0.466±0.527)ml·min-1·ml-1, MTT was (26.712±12.934)s, and BV was 0.179±0.117.As for BF and BV, there was significant difference between the two groups. When the hemodynamic parameters were used to discriminate the breast lesions, area under the ROC curve(AUCROC)value of BF was 0.832±0.086, it was the maximum. AUCROC value of BV was 0.695±0.092, there was no significant difference between them. AUCROC value of MTT was 0.473±0.102, it was minimum; as the threshold of BF was 0.381 ml·min-1·ml-1, the sensitivity was 82.3%, the specificity was 73.2%, the positive likelihood ratio was 3.071, the negative likelihood ratio was 0.242; as the threshold of BV was 0.190, the sensitivity was 73.3%, the specificity was 56.5%, the positive likelihood ratio was 1.685, the negative likelihood ratio was 0.473. Conclusion CT hemodynamic parameters (BF and BV) have certain diagnostic value in breast cancer, but BF or BV can not be single index to confirm or deny the diagnosis of breast cancer.
Quantification of CT perfusion curve shape and it's diagnosis of breast cancer: Computer simulation and breast perfusion

Abstract:Objective To assess the influence of hemodynamic parameters and parameter combinations on shapes of CT perfusion time density curve (TDC),and to seek a method of quantitative evaluation on TDC shapes,as well as to use it in prospective research of CT perfusion in breast cancer. Methods Programming Excel 2003 to automate convolution and deconvolution calculations,Weibull function was used as impulse residue function (R(t)). When fixing the other parameters,progressive value of blood flow (BF),mean transit time (MTT),BF to MTT ratio (BF/MTT),parameters were taken respectively to obtain a series of virtual TDC,by which influence of the above parameters on TDC shape was verified and parameters that could describe TDC shape quantitatively were screened out. Twenty-eight patients with breast tumors underwent CT perfusion scan,and the presentations were classified as washout,persistent and plateau types according to TDC shape. Taking postoperative pathologic diagnosis as the golden standard,the value of BF,MTT,BV,BF/MTT and TDC shape type in the diagnosis of breast cancer was analyzed with receiver operating characteristic curve (ROC). Results ①The shape parameter of Weibull function had little influence on TDC. BF affected the height of TDC:the larger the BF was,the higher the TDC was. MTT affected the height and peak time of TDC:the larger the MTT was,the higher the TDC and at the same time peak time was delayed. BF/MTT determined TDC shape,TDC presenting as a washout type when relatively large values were taken,and as a persistent type when relatively small values were taken. ②BF/MTT of breast tumor was correlated with the TDC shape type. ③The area under ROC of all parameters of breast perfusion decreased in sequence as:BF/MTT,BF,TDC shape type,blood volume (BV) and MTT. The three former were larger than the area under the diagonal line (P<0.05),and had significance in differential diagnosis of malignant and benign breast tumors. Conclusion BF/MTT can describe TDC shape of CT perfusion quantitatively. It integrates the diagnostic information of BF and MTT in a non-linear way,and is therefore of significant reference value in the diagnosis of breast cancer.
Value of CT Perfusion and Mammography in the Diagnosis of Breast Diseases

Abstract:Objective:To analyze the value of CT perfusion and mammography in the diagnosis of breast diseases.Methods:45 patients with pathology proved breast disease were examined by mammography and CT perfusion,the imaging results were analyzed and correlated with pathology.Results:The diagnostic accuracy of mammography was 77.28%,CT perfusion was 88.89% and the diagnostic accuracy of mammography in combination with CT perfusion was 95.56%,significant statistical differences were existed (P0.05).The mean value of blood flow (BF),blood volume (BV) and surface permeability (PS) of breast malignancy on CT perfusion were markedly higher than that of benignancy.CT perfusion was superior to mammography in the diagnosis of extent of lesion and existence of necrosis (P0.05). Mammography was superior to CT perfusion in the identification of micro-calcifications (P0.05).Conclusion:The diagnostic accuracy of breast carcinoma could be improved by mammography in combination with CT perfusion.
Correlation between quantitative perfusion histogram parameters of Dynamic Contrast-Enhanced MRI and tumor tis-sue microvessel density in patients with lung cancer

