Abstract:Dementia is a neurological disease characterized by deterioration of cognitive, behavioral, social and emotional functions. [ 1 ] Clinically, it is characterized by memory loss, accompanied by personality changes of different degrees. With the continuous development of science and technology and society, the global population aging trend is increasing, dementia has become the fourth leading cause of death after cardiovascular diseases, tumors, stroke and so on. [ 2 ] According to statistics, about 46.8 million people worldwide suffered from dementia in 2015, and the number of dementia patients is expected to reach 135 million by 2050. [ 3 ] Dementia mainly includes Alzheimer's disease (AD) and vascular dementia (VD). AD will lead to memory loss, thinking disorders and decreased ability of daily activities in the elderly, [ 4 ] and also cause significant adverse impact on the patient's family, caregivers and healthcare system. [ 5 ] VD is a clinical dementia caused by brain tissue damage caused by cerebrovascular disease factors. With the incidence of cerebrovascular disease increasing year by year, the incidence of VD also increases year by year. [ 6 ] Given the absence of an effective treatment for this condition, the significance of dementia prevention is paramount. Identifying modifiable risk factors becomes imperative in the quest to proactively address and stave off the onset of dementia. [ 7 ]

Abstract:Recently, research into the link between thyroid dysfunction and Alzheimer's disease (AD) remains a current topic of interest. Previous research has primarily concentrated on examining the impact of thyroid dysfunction on the risk of developing AD, or solely explored the mechanisms of interaction between hypothyroidism and AD, a comprehensive analysis of the mechanisms linking thyroid dysfunction, including hyperthyroidism and hypothyroidism, to Alzheimer's disease (AD) still require further elucidation. Therefore, the aim of this review is to offer a thorough and comprehensive explanation of the potential mechanisms underlying the causal relationship between thyroid dysfunction and AD, highlighting the existence of a vicious circle. The effect of thyroid dysfunction on AD includes neuron death, impaired synaptic plasticity and memory, misfolded protein deposition, oxidative stress, and diffuse and global neurochemical disturbances. Conversely, AD can also contribute to thyroid dysfunction by affecting the stress repair response and disrupting pathways involved in thyroid hormone (TH) production, transport, and activation. Furthermore, this review briefly discusses the role and significance of utilizing the thyroid as a therapeutic target for cognitive recovery in AD. By exploring potential mechanisms and therapeutic avenues, this research contributes to our understanding and management of this devastating neurodegenerative disease.

Thyroid Function and Cognitive Decline: A Narrative Review

Abstract:Objective: As the population of older adults in the United States continues to rise, understanding modifiable risk factors that contribute to cognitive decline and dementia becomes increasingly important. This narrative review summarizes existing literature on the association between thyroid function in the euthyroid range, hypothyroidism and hyperthyroidism, and cognitive outcomes in older adults. Methods: A comprehensive literature search of the PubMed and Ovid/Medline databases was conducted. Randomized controlled trials, systematic reviews, meta-analyses, and observational studies published in English between January 2000 and December 2023 were included. Results: Overall, existing studies yielded conflicting results, failing to delineate a concrete relationship between thyroid function and cognitive outcomes and/or dementia in older adults. There may be a possible association between higher thyroid stimulating hormone in the reference range and lower risk of incident dementia, which may be more pronounced in women. Majority of studies elucidated a possible association between low thyroid stimulating hormone and incident dementia, with suggestion that duration of hyperthyroidism may contribute to increasing dementia risk. Even though evidence on the association of hypothyroidism and cognitive decline are disparate, current data do not support treatment of subclinical hypothyroidism to improve cognitive outcomes in older adults. Conclusion: Despite numerous studies, there is no conclusive evidence that supports a direct relationship between hyperthyroidism or hypothyroidism and cognitive decline. Study limitations include heterogeneity in study designs, measurement methodologies, and cognitive assessment tools. Future research is needed to better delineate whether an association exists and whether treatment of thyroid dysfunction ameliorates cognitive impairment.

