Abstract:PURPOSE:The aim of this study is to evaluate the effect of image-guided navigation on gap arthroplasty in the treatment of temporomandibular joint (TMJ) ankylosis.METHODS:Four patients with unilateral TMJ ankylosis underwent gap arthroplasty with the guide of computer-assisted navigation. With preoperative planning and 3-dimensional simulation, normal anatomical structures of the TMJ were created by superimposing and comparing the unaffected and the affected side. The amount and range of ankylotic bone was determined and displayed. All patients were treated by TMJ gap arthroplasty under the guide of navigation system.RESULTS:Through registration, an accurate match between the intraoperative anatomy and the CT images had been achieved. With the guide of the navigation, anatomy structures and the position of surgical instruments were shown real-time on the screen. No complications occurred in all patients and the systematic error was within 1 mm. The use of image-guided navigation resulted in the promotion of safe surgical excision of the ankylosed skull base tissue.CONCLUSIONS:Navigation-guided resection of the ankylotic bone in the TMJ gap arthroplasty is a valuable and safe technique in this potentially complicated procedure. Abstract:Gap arthroplasty, used in the treatment of temporomandibular joint (TMJ) ankylosis, is challenging, requiring resecting of massive abnormal bone formation at the skull base with complex and distorted anatomy. This study evaluated the application of image-guided navigation to gap arthroplasty. Four gap arthroplasties were performed on patients with unilateral TMJ ankylosis under computer-assisted navigation guidance. After preoperative planning and 3-dimensional simulation, the normal anatomic structures of the TMJ were created by superimposing and comparing the unaffected and affected sides. The amount and range of ankylotic bone to be resected was determined and displayed. Registration achieved an accurate match between the intra-operative anatomy and the CT virtual images. Anatomic structures and the position of surgical instruments were shown real time on the screen. In all cases the accuracy of the system measured by the computer did not exceed 1mm. No complications occurred and the mean minimal thickness of the skull base between middle cranial fossa and reconstructed glenoid fossa was 1.97mm. Using image-guided navigation resulted in safe surgical excision of the bony ankylosis from the skull base. Navigation-guided resection of the ankylotic bone in the TMJ gap arthroplasty was a valuable and safe technique in this potentially complicated procedure.
Computer-Aided Surgical Simulation and Navigation in Reconstruction of Old Complicated Cranio-Maxillofacial Fractures

Abstract:Reconstruction of the old complicated craniomaxillofacial (CMF) fractures requires a precise and safe therapy to improve the functional and aesthetic outcome. In this paper, an image-guided simulation and navigation system was developed (Navi-CMFS)to improve the accuracy and reliability of the reconstruction surgery of the old complicated CMF fractures. Pre-operative planning and simulation based on 3-dimension (3D) model can provide enough 3D information of the patient. Intra-operative navigation can widen the surgeons’ vision field and locate the continually updated position of the instruments and the patient, to avoid injury to the important tissue. Clinical studies were conducted on 6 patients with complicated CMF fractures. Firstly, a virtual 3D skull model was reconstructed from pre-surgical image dataset information. In this virtual 3D model, surgeon moved or rotated the fractured bone and relocated it in a desired site according to the mirrored part from the unaffected side. After patient-to-image registration, the surgical instrument and patient were tracked in real-time by the optical tracking system during operation. In this way the old complicated CMF fractures were relocated satisfactorily guided by the virtual image. The results show that this system is proved to be essential helpful in reconstruction of the old complicated CMF fractures.
Computer-assisted Navigation in Mandibular Reconstruction with Fibula Free Flap