Abstract:Objective To investigate the correlation between quantitative perfusion histogram parameters of DCE-MRI and tumor tissue microvessel density(MVD) in patients with lung cancer.Methods 30 patients with lung cancer confirmed by pathology who underwent preoperative DCE-MRI were enrolled in this retrospective study .Quantitative perfusion histogram pa-rameters( including median, mean, skewness, kurtosis, energy, entropy) were measured for each patient using Exchange mo-dle.Using the Immunohistochemical method to detect the expression of CD34 in tumor tissue, and counting the number of mi-crovessels under microscope.SPSS 19.0 was used to carry out statistical analysis.The correlation between MVD and quantita-tive perfusion histogram parameters of DCE-MRI measured by exchange model was evaluated by Pearson linear correlation anal-ysis.Results There was no significant difference in MVD and each quantitative perfusion histogram parameters between the three different pathological groups of lung cancer(P >0.05).Ktrans perfusion histogram parameters(mean, 25％, 50％, 50％, 75％, 90％, 95％), Kep perfusion histogram parameters(entropy, 10％, 25％, 50％, 75％, 90％), Fp perfusion histogram parameters( mean, 25％, 50％, 75％, 90％, 95％) and Vp perfusion histograms parameters ( entropy, 75％, 90％, 95％ ) were positively correlated with MVD(P<0.05).Ktrans perfusion histogram parameters(energy) and Vp perfu-sion histogram parameters(skewness, kurtosis, energy) were negatively correlated with MVD(P<0.05).There was no signifi-cant correlation between Ve perfusion histogram parameters and MVD(P>0.05).Conclusion There was a certain correla-tion between the perfusion histogram parameters of DCE-MRI and MVD,suggesting that the quantitative perfusion histogram of DCE-MRI in lung cancer can reflect the MVD value of cancer tissue .
Tumor region of interest selection in CT colorectal cancer perfusion imaging using 256-slice CT:a preliminary study

Abstract:Objective:To prospectively determine whether position and size of tumor region of interest(ROI)influence estimate of colorectal cancer vascular parameters at computed tomography(CT).Methods:After institutional review board approval and informed consent,13 patients with colorectal cancer underwent CT perfusion study.Blood volume and blood flow were determined for 30mm2 or 90mm2 circular ROIs placed at the tumor edge and center and around(outlining)visible tumor.ROI analysis was repeated by two observers in different subsets of patients to assess intra-and interobserver variation.Measurements were compared by using analysis of variances.Results:Blood volume,blood flow measurements were substantially higher at the edge than at the center for both 30mm2 and 90mm2 ROIs.For outlined tumor ROI,blood volume was 30.28ml/100g·min and blood flow was 19.65ml/100g.Measurements varied substantially depending on the ROI size.Values for the ROI for outlined tumor were intermediate between those at the tumor edge and center(P<0.05).Conclusion:Position and size of tumor ROI and observer variation substantially influenced ultimate perfusion values.ROI for outlined entire tumor is more reliable for perfusion measurements and more appropriate clinically than use of arbitrarily determined smaller ROIs.
Dynamic Contrast-Enhanced Computed Tomography Functional Imaging for the Evaluation of Tumor Angiogenesis in Patients with Lung Carcinoma

Abstract:Objective To investigate the role of dynamic contrast-enhanced computed tomography(DCE-CT) in the evaluation of tumor angiogenesis in patients with lung carcinoma. Methods 34 patients with pathology-proved pulmonary carcinoma underwent DCE-CT scan before operation. The functional imaging parameters: peak height(PH), the time of peak height (Tp), the ratio of PH of the mass to aorta(M/A), perfusion, the relative blood volume (rBV) and permeability (Pm) were calculated and correlated with the microvessel density (MVD) of the carcinoma. The functional imaging parameters MVD and the lymph node involvement between VEGF positive group and VEGF negative group were analyzed. Results The Perfusion, PH, M/A and rBV correlated positively with MVD (r= 0.76, 0.67, 0.70, 0.65 respectively;P0.0001), and no significant correlation was found between Pm and MVD (r= 0.30, P0.05). The functional imaging parameters and MVD of VEGF positive group were higher than those of VEGF negative group (P0.05)and the difference of lymph node involvement between two groups was significant(χ2= 9.409,P0.05). Conclusion DCE-CT functional imaging technique provides general information of lung carcinoma angiogenesis and alternates non-invasive option for evaluation of tumor angiogenesis in patients with lung carcinoma.
Quantitative Assessment of Lung Cancer Tumor Vascular Parameters by Using High Speed Perfusion CT: Influence of Tumor Region of Interest