Abstract:It is still uncertain whether the risk of dementia and cognitive impairment is related to thyroid disease. we carried out a meta-analysis and systematic review (PROSPERO: CRD42021290105) on the associations between thyroid disease and the risks of dementia and cognitive impairment.We searched PubMed, Embase, and Cochrane Library for studies published up to August 2022. The overall relative risk (RRs) and its 95% confidence interval (CIs) were calculated in the random-effects models. Subgroup analyses and meta-regression were conducted to explore the potential source of heterogeneity among studies. We tested and corrected for publication bias by funnel plot-based methods. The Newcastle-Ottawa Scale (NOS) or Agency for Healthcare Research and Quality (AHRQ) scale were used to evaluate the study quality of longitudinal studies and cross-sectional studies, respectively.A total of 15 studies were included in our meta-analysis. Our meta-analysis showed that hyperthyroidism (RR = 1.14, 95% CI = 1.09-1.19) and subclinical hyperthyroidism (RR = 1.56, 95% CI = 1.26-1.93) might be associated with an elevated risk for dementia, while hypothyroidism (RR = 0.93, 95% CI = 0.80-1.08) and subclinical hypothyroidism (RR = 0.84, 95% CI = 0.70-1.01) did not affect the risk.Hyperthyroidism and subclinical hyperthyroidism are predictors of dementia.PROSPERO, Identifier: CRD42021290105.

Abstract:The thyroid gland is crucial for the regulation of metabolism, growth, and development of various tissues, organs, systems, including the central nervous system. Recent studies have implicated the role of thyroid dysfunction in the etiology of Alzheimer's disease (AD), while AD leads to a significant increase in the prevalence of thyroid dysfunction. In this review, we have analyzed the role of thyroid function in the pathophysiology of AD as well as its biomarkers. The present review aims to provide encouraging targets for early screening of AD risk factors and intervention strategies.

Abstract:The incidence of most thyroid diseases are prevalent in women in ratio 8 : 1 to men, and especially hypothyroidism arises with age. Unrecognized thyroid dysfunction leads to increased: cardiovascular risk, bone fractures, cognitive impairment, depression, and mortality. The symptoms of thyroid diseases can be nonspecific or common in seniors with ageing complaints. The interpretation of thyroid function tests, the physiological changes in secretion and metabolism of thyrotropin (TSH) and thyroid hormones must be considered, as well as the influence of comorbidities, certain drugs, and individual "set point" of pituitary gland. According to many observations the serum TSH, thyroxine (T4), concentrations depend on age, comorbidities, and medical treatment - these together sometimes make the diagnosis of thyroid dysfunction complicated in older population. The observational data may suggest a diminished pituitary sensitivity to T4 in the ageing population. According to several studies, serum TSH concentration is probably age-dependant and the upper limit of TSH could be 5.28-5.9 mIU/l in those who are > 70 years old. Therapy of thyroid dysfunction is different in elderly persons than in young people; hypothyroidism should be treated with caution, because high doses of thyroxine can lead to cardiac arrhythmias and increased bone turnover. Hyperthyroidism could be treated either with surgery or preferable with radioiodin. Especially the diagnosis of subclinical hypothyroidism should be made with caution after concerning different important circumstances. Nevertheless there are certain conditions, when subclinical hypothyroidism must be treated. Key words: ageing - hyperthyroidism - hypothyroidism - thyroid diseases.
Subclinical hyperthyroidism and the risk of dementia: A meta‐analysis

Abstract:A meta‐analysis incorporating nine cohort studies of 49218 community‐derived participants showed that subjects with subclinical hyperthyroidism may have a higher risk of dementia compared to those with euthyroidism. These results highlight the importance of the evaluation of thyroid dysfunction in the risk stratification of dementia in the general population, even for those with subclinical hyperthyroidism. Background Accumulating evidence suggests that thyroid dysfunction may be related to the risk of dementia. However, previous studies evaluating the association between subclinical hyperthyroidism and the risk of dementia showed inconsistent results. This systematic review and meta‐analysis were performed to evaluate the relationship between subclinical hyperthyroidism and the incidence of dementia in the general population. Methods Cohort studies relevant were retrieved by searching the electronic databases including PubMed, Web of Science, and Embase. A random‐effects model was used to combine the data by incorporating the influence of between‐study heterogeneity. Subgroup and meta‐regression analyses were performed to investigate the source of heterogeneity. Results Nine cohort studies including 49,218 community‐derived participants were included. Among them, 3177 (6.5%) had subclinical hyperthyroidism at baseline. During a mean follow‐up of 10.2 years, 4044 participants developed dementia. The pooled results showed that compared to the participants with euthyroidism, those with subclinical hyperthyroidism had a higher incidence of dementia (risk ratio: 1.38, 95% confidence interval: 1.09 to 1.74, p = .006; I2 = 47%). Subgroup analyses according to study design, age of the participants, methods for diagnosis of dementia, or analytic model did not significantly change the results. The univariate meta‐regression showed that the cutoff of thyroid‐stimulating hormone for defining subclinical hyperthyroidism negatively affected the association between subclinical hyperthyroidism and dementia (coefficient: –1.44, p = .009), which completely explained the heterogeneity (residual I2 = 0%). Conclusion Subjects with subclinical hyperthyroidism may have a higher risk of dementia compared to those with euthyroidism.