Abstract:Background: The mandible is an important component of the face, as it contributes to motor functions and facial appearance. In mandibular reconstruction, accurate location of the reconstructed mandibular angle and condyle becomes important and challenging, because the mandibular angle is important for facial symmetry, and the condyle is closely related to temporomandibular joint function. Objectives: To present our experiences of computer-assisted navigation in mandibular reconstruction and evaluate its effectiveness. Methods: 10 patients who underwent navigation-guided mandibular reconstruction with a fibula free flap were reviewed. During preoperative planning and simulation, the ideal position for the reconstructed mandibles was determined based on mirror imaging of the unaffected side. Under intraoperative navigation, the ideal positions of the mandibular angles and condyles were confirmed by the navigation probe. The surgical results were evaluated through postoperative panoramic radiographs, coronal computed tomography (CT) scans, image fusion and clinical examinations. Findings: Navigation-guided mandibular reconstructions were successfully completed based on preoperative planning and simulation. The navigation system could continuously track the position of the probe, which guided the positioning of the fibula flaps. The surgical discrepancy in the mandibular angle between the actual surgical results and the preoperative designs was 2.29 ± 1.14 mm. Panoramic radiographs and coronal CT scans illustrated that all the condyles fitted into their glenoid fossae. All patients were satisfied with their functional and aesthetic outcomes. Conclusion: Computer-assisted navigation is a viable technology for improving surgical outcomes in mandibular reconstruction, which can assist the surgeons by providing real-time three-dimensional surgical references during the operation.

Abstract:This study introduces a novel surgical technique that leverages digital design for the precise repair of mandibular defects resulting from benign jaw tumors. The restoration of the mandibular defect is accomplished through autologous bone grafting from the mandible itself. This method significantly diminishes surgical complexity and risk, meeting the patient’s preference to avoid additional surgical sites. Notably, 15 months postsurgery, the patient’s mandible dimensions were suitable for dental implantation. Therefore, this technique has proven effective in repairing mandibular defects caused by the excision of benign tumors.

Abstract:Background Nowadays, there is a trend towards computer-aided oral and maxillofacial surgery. However, due to complicated preoperative planning, traditional neurosurgery-orientated navigation systems are not suitable and have to be adapted.Methods A surgical navigation system is presented for oral and maxillofacial surgery, named AccuNavi, which integrates 3D medical modelling, preoperative surgical planning and intraoperative tracking. Various algorithms were introduced, for each of which a set of dynamic link libraries (DLLs) were developed using C++, as well as the VTK and ITK, via object-orientated programming methodology, allowing for expandability, accessibility and maintainability in our system.Results The advantage of AccuNavi is validated through clinical applications, especially in a case report about the planning and subsequent precise treatment of old zygomatic fractures. The postoperative 3D-reconstructed CT image demonstrated that the fractured zygoma was fixed in the appropriate position and orientation.Conclusions The clinical application demonstrates tyhe feasibility and reliability of AccuNavi, and it can be extended to various areas in oral and maxillofacial surgery. Abstract:The mandible has an important role in appearance and function. The aim of this study was to describe and evaluate surgical navigation-assisted mandibular reconstruction with the fibula flap. Patients recruited into the study had a custom dental splint fabricated to maintain the mandible in a fixed position. Later, the computed tomography (CT) scan, preoperative design, and operation on the mandible were done in the same position. At 1 week after surgery, a CT scan was done to evaluate the repeatability between the preoperative design and the postoperative result. Twenty patients were enrolled in this study. Good repeatability between the postoperative CT and the preoperative design was found. The repeatability between the preoperative plan and postoperative outcome was 79.1 ± 8.6% at within 1mm, 87.1 ± 6.7% at within 2mm, and 91.9 ± 5.4% at within 3mm. From this study, it can be concluded that surgical navigation techniques can precisely transfer the preoperative design to the operation in mandible reconstruction with a fibula flap. This will assist the surgeon in achieving good cosmetic and functional outcomes.
Accuracy of Digital Templates for Guidance of Custom-Made Total Temporomandibular Joint Replacement.