Abstract:Objective To prospectively determine whether position and size of tumor region of interest (ROI) influence estimates of lung cancer vascular parameters at 256-slice CT perfusion study.Methods After institutional review board approval and informed consent,16 men and 11 women with lung cancer underwent 70-second CT perfusion study. Perfusion, blood volume, peak enhancement weredetermined for 60 or 120 mm2 circular ROIs placed at the tumor edge and center and around (outlining) visible tumor. ROI analysis was repeated twice by two observers in the same way to get the average values.The correlation between the measurements and microvascular density counts was determined in 19 patients with operation.Measurements were compared by using Pair-Samples t test,a difference with P≤0.05 was significant.Results Perfusion,blood volume,peak enhancement measurements were substantially higher at the edge than at the center for both 60 and 120 mm2 ROI. For 60 and 120 mm2 ROI,median of the three measurements were (M=15.40,9.50 ml · min- 1 · 100 ml - 1,M =35.60,25.40 ml · 100 g-1,and M=16.47,10.20 HU),respectively,at the edge versus(M =14.80,8.52 ml · min- 1 · 100 ml- 1,M =33.50,23.90 ml · 100 g- 1,and M =15.64,9.40 HU),respectively,at the center(t value was 10.95,9.80,7.06,2.93,7.74 and 10.09,respectively,all P ＜ 0.05 ). Measurements varied substantially depending on the ROI size,values for the ROI for outlined tumor were intermediate between those at the tumor edge and center.Perfusion(M=12.51 ml.min- 1 · 100 ml-1),blood volume(M=29.31 ml.100 g-1),peakenhancement (M =12.93 HU) for the ROI outlining tumor were intermediate between those at the tumor edge and center.There was a good correlation between perfusion,blood volume,peak enhancement for the ROI outlining tumor and microvascular density[ ( 19.43 ± 8.78 ) vessels/0.74 mm2 ],respectively ( r value 0.732,0.590 and 0.544,all P ＜ 0.05 ).Conclusions Position and size of tumor ROI substantially influence ultimate perfusion values.ROI for outlined entire tumor is more reliable for perfusion measurements and more appropriate clinically than use of arbitrarily determined smaller ROI.

Abstract:Objective To evaluate the correlation between regional blood perfusion and biological features of breast cancer. Methods Spiral CT technique was applied to quantitatively detect the central and marginal blood perfusion, including blood flow ( BF ) , blood volume ( BV) and permeability of surface (PS). Results The central and marginal blood perfusion of breast cancer were significantly higher than that of normal breast tissues. The marginal blood perfusion was higher than central blood perfusion. The regional blood perfusion of breast cancer varied with tumor size, clinical stage and histological grading. Conclusion The regional blood perfusion correlates with biological markers in breast cancer and can be used to evaluate the biological characteristics as a noninvasive marker before neoadjuvant chemotherapy.
Quantitative analysis of contrast-enhanced ultrasonography of benign and malignant breast tumors

Abstract:Objective To study the perfusion curve characteristics and the clinical value of contrast -enhanced ultrasound (CEUS) in benign and malignant breast tumors. Methods The CEUS images of 87 patients with breast tumors were analyzed by the off-line Sonolver software,the perfusion curve parameters of analysis area and reference area were received and and quantitatively analyzed. The results were compare with those of pathology. the characteristics of CEUS and perfusion curve of breast benign and malignant tumors were analyzed. Results Compared with benign breast tumors, rise time (RT), time to peak (TTP),mean trasit time( mTT) of malignant breast tumors were significantly shorter and rise slope( RS), descending slope(DS) were increased in analysis area (P<0.05). The 95% CI of RT,TTP,mTT were significantly decreased in analysis area of malignant tumors (P<0.05). RT was shorter, RS and DS were increased in reference area of malignant tumors (P<0.05). For benign and malignant tumors,The RT, TTP and mTT in analysis area were shorter than those in reference area. the IMAX, RS, DS, QOF of analysis area were higher than those of reference area, there was significant difference (P<0.05). Conclusion Perfusion parameters of breast tumor evaluated by CEUS can assess blood perfusion within the tumor, it provides a new quantitative indicator for diagnosis of breast benign and malignant tumors.