Abstract:The increase in the incidence of dementia over the last century correlates strongly with the increases in post-reproductive lifespan during this time. As post-reproductive lifespan continues to increase it is likely that the incidence of dementia will also increase unless therapies are developed to prevent, slow or cure dementia. A growing body of evidence implicates age-related endocrine dyscrasia and the length of time that the brain is subjected to this endocrine dyscrasia, as a key causal event leading to the cognitive decline associated with aging and Alzheimer's disease (AD), the major form of dementia in our society. In particular, the elevations in circulating gonadotropins, resulting from the loss of gonadal sex hormone production with menopause and andropause, appear central to the development of AD neuropathology and cognitive decline. This is supported by numerous cell biology, preclinical animal, and epidemiological studies, as well as human clinical studies where suppression of circulating luteinizing hormone and/or follicle-stimulating hormone with either gonadotropin-releasing hormone analogues, or via physiological hormone replacement therapy, has been demonstrated to halt or significantly slow cognitive decline in those with AD. This review provides an overview of past and present studies demonstrating the importance of hypothalamic-pituitary-gonadal hormone balance for normal cognitive functioning, and how targeting age-related endocrine dyscrasia with hormone rebalancing strategies provides an alternative treatment route for those with AD.

We also studied gene perturbation correlation in the FAD4T mouse thyroid, bone marrow, and brain by further single-cell RNA sequencing (scRNA-seq) data of the bone marrow and brain.RESULTS:In this study, we found alterations in THs in both AD and aging mice. RNA-seq data showed significant upregulation of T-cell infiltration- and cell proliferation-related genes in FAD4T mouse thyroid. In addition, Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) analyses revealed that upregulated genes were enriched in the functional gene modules of activation of immune cells. Downregulated energy metabolism-related genes were prominent in aging thyroids, which reflected the reduction in THs. GSEA showed a similar enrichment tendency in both mouse thyroids, suggesting their analogous inflammation state. In addition, the regulation of leukocyte activation and migration was a common signature between the thyroid, brain, and bone marrow of FAD4T mice.CONCLUSIONS:Our findings identified immune cell infiltration of the thyroid as the potential underlying mechanism of the alteration of THs in AD and aging.

Abstract:The identification of critical factors that can worsen the mechanisms contributing to the pathophysiology of Alzheimer disease is of paramount importance. Thyroid hormones (TH) fit this criterion. Epidemiological studies have identified an association between altered circulating TH levels and Alzheimer disease. The study of human and animal models indicates that TH can affect all the main cellular, molecular, and genetic mechanisms known as hallmarks of Alzheimer disease. This is true not only for the excessive production in the brain of protein aggregates leading to amyloid plaques and neurofibrillary tangles but also for the clearance of these molecules from the brain parenchyma via the blood-brain barrier and for the escalated process of neuroinflammation-and even for the effects of carrying Alzheimer-associated genetic variants. Suboptimal TH levels result in a greater accumulation of protein aggregates in the brain. The direct TH regulation of critical genes involved in amyloid beta production and clearance is remarkable, affecting the expression of multiple genes, including APP (related to amyloid beta production), APOE, LRP1, TREM2, AQP4, and ABCB1 (related to amyloid beta clearance). TH also affects microglia by increasing their migration and function and directly regulating the immunosuppressor gene CD73, impacting the immune response of these cells. Studies aiming to understand the mechanisms that could explain how changes in TH levels can contribute to the brain alterations seen in patients with Alzheimer disease are ongoing. These studies have potential implications for the management of patients with Alzheimer disease and ultimately can contribute to devising new interventions for these conditions.
Thyroid Disorders and Dementia Risk: A Nationwide Population-Based Case-Control Study

