Abstract:SummaryThis paper discusses the progress and prospects of China's complex health care reform beginning in 2009. The Chinese government's undertaking of systemic reform has achieved laudable achievements, including the expansion of social health insurance, the reform of public hospitals, and the strengthening of primary care. An innovative policy tool in China, policy experimentation under hierarchy, played an important role in facilitating these achievements. However, China still faces gaps and challenges in creating a single payer system, restructuring the public hospitals, and establishing an integrated delivery system. Recently, China issued the 13th 5‐year plan for medical reform, setting forth the goals, policy priorities, and strategies for health reform in the following 5 years. Moreover, the Chinese government announced the “Healthy China 2030” blueprint in October 2016, which has the goals of providing universal health security for all citizens by 2030. By examining these policy priorities against the existing gaps and challenges, we conclude that China's health care reform is heading in the right direction. To effectively implement these policies, we recommend that China should take advantage of policy experimentation to mobilize bottom‐up initiatives and encourage innovations.

Abstract:The paper reviews the results,experience and issues in five key reform areas in China.The main policy recommendations have been proposed.It is necessary to establish a unified administration system for coordinating medical insurance system in urban and rural residents,build up selective criteria in essential medicine list at provincial level,strengthen grassroots health facilities to access primary health care more equitable,efficient and of high quality,improve the level of equality in public health service,and,finally,exploit the pathway of public hospital reform through changing reimbursement and payment system.

Abstract:China's 3 year, CN¥850 billion (US$125 billion) reform plan, launched in 2009, marked the first phase towards achieving comprehensive universal health coverage by 2020. The government's undertaking of systemic reform and its affirmation of its role in financing health care together with priorities for prevention, primary care, and redistribution of finance and human resources to poor regions are positive developments. Accomplishing nearly universal insurance coverage in such a short time is commendable. However, transformation of money and insurance coverage into cost-effective services is difficult when delivery of health care is hindered by waste, inefficiencies, poor quality of services, and scarcity and maldistribution of the qualified workforce. China must reform its incentive structures for providers, improve governance of public hospitals, and institute a stronger regulatory system, but these changes have been slowed by opposition from stakeholders and lack of implementation capacity. The pace of reform should be moderated to allow service providers to develop absorptive capacity. Independent, outcome-based monitoring and evaluation by a third-party are essential for mid-course correction of the plans and to make officials and providers accountable.

Abstract:<h2 class="section-title u-h3 u-margin-l-top u-margin-xs-bottom">Summary</h2><p>In 2009, China launched a major health-care reform and pledged to provide all citizens with equal access to basic health care with reasonable quality and financial risk protection. The government has since quadrupled its funding for health. The reform's first phase (2009–11) emphasised expanding social health insurance coverage for all and strengthening infrastructure. The second phase (2012 onwards) prioritised reforming its health-care delivery system through: (1) systemic reform of public hospitals by removing mark-up for drug sales, adjusting fee schedules, and reforming provider payment and governance structures; and (2) overhaul of its hospital-centric and treatment-based delivery system. In the past 10 years, China has made substantial progress in improving equal access to care and enhancing financial protection, especially for people of a lower socioeconomic status. However, gaps remain in quality of care, control of non-communicable diseases (NCDs), efficiency in delivery, control of health expenditures, and public satisfaction. To meet the needs of China's ageing population that is facing an increased NCD burden, we recommend leveraging strategic purchasing, information technology, and local pilots to build a primary health-care (PHC)-based integrated delivery system by aligning the incentives and governance of hospitals and PHC systems, improving the quality of PHC providers, and educating the public on the value of prevention and health maintenance.</p>

Abstract:The Chinese health reform did make some significant achievements since 2009. However, affordability and accessibility to primary care and public health remain to be serious concerns. Health governance reforms are necessary to address affordability and accessibility issues in the next stage of health reform. In 2009, China initiated a new round of health reform to establish a well‐functioning health system. The 2009 health reform did make some significant achievements in improving affordability and accessibility. In particular, social health insurance has been expanded significantly and various social health insurance plans have covered over 95% of total population in China by 2011. The Chinese government also has significantly increased fiscal input for health financing. However, affordability remains a serious concern as the social health insurers, as purchasers, were not very effective in containing the growth of health expenditure. Accessibility to primary care and public health is another concern. In this context, health governance reforms are necessary to address affordability and accessibility issues. Health governance set rules for key actors of the health system (including service providers, health insurers and government departments) by taking into account the strategies and incentives of these actors in their interactions. In recent years, a series of reforms in health governance have been initiated. Some progresses have been achieved. For the next stage of health reform, issues including how to further reform the governance structure of medical institutions and how to improve resource allocation in the health system are critical.

Abstract:Objective:To find out the concrete progress of the health planning through interim evaluation of“Twelfth Five-year Plan”of health in Shanghai. The results of the evaluation would provide scientific empirical basis for the planning in the next stage. Methods:Self-evaluation of district(county),health bureaus(commissions of health and family planning)and affiliated institutions was conducted. Field research was organized by Shanghai Municipal Commission of Health and Family Planning and third party self-evaluation was conducted. Results:The interim completion of indicators of“Twelfth Five-year Plan”of health in Shanghai showed that the planning achieved remarkable results. The planning basically reached the goal that more than half of the time passed and more than half of the tasks had been completed. But there were still problems in resource allocation,human resources and emergency medical services. Conclusion:The principal contradiction of imbalance between delivery of health services and residents’ demand needs to be focused and resolved. The matched policies need to be improved and appropriate solutions need to be developed to ensure the smooth implementation of the health planning.