Abstract:Background: To investigate value of the quantitative perfusion parameters of dynamic triple-phase enhanced CT in differential diagnosis of pulmonary lesions, and explore the correlation between perfusion parameters of lung cancer with microvessel density (MVD) and vascular endothelial growth factor (VEGF). Methods: 73 consecutive patients with lung lesions who successfully underwent pre-operative CT perfusion examination with dynamic triple-phase enhanced CT and received a final diagnosis by postoperative pathology or a clinical follow-up. The cases were divided into malignant and benign groups according to the pathological results. CT perfusion parameters, such as Median, Mean, Standard deviation (Std), Q10, Q25, Q50, Q75, Q90 of pulmonary artery perfusion (PAP), bronchial artery perfusion (BAP), perfusion index (PI) and arterial enhancement fraction (AEF) were obtained by performing computed tomography perfusion imaging (CTPI). Computed tomography perfusion (CTP) parameters were compared between malignant and benign lesions. The receiver operating characteristic (ROC) curve was used to assess the diagnostic efficiency of CTP parameters in diagnosing malignant lesions. The correlations between CTP parameters with MVD and VEGF were analysed in 36 lung cancer patients who had extra sections be used for immunohistochemistry staining of CD34 and VEGF. Results: BAP (Mean, Std, Q90) and PI Std of benign lesions were higher than malignant lesions (p < 0.05), and PAP (Q10, Q25), PI (Median, Mean, Q10, Q25, Q50) of malignant lesions were higher than the benign (p < 0.05). The area under the ROC curve of PI Mean, PI Q10 and PI Std was 0.722 (95% CI = [0.595-0.845]), 0.728 (95% CI = [0.612-0.844]) and 0.717 (95% CI = [0.598-0.835]) respectively. Partial perfusion parameters of BAP and AEF Q10 were positively correlated with MVD (p value range is < 0.001-0.037, ρ value range is 0.483-0.683), and partial perfusion parameters of PI were negatively correlated with MVD (p value range is 0.001-0.041,ρvalue range is - 0.523-- 0.343). Partial perfusion parameters of BAP and AEF Q10 were positively correlated with VEGF (p value range is 0.001-0.016, ρvalue range is 0.398-0.570), meanwhile some perfusion parameters of PAP and PI were negatively correlated with VEGF (p value range is 0.001-0.040, ρ value range is - 0.657-0.343). Conclusions: Quantitative parameters of dynamic triple-phase enhanced CT can provide diagnostic basis for the differentiation of lung lesions, and there were connection with tumor angiogenesis and vascular endothelial growth factor expression.
MSCT Perfusion Imaging in Diagnosis of Breast Cancer

Abstract:Objective To study the characteristics of MSCT perfusion imaging(MSCTPI) in breast cancer,breast benign tumors and corresponding non-tumor normal tissues.Methods MSCTPI was performed on 38 patients with breast tumors.Chi-square tests and independent samples t-test were applied to compare the TDC types and perfusion parameters' values of breast cancer,breast benign tumors and corresponding non-tumor breast tissues.Perfusion parameters included maximum intensity projection(MIP),average value(AV),blood flow(BF),blood volume(BV),time to start(TTS),time to peak(TTP),permeability surface(PS) and Patlak blood volume(PBV).Results TDC of corresponding non-tumor breast tissues were all flat-type.Among outflow-type and platform-type TDC,76.47% and 71.43% were breast cancer,while 85.72% breast tumors with inflow-type TDC were benign.The means of BF,BV,PS and PBV in breast cancer were higher than those of benign breast tumor and corresponding non-tumor normal tissues.The means of MIP,AV,BF,BV and PBV of center zone were lower than those of boundary zone in breast cancer focus ≥2.0 cm,while the means of MIP,AV,BF,BV and PBV of center zoon were higher than those of boundary zone in benign breast tumors.Conclusion Breast cancer has special MSCTPI characteristics that being helpful to the diagnosis and differential diagnosis of breast cancer.
Value of perfusion parameters and histogram analysis of triphasic computed tomography in pre-operative prediction of histological grade of hepatocellular carcinoma