Abstract:Background and Objectives Dementia has been gaining attention in aging societies and is estimated to affect 50 million adults globally in 2020, and 12% of the US population may develop a thyroid disorder in their lifetime. There have been limited studies investigating the correlation between thyroid disorder and dementia in the Asian population. Methods Our large nationwide population-based case-control study used the Taiwanese National Health Insurance Research Database. A total of 7,843 adults with newly diagnosed dementia without a history of dementia or neurodegenerative disease between 2006 and 2013 were identified and included in our study. In addition, 7,843 adults without dementia diagnosis before the index date were age and sex-matched as controls. Diagnosis of hyperthyroidism or hypothyroidism before the diagnosis of dementia or the same index date was identified. Results were obtained from logistic regression models and adjusted for sex, age, history of hypertension, diabetes, coronary artery disease, depression, hyperlipidemia, alcohol dependence syndrome, tinnitus, hearing loss, and radioactive iodine treatment. Results A total of 15,686 patients were included in the study. Both case and control groups were slightly predominantly female (4,066 [51.8%]). The mean (SD) age for those with dementia was 74.9 (11.3) years and for those without dementia was 74.5 (11.3) years. Among patients aged 65 years or older, a history of hypothyroidism was associated with an increased risk of being diagnosed with dementia (adjusted odds ratio [aOR] 1.81; 95% CI 1.14–2.87; p = 0.011), which was an association not present in patients older than 50 years but younger than 65 years. We found that this association was most significant among patients aged 65 years or older with a history of hypothyroidism who received hypothyroidism medication (aOR 3.17; 95% CI 1.04–9.69; p = 0.043). Discussion Our large-scale case-control study found that among people aged 65 years or older, those with a history of hypothyroidism were associated with an 81% increased risk of having dementia and among those, there was a more than 3-fold increased dementia risk with thyroid conditions that required thyroid hormone replacement treatment. Future well-controlled prospective longitudinal studies should be conducted to elucidate these potential mechanisms and relationships. Classification of Evidence This study provides Class III evidence that among patients aged 65 years or older, a history of hypothyroidism was associated with an increased risk of being diagnosed with dementia.

Abstract:Imbalances in thyroid hormones have been linked with Alzheimer’s dementia. Several studies have reported an association between thyroid disorders, such as hyper- or hypothyroidism, with Alzheimer’s disease. However, there remains no consensus about the precise role of thyroid dysfunction in Alzheimer’s disease. In this study we systematically searched PubMed, Embase and Scopus for clinical studies which reported the prevalence of hyper- or hypothyroidism in people with Alzheimer’s disease compared to controls. Meta-analysis was performed to compare thyroid disorder prevalence in Alzheimer’s disease and controls. Subgroup analysis was performed to assess the clinical and subclinical thyroid dysfunction, separately. Seven studies, including 1189 people with Alzheimer’s disease and 72711 controls, were included in our sample. Hypothyroidism was significantly more prevalent in Alzheimer’s disease compared with controls (6.4% vs 2.4%, p=0.01). Subgroup analysis showed that clinical hypothyroidism was not significantly different between Alzheimer’s disease compared to controls (10.0% vs 5.3%, p=0.35). There was no difference in the crude overall prevalence of clinical and subclinical hyperthyroidism in Alzheimer’s disease versus controls (2.4 vs 1.9%, p=0.73). Our analyses revealed a higher prevalence of hypothyroidism in Alzheimer’s disease. Whether this finding is explained by hypothyroidism being a risk factor for, or consequence of, Alzheimer’s disease requires longitudinal analysis. Our review supports further work into a potential role for treatment of hypothyroidism in the prevention or delay of Alzheimer’s disease.