Abstract:Objective:To analyze the present situation and main problems of Chinese health resources and put forward the related countermeasure suggestions for references for the Chinese health reform and development during 13th Five-Year Plan. Methods:Data analysis and literature study were conducted. Results:During the health system reform of 2009-2013,the hospital health technical personnel in mainland China increased year by year. Health and technical personnel in hospitals in 2013 accounted for more than 60%of the national total health technical personnel. More than 65%of the government health expenditure was for disease treatment,and less than 15%was used for disease prevention. Compared with data of 2008,the two-week prevalence rate of the residents increased by 5.2 percentage points in 2013,and the prevalence rate of chronic diseases among the population aged 15 and above increased by nine percentage points. Compared with data of 2009,the annual visits increased by 1.82 billion,yearly discharged patients increased by 59.65 million in 2013. Conclusion:Since the initiation of health system reform,China has input a lot of health resources into hospitals where treatment is given the priority,which has improved the residents’ health service utilization to some extent,but the residents’ two-week prevalence rate and chronic disease prevalence rate have also increased rapidly. It is recommended that transferring health care service model from treatment oriented to prevention oriented be the priority measure during 13th Five-Year Plan.

Abstract:Summaries the opinions of hospital directors who have attended the training course in the UK-China Partners in Health Innovation Programme organized by China Ministry of Health on hospital reform,including aspects of objectives of the reform,health system,healthcare financing,hospital payment and health service delivery in order to provide information for relevant health decision makers.The hospital reform not only includes the reform on hospital internal system and management,but also includes aspects related to whole social system reform.Thus,all relevant partners should work together.It should be the one of government's main tasks,not the sole task of health sectors.The governments should take main roles in the internal market resulting in a purchaser/provider split.

Abstract:China experienced both economic and epistemological transitions within the past few decades, greatly increasing demand for accessible and affordable health care. These shifts put significant pressure on the existing outdated, highly centralized bureaucratic system. Adjusting to growing demands, the government has pursued a new round of health reforms since the late 2000s; the main goals are to reform health care financing, essential drug policies, and public hospitals. Health care financing reform led to universal basic medical insurance, whereas the public hospital reform required more complex measures ranging from changes in regulatory, operational, and service delivery settings to personnel management. This article reviews these major policy changes and the literature-based evidence of the effects of reforms on cost, access, and quality of care. It then highlights the outlook for future reforms. We argue that a better understanding of the unintended consequences of reform policies and of how practitioners’ and patients’ interests can be better aligned is essential for reforms to succeed.

Abstract:It is now about 15 years since the introduction of the market into health care in China. This produced fundamental changes in the way that health care is financed and resulted in the disappearance of universal free basic health care. Responsibility for provision of health services has been devolved to the provincial and county governments, and healthcare providers have been given considerable financial independence. A fee for service system has been introduced, and several different payment mechanisms are now in operation. The new financing and pricing structures are responsible for greater inequity of access to services and more inefficient use of resources. These problems are widely acknowledged, and a range of solutions is being developed and tested. Since the introduction of the reforms the measurable health status of the population has not declined, probably as a result of overall improved socioeconomic conditions and a continued emphasis on prevention.

Abstract:The year 2006 was a major turning point in the development of China's healthcare system, as the government has pledged to take major responsibility in the health sector. This adjustment is based upon progressions in the Chinese reform's guiding principles, using conclusions from more than 20 years' experience in economic reform. It is a reform that is needed to solve a series of social problems, and it signifies a new stage of economic development for China. The issues that were debated in 2006 delved broader and deeper into this topic. Some of the issues covered were the Suqian healthcare reform, models of a future healthcare system and the feasibility of an Universal Healthcare System. These discussions have increased cogitation and consensus on the healthcare reform, and provided counsel for the reform. At the same time, affordable hospitals, community healthcare services and new Collective Medical Service(CMS) in rural areas are providing a foundation from which the reform will expand.

Abstract:After the launch of the historical Reform of the Medical and Health Care System in 2009, China's primary health care (PHC) system has witnessed significant progress and breakthroughs. In this review, we summarized published literature and official policies and synthesized data from the electronic registration information system of the National Health Commission, national statistical reports, or yearbooks in health care. The review describes the structural design and major achievements of PHC in China in the last decade, emphasizing the national policy foundation, budgeting and insurance coverage, evolution of service delivery, professional team development, and the advancement of supporting technologies. Over 40 years ago, primary health care (PHC) was defined in the Alma‐Ata Declaration as a critical component of the health care system to address the basic health demand of the people. In China, the Government attaches great importance to health care at the primary level. After the launch of the historical Reform of the Medical and Health Care System in 2009, the PHC system in China has witnessed major progress and breakthroughs, especially in its steadily increased capacity, continuously improved accessibility, and betterment in equality. In this review, we summarized published literatures and official policies, synthesized data from the electronic registration information system of the National Health Commission, national statistical reports, and yearbooks in health care. The review is intended to describe the systematic development of PHC in China in the last decade. The main results include: the solid national policy foundation, increasing number of PHC institutions and workforce, better training of PHC professionals, major achievements in primary health indicators, government financial support to PHC institutions, improved PHC budgeting and insurance coverage, and the advancement of supporting technologies. Challenges and prospects are also discussed.