Abstract:<span>Background: Pre-operative non-invasive histological evaluation of hepatocellular carcinoma (HCC) remains a challenge. Tumor perfusion is significantly associated with the development and aggressiveness of HCC. The purpose of the study was to evaluate the clinical value of quantitative liver perfusion parameters and corresponding histogram parameters derived from traditional triphasic enhanced computed tomography (CT) scans in predicting histological grade of HCC.Methods: Totally, 52 patients with HCC were enrolled in this retrospective study and underwent triple-phase enhanced CT imaging. The blood perfusion parameters were derived from triple-phase CT scans. The relationship of liver perfusion parameters and corresponding histogram parameters with the histological grade of HCC was analyzed. Receiver operating characteristic (ROC) curve analysis was used to determine the optimal ability of the parameters to predict the tumor histological grade.Results: The variance of arterial enhancement fraction (AEF) was significantly higher in HCCs without poorly differentiated components (NP-HCCs) than in HCCs with poorly differentiated components (P-HCCs). The difference in hepatic blood flow (HF) between total tumor and total liver flow (ΔHF = HFtumor − HFliver) and relative flow (rHF = ΔHF/HFliver) were significantly higher in NP-HCCs than in P-HCCs. The difference in portal vein blood supply perfusion (PVP) between tumor and liver tissue (ΔPVP) and the ΔPVP/liver PVP ratio (rPVP) were significantly higher in patients with NP-HCCs than in patients with P-HCCs. The area under ROC (AUC) of ΔPVP and rPVP were both 0.697 with a high sensitivity of 84.2% and specificity of only 56.2%. The ΔHF and rHF had a higher specificity of 87.5% with an AUC of 0.681 and 0.673, respectively. The combination of rHF and rPVP showed the highest AUC of 0.732 with a sensitivity of 57.9% and specificity of 93.8%. The combined parameter of ΔHF and rPVP, rHF and rPVP had the highest positive predictive value of 0.903, and that of rPVP and ΔPVP had the highest negative predictive value of 0.781.Conclusion: Liver perfusion parameters and corresponding histogram parameters (including ΔHF, rHF, ΔPVP, rPVP, and AEFvariance) in patients with HCC derived from traditional triphasic CT scans may be helpful to non-invasively and pre-operatively predict the degree of the differentiation of HCC.</span>

A study on CT radiomics approach to predict outcomes of simultaneously pulmonary nodules in breast cancer patients after treatment

Abstract:Objective:To evaluate the feasibility of CT radiomics method in predicting outcomes of simultaneous pulmonary nodules in breast cancer patients after treatment.Methods:Patients with breast cancer confirmed by pathology and with simultaneous pulmonary nodules (diameter>5 mm, number≤5) detected by preoperative CT were retrospectively enrolled in this study. Eighty female patients were included (median age: 52, quartile range: 45, 61). The pulmonary nodules (median size: 6.0 mm, quartile range: 5.5, 7.2 mm) were classified into stable group (without change over 2 years) and change group according to follow-up CT findings. The change group was further divided into improved group and progressive group. Eventually, 54 cases were in the stable group, 26 cases were in the change group. One hundred and five texture features were extracted using the python-based pyradiomics package based on preoperative CT images. Stepwise regression was used to exclude features without significant difference in predicting changes of pulmonary nodules. Classifiers model and 5 fold cross validation method were used to obtain the highest performance in predicting outcomes of pulmonary nodules. Receiver operating characteristic (ROC) curve was performed to evaluate the diagnostic performance of the model.Results:After features exclusion and selection, three radiomics features were used to establish classifiers between stable group and change group. It was showed that the linear discriminate analysis was the optimal model with the specificity, sensitivity, accuracy and area under the ROC curve (AUC) as 0.980, 0.460, 0.813 and 0.770 respectively. One radiomics feature was chosen to establish classifiers between improved group and progressive group. The coarse gaussian support vector machine (CGSVM) was the optimal model, with the specificity, sensitivity, accuracy and AUC as 0.540, 0.920, 0.713 and 0.880 respectively.Conclusions:CT radiomics analysis has the potential to predict the outcomes of simultaneous indeterminate pulmonary nodules in breast cancer patients after treatment, and it may contribute to preoperative treatment and postoperative follow-up planning.