Abstract:Introduction: Understanding the particularities of thyroid-cognition interactions in the elderly is crucial in assessing the risks and evaluating therapeutic options. Methods: Cross-sectional analyses where participants from Alzheimer s Disease Neuroimaging Initiative (ADNI) with mild cognitive impairment (MCI) and healthy controls (HC), with complete neurocognitive tests, thyroid stimulating hormone (TSH) <10 uIU/mL, and geriatric depression scale (GDS) <5 were eligible. Linear and logistic regression models, including testing for non-linearity, were performed. Sex and neurodegeneration-related stratifications were explored. Results: Of the total 1845 participants, with a median age of 73 (IQR: 68, 78); 887 (48%) were females, and 1056 (57%) had MCI. The median TSH level was 1.70 uIU/mL (IQR: 1.15, 2.40). There was a significant association between TSH and cognition only in males (adj. Beta Males: -0.40; 95% CI: -0.74, -0.07; p-value: 0.019). The odds of being diagnosed with MCI at baseline decreased with higher TSH levels in the total study population (adj. OR Total: 0.87; 95% CI: 0.79, 0.95; p-value: 0.002), and in males (adj. OR Males: 0.80; 95% CI: 0.70, 0.92; p-value: 0.001). The median TSH value was a significant cutoff in this association. Conclusions: The association between thyroid function and cognitive decline in the elderly is subject to a sex-driven effect modification and depends on a cutoff value.

Abstract:Objective To evaluate the relationship between thyroid dysfunction and bone mineral loss in the aged women.Methods Forty-eight women, aged 65-79,with bone mineral loss were retrospectively studied.Serum FT_3,FT_4 and TSH were analyzed by radioimmunoassay.Bone mineral density was detected by DEXA bone densitometry.Results 20.8% of the 48 women were found with thyroid dysfunction.The abnormal thyroid function was related to bone mineral loss in hips.Conclusions Thyroid function should be tested and followed-up in the aged women with bone mineral loss.
Clinical study on levels of blood fat,folic acid,vitamin B_(12) and thyroid hormone in elderly patients with mild cognitive impairment

Abstract:Objective It is to approach the relationship between and mild cognitive impairment(MCI) and the levels of blood fat,folic acid,vitamin B12 and thyroid hormone.Methods The serum levels of blood fat,folic acid,vitamin B12 and thyroid hormone in 115 MCI patients,105 AD patients and 80 normal old people(control group) were detected and compared with each group.Results The levels of total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),folic acid and vitamin B12 in MCI group and AD group were all significantly higher than those in control group(P<0.05 or 0.01),while the levels of triglyceride(TG) and high density lipoprotein cholesterol(HDL-C) in MCI group and AD group all had no significant differences with control group(all P>0.05).The levels of blood fat,folic acid and vitamin B12 in MCI group all had no significant differences with AD group(all P>0.05).The levels of triiodothyronine(T3) and free triiodothyronine(FT3) in MCI group and AD group were all significantly lower than those in control group(all P<0.05).The tendency of T3 and FT3 in AD group both decreased than those in MCI group(all P<0.05),while there were no significant differences on the levels of thyroxine(T4) and free thyroxin(FT4) among MCI group,AD group and control group(all P>0.05).On the level of thyrotropic-stimulating hormone(TSH),there was no significant difference between MCI group and control group(P>0.05),while the levels of TSH in AD group significantly decreased than those in MCI group and control group(both P<0.05).Conclusion The levels of TC and LDL-C heightened and the levels of folic acid,vitamin B12,T3 and FT3 lowered are all related to the pathogenesis of MCI and AD.TSH maybe has clinical referenced value in transformation of MCI to AD.Lowering lipid,adding folic acid and Vitamin B12 and maintaining normal thyroid function maybe has determinate action in the prevention and cure of MCI and AD.
Associations between Thyroid Hormones and Cognitive Impairment in Patients with Parkinson's Disease