Abstract:This study evaluated the application of digital templates to guide custom-made total temporomandibular joint (TMJ) replacement. Patients treated with a custom-made total TMJ prosthesis for TMJ osteoarthrosis, ankylosis, or tumours were included prospectively. Before surgery, two types of digital template (articular eminence and condyle neck templates) were designed to guide the bone osteotomy and prosthesis positioning. The reconstructive outcomes were assessed through clinical examinations and accuracy analysis by superimposing the postoperative three-dimensional craniomaxillofacial model onto the preoperative virtual plan. Thirty-seven patients (45 joints) underwent successful TMJ reconstruction with the custom-made TMJ prosthesis guided by the digital templates, without intermaxillary fixation. For all patients, the occlusal relationship was stable in reference to the preoperative state. There were significant improvements in pain and maximum inter-incisal opening. Through merging of the pre-and postoperative craniomaxillofacial models, the maximum implanted error was 1.17 +/- 0.23 mm in linear measurement and 1.19 +/- 0.14 mm in surface deviation. There was more deviation in the anteromedial part of the fossa and inferior part of the mandibular handle. The digital templates were able to assist in the accurate placement of the TMJ prosthesis without the need for intermaxillary fixation.

Abstract:This retrospective study evaluated the effect of surgical computer-assisted navigation in temporomandibular joint (TMJ) ankylosis gap arthroplasty. Eighteen patients (25 sides) with bony ankylosis who underwent surgical treatment under computer-assisted navigation (navigation group) from May 2011 to April 2013 were assessed, along with 19 such patients (25 sides) treated without computer-assisted navigation (non-navigation group) from March 2009 to April 2011. The navigation group patients underwent surgery with the preservation of ≥3mm bone thickness in the skull base and anterior wall of the external auditory canal. Postoperatively, computed tomography (CT) was used to measure the residual bone thickness in the skull base and anterior wall of the external auditory canal. Maximum mouth opening (MMO) changes were evaluated at >1 year of follow-up. Postoperative CT measurements showed that the lowest skull base thickness in the navigation group was significantly lower than that in the non-navigation group (3.86±1.95mm vs. 6.01±3.07mm, P=0.009). The lowest thicknesses of the anterior wall of the external auditory canal were similar in the two groups. Postoperative follow-up showed similar average MMO in the two groups. Therefore, with the navigation system, TMJ ankylosis gap arthroplasty can achieve more extensive removal of the ankylosed bone, at least towards the skull base, under the premise of ensuring a safety distance of 3mm.
Developing a universal and reliable temporomandibular joint reference system for quantifying morphological and positional changes

Abstract:Objectives: This study aimed to provide a universal and reliable reference system quantifying temporomandibular joint (TMJ) morphological and positional changes. Methods: Large field-of-view (FOV) cone-beam computed tomography (CBCT) images (20 TMJs) from 10 preorthognathic surgery patients and limited FOV CBCT images (40 TMJs) from 20 splint therapy-treated patients with temporomandibular disorders were collected. TMJ-specific reference system including a TMJ horizontal reference plane (TMJHP) and a local coordinate system (TMJCS) was constructed with landmarks on cranial base. Its application for TMJ measurements and its spatial relationship to common Frankfort horizontal plane (FHP) and maxillofacial coordinate system (MFCS) were evaluated. Results: Five relevant landmarks were selected to optimally construct TMJ-specific reference system. General parallelism between TMJHP and FHP was demonstrated by minimal angular and constant distance deviation (1.714 ± 0.811º; 2.925 ± 0.817 mm). Additionally, tiny axial orientational deviations (0.181 ± 6.805º) suggested TMJCS rivaled MFCS. Moreover, small deviations in orientations and distances (1.232 ± 0.609º; 0.310 ± 0.202 mm) indicated considerable reliability for TMJCS construction, with intraclass correlation coefficients (ICCs) ranging from 0.999 to 1.000. Lastly, slight discrepancies in translations and rotations revealed high reliability for condylar positional and morphological measurements (ICC, 0.918-0.999). Limitations: TMJ-specific reference system was merely tested in two representative FOVs. Conclusions: This study provides a universal and reliable reference system for TMJ assessment that is applicable to both limited and large FOV CBCT. It would improve comparability among diverse studies and enable comprehensive evaluations of TMJ positional and morphological changes during TMJ-related treatment follow-up such as splint therapy and disease progression.