Abstract:Objective To discuss the difference of CT perfusion imaging of clear cell renal cell carcinoma in different groups.Materials and Methods CT perfusion scans were performed through the central slices of the tumor in 40 cases with pathologically confirmed clear cell renal cell carcinoma.The software of deconvolution calculated a series of perfusion parameters of regions of interest(ROIs)in tumor and cortexes of both kidney,including blood flow(BF),blood volume(BV),mean transit time(MTT),permeability surface(PS)and the time-density curve(TDC curve).The 40 cases with clear cell renal cell carcinoma were differentiated by neovascularization,affect on renal calices and cancer embolus,and the perfusion parameters were analyzed statistically.Results PS of tumor with visible neovascularization was lower than that of tumor without visible neovascularization(P0.05);BF of tumor side renal cortex of tumor with pressed renal calices is higher than that of tumor without affect on renal calices(P0.01);Blood volume of both renal cortex and MAX ROI(the area in tumor with maximal perfusion)of tumor with cancer embolus is lower than that of tumor without cancer embolus(P0.05).Conclusion CT perfusion imaging is valuable for evaluation of prognosis of clear cell renal cell carcinoma.

Abstract:Background: Numerous studies have explored diagnosis of pulmonary nodules using perfusion computed tomography (CT); however, findings were not always consistent between studies. The present study aimed to summarize evidence on the diagnostic value of perfusion CT for distinguishing between lung cancer and benign lesions.Material/Methods: We performed a systematic literature search on lung cancer and benign pulmonary lesions performed with perfusion CT. The searches were undertaken in English or Chinese language in Medline, PubMed, Embase, Cochrane Library, Web of Science, and China National Knowledge Infrastructure database from Jan 2010 to Nov 2018. Standardized mean differences (SMDs) and 95% confidence intervals (CIs) of blood volume (BV), blood flow (BF), mean transit time (MTT), and permeability surface (PS) were calculated using Review Manager 5.3. Publication bias, sensitivity, specificity, and the area under the curve (AUC) were calculated using Stata12.0.Results: Fourteen studies comprising 1032 malignant and 447 benign pulmonary lesions were analyzed. Lung cancer had higher BV, BF, MTT, and PS values than benign lesions. SMDs and 95% CIs of BV, BF, MTT, and PS were 2.29 (1.43, 3.16), 0.50 (0.14, 0.86), 0.55 (0.39, 0.72), and 1.21 (0.87, 1.56), respectively. AUC values of BV and PS were 0.92 (0.90, 0.94) and 0.83 (0.80, 0.86), respectively.Conclusions: CT perfusion imaging is a valuable technique for the diagnosis of pulmonary nodules. Lung cancer had higher perfusion and permeability than benign lesions. The evidence suggests blood volume is the best surrogate marker for characterizing the blood supply, while permeability surface has a high specificity in quantifying the vascular permeability.

Abstract:Objective To investigate th e hemodynamic features of different cytopathological types of lung malignancies on multislice spiral CT perfusion imaging.Methods Forty-five patients with histologically proven lung cancers were involved in this study.A conventional thoracic CT was obtained as a routine to locate the lung lesions.Fifty milliliters of nonionic contrast(300 mgI/ml) were injected at 6 ml/s,then,followed by dynamic CT scans at the same location.Data postprocessing was performed on Siemens workstation using lung cancer analyzing software in the package of Body Perfusion.The perfusion measurements were calculated for each dynamic scan.The perfusion values of different types of lung malignancies were then compared using analysis of variance.Results The time to start perfusion of lung adenocarcinomas was later than the time of squamous cell carcinomas and small cell undifferentiated carcinomas.The time to peak perfusion of squamous cell carcinomas was earlier than lung adenocarcinomas and small cell undifferentiated carcinomas.No significant differences were noted on other perfusion values among different cytopathological types of lung malignancies.Conclusion The Significant differences were noted on the values of time to start and time to peak perfusion among different cytopathological types of lung malignancies,but further investigation is necessary for their significance of prediction the cytopathology of lung malignancies.No significant differences were noted on other perfusion values among different types of lung malignancies.They may be of little help on differentiation of different cytopathological types of lung malignancies.