Abstract:This study aims to explore the correlation of serum thyroid hormone levels to cognitive impairments in Parkinson's disease (PD) patients. In this retrospective study, 106 Chinese patients without cognitive impairments and 94 patients with cognitive impairments, including 55 with mild cognitive impairment (PD-MCI) and 39 with PD dementia (PDD), were analyzed. Clinical data regarding the PD assessments, including disease duration, Unified Parkinson's Disease Rating Scale (UPDRS) Part 3 scores, and Hoehn and Yahr (H–Y) staging, were analyzed. Cognitive functions were evaluated using the Montreal Cognitive Assessment score. Serum levels of thyroid-stimulating hormone (TSH), free thyroxine (FT4), and free triiodothyronine (FT3), were measured using ELISA. Significantly altered H–Y staging, disease duration, and UPDRS Part 3 scores were observed in PD patients with cognitive impairment compared with those without. Serum levels of FT3 were significantly decreased, while FT4 and TSH levels were significantly elevated in PD patients with cognitive impairment compared with those without. Combined detection of TSH, FT3, and FT4 showed value in distinguishing PD patients with and without cognitive impairment. Furthermore, a comparison of serum levels between PD-MCI and PDD patients revealed significant association between thyroid hormone levels and the degree of cognitive impairment in PD patients. Our findings suggest a relationship between changes in serum thyroid hormone levels and cognitive impairments in PD patients. Thyroid hormone levels, particularly FT3, may serve as potential markers for cognitive dysfunction in PD.

Abstract:Abstract Context Hypothyroidism is often associated with cognitive and emotional dysregulation; however, the underlying neuropathological mechanisms remain elusive. Objective The study aimed to characterize abnormal alterations in hippocampal subfield volumes and functional connectivity (FC) in patients with subclinical hypothyroidism (SCH) and overt hypothyroidism (OH). Methods This cross-sectional observational study comprised 47 and 40 patients with newly diagnosed adult-onset primary SCH and OH, respectively, and 53 well-matched healthy controls (HCs). The demographics, clinical variables, and neuropsychological scale scores were collected. Next, the hippocampal subfield volumes and seed-based FC were compared between the groups. Finally, correlation analyses were performed. Results SCH and OH exhibited significant alterations in cognitive and emotional scale scores. Specifically, the volumes of the right granule cell molecular layer of the dentate gyrus (GC-ML-DG) head, cornu ammonis (CA) 4, and CA3 head were reduced in the SCH and OH groups. Moreover, the volumes of the right molecular layer head, CA1 body, left GC-ML-DG head, and CA4 head were lower in SCH. In addition, the hippocampal subfield volumes decreased more significantly in SCH than OH. The seed-based FC decreased in SCH but increased in OH compared with HCs. Correlation analyses revealed thyroid hormone was negatively correlated with FC values in hypothyroidism. Conclusion Patients with SCH and OH might be at risk of cognitive decline, anxiety, or depression, and exhibited alterations in volume and FC in specific hippocampal subfields. Furthermore, the reduction in volume was more pronounced in SCH. This study provides novel insights into the neuropathological mechanisms of brain impairment in hypothyroidism.

Abstract:Abstract Context Subclinical hypothyroidism is associated with metabolic diseases; however, it remains controversial in older individuals. Objective This work aimed to investigate the relationship between thyrotropin (TSH) levels and metabolic diseases. Methods In this cross-sectional study, sampling was conducted from nationally representative general communities from 31 provinces in mainland China. A total of 6791 older (aged ≥65 years) and 55 303 young participants (aged 18-64 years) were selected after excluding individuals with overt hyperthyroidism or overt hypothyroidism. According to the kit, TSH reference range (0.27-4.2 mU/L) and the age-specific TSH range previously formulated (an upper limit of 8.86 mU/L for older adults and 6.57 mU/L for young adults), the older adults and young adults were separately divided into 4 groups based on their TSH levels. Main outcome measures included anthropometric assessments, serum concentrations of thyroid functions, and various metabolic parameters. Results In contrast to young adults, there was no significant increase in the prevalence of any metabolic disorders assessed in the slightly elevated TSH group (TSH 4.21-8.86 mU/L) compared to the euthyroid group (TSH 0.27-4.2 mU/L) among older adults. After adjusting for interference factors, a TSH level higher than 8.86 mU/L was found to be an independent risk factor for low high-density lipoprotein cholesterol (OR, 1.84; 95% CI, 1.14-2.98) and dyslipidemia (OR, 1.49; 95% CI, 1.09-2.04) when compared to the euthyroid group in older adults. Conclusion Slightly elevated TSH levels are not associated with an increased risk of metabolic diseases in older adults. Therefore, we recommend raising the upper limit of the TSH range for individuals aged 65 years and older.