Abstract:PURPOSE We aimed to examine the usefulness of utilizing a specific contrast-enhanced computed tomog raphy (CT) region of interest (ROI) to differentiate renal oncocytoma (RO) from small clear cell renal cell carcinoma (ccRCC) and chromophobe renal cell carcinoma (chRCC). METHODS A retrospective analysis of pre-contrast phase (PCP), corticomedullary phase (CMP), and nephro graphic phase (NP) contrast-enhanced CT images of the histopathologically confirmed initial cohort (27 ROs, 74 ccRCCs, and 36 chRCCs) was conducted. Small, medium, large, and whole ROIs (S-ROI, M-ROI, L-ROI, and W-ROI, respectively) were utilized for CT attenuation value of tumor (AVT), lesion-to-cortex attenuation (L/C), and heterogeneous degree of tumor (HDT) calcula tions. Differences in these parameters were then compared between RO and ccRCC/chRCC, with receiver operating characteristic (ROC) curves being utilized to gauge the diagnostic utility of the statistically significant parameters. Logistic regression analyses were employed to identify key factors capable of differentiating RO and ccRCC/chRCC, with predictive models further being established. A validation cohort (6 ROs, 30 ccRCCs, and 12 chRCCs) was then employed to vali date the performance of the predictive models. RESULTS Of the parameters evaluated using different ROIs, L/C-CMP (S-ROI) (0.88 ± 0.15 vs. 1.13 ± 0.25, P < .001) and HDT-CMP (W-ROI) (23.02 (12.00-51.21) vs. 37.81 (16.09-89.45), P < .001) were best suited to differentiating RO and ccRCC, yielding respective area under the curve (AUC) values of 0.803 and 0.834. AVT-NP (S-ROI) (122.85 ± 18.87 vs. 86.50 ± 18.65, P < .001) and AVT-NP (M-ROI) (119 (86-167) vs. 81.5 (53-142), P < .001) were better able to differentiate RO and chRCC, yielding respective AUC values of 0.918 and 0.906. Logistic regression analyses revealed that L/C-CMP (S-ROI) and HDT-PCP, as well as AVT-NP (S-ROI) and HDT-CMP, were the primary factors capable of differentiating RO from ccRCC and chRCC, respectively. The predictive model developed to dif ferentiate between RO and ccRCC exhibited a sensitivity of 66.67% and 55.14% in the initial and validation cohorts, respectively, with corresponding specificity of 94.59% and 93.55%, accuracy of 87.13% and 86.84%, and AUC of 0.908 and 0.876. The predictive model developed to differ entiate between RO and chRCC exhibited a sensitivity of 85.19% and 100.00% in the initial and validation cohorts, respectively, with corresponding specificity of 94.59% and 92.86%, accuracy of 87.30% and 95.24%, and AUC of 0.944 and 0.959. CONCLUSION These data demonstrate that a combination of quantitative parameters measured with particu lar ROIs can enable the efficient and reliable differentiation of RO from ccRCC and chRCC for use in routine patient differential diagnosis.

Abstract:Purpose:To study The difference of CT perfusion imaging of renal clear cell carcinoma at difference stage.Materials and Methods:CT perfusion scans by GE lightspeed 16 were performed through the central slices of the tumor in 40 pathologically confirmed renal clear cell carcinoma.The software of deconvolution calculated a series of perfusion parameters of regions of interest(ROIs) in tu- mor and cortexes of both kidney,including blood flow(BF),blood volume(BV),mean transit time (MTT),permeability surface(PS) and the time-density curve(TDC curve).The 40 cases of clear cell carcinoma were differentiated by stage and the perfusion parameters was analyzed statistically.Results: PS of MAX-ROI(the area in tumor with maximal perfusion) in early stage tumor is higher than that in medium-late stage tumor(P=0.027).MTT of MAX-ROI in early stage tumor is shorter than that in medium-late stage tumor(P=0.039).The effect on perfusion of both renal cortex is more obviously in medium-late stage tumor than that in early stage tumor.Conclusion:CT perfusion imaging is valuable for stage of renal clear cell carcinoma.

Abstract:Angiogenesis is an important feature of malignant neoplasm and is the fundamental of genesis and growth of tumors. CT perfusion imaging (CTPI), as one of the functional imaging techniques, can detect neovascularity of neoplasm. It has been applied in a range of clinical areas, such as lesion characterization, staging, predicting patient outcome or response to therapy, assessment of response to different therapies, and so on. However, the major limitations of CTPI are the risk of exposure to ionizing radiation and the dispute of reproducibility of parameters which limit its routine use. Low-dose CT perfusion can be better used in clinical work. In this article we reviewed the basic principles, technique, and various oncological applications of Low-dose CT perfusion.