Abstract:OBJECTIVE:To investigate the method of the three-dimensional reconstruction of temporomandibular joint (TMJ) on the basis of CT and MRI medical image fusion technology, which establishes good foundation for a series biomechanical studies and clinical analysis.METHODS:The geometric data for the TMJ model were obtained from a young and healthy male volunteer. Twelve spherical markers were located on the regions of both TMJ and mental tubercles, where CT and MRI scans can reach. The three-dimensional geometric of the hard and soft tissues of TMJ was reconstructed by CT and MRI scans data, which was based on the registration procedures of spherical markers located on the face of the volunteer.RESULTS:The three-dimensional geometric model of TMJ was obtained, including hibateral articular disc, mandible, glenoid fossa and inferior teeth, and it had better geometric similarity. The results showed that the methods of three-dimensional reconstruction of TMJ with CT and MRI medical image fusion technology can be operated easily and promptly. The constructed model had good morphology.CONCLUSION:It is feasible to construct the three-dimensional model of TMJ on the basis of the registration procedures of spherical markers and CT-MRI medical image fusion technology. It has the advantage of simulating both hard and soft tissues reasonably. The model well expresses the shape of the TMJ.
A Novel Precise Optical Navigation System for Craniomaxillofacial Surgery Registered With an Occlusal Splint

Abstract:Background: An augmented reality tool allows visual tracking of real anatomical structures and superimposing virtual images, so it can be used for navigation of important structures during surgery. Objectives: The authors have developed a new occlusal splint-based optical navigation system for craniomaxillofacial surgery. In this study, the authors aim to measure the accuracy of the system and further analyze the main factors influencing precision. Methods: Ten beagle dogs were selected and a three-dimensional model was established through computed tomography scanning, dental model making, and laser scanning, and then registration was performed according to the tooth marking points. The bilateral mandibular osteotomy was performed on Beagle dogs under navigation system based on the occlusal splint. The left side was taken to compare the deviation between the preoperative plan and the surgical results, and the accuracy of distance and angle and the stability of the system were analyzed. Results: The average position deviation between the preoperative design and intraoperative navigation was: 0.01 +/- 0.73mm on the lateral height of the mandibular ramus, 0.26 +/- 0.57mm on the inner height of the mandibular ramus, and 0.20 +/- 0.51mm on the osteotomy length. The average angle deviation is 0.948 +/- 1.388 on the angle between the mandibular osteotomy plane and ramus plane and 0.668 +/- 0.978 on the angle of the retained mandibular angle. And most of the data showed good consistency. Conclusions: In summary, the accuracy of the system can meet clinical requirements and can be used as a useful tool to improve the accuracy of craniomaxillofacial surgery.

Abstract:Tendon sheath giant cell tumor is an idiopathic proliferative and destructive disease of the synovium. It is rare and tends to arise in large joints, for example, knee and ankle, but it can also arise in temporomandibular joints (TMJs). Because of its destructive nature, immediate treatment upon diagnosis is recommended. Radical resection proved to be an excellent choice for superior local control. However, the unfavorable anatomic location of TMJ and infratemporal fossa tumor with intradural extension make such a resection impractical. Hereby, we reported a case of resection of a TMJ tendon sheath giant cell tumor with intradural extension using a transcranial approach. This involves a complex radical resection with subsequent reconstruction. Transposition of temporal bone flap is a novel state-of-the-art technique in reconstructing the middle fossa floor defect by providing a three-dimensional rigid architecture to support the brain. Temporal bone flap is a reliable plug for rigid support in preventing brain hernia and cerebrospinal fluid leak. Despite its complexity, this cost-effective technique is relatively straightforward to learn and is applicable across all socioeconomic groups.

Abstract:Purpose: To introduce the preliminary application of rapid prototyping (RP) for temporomandibular joint (TMJ) surgery.Materials and Methods: This study included 11 consecutive patients (13 joints) seeking TMJ replacement. All patients had previously undergone 3-dimensional computed tomography (CT) scanning (0.625-mm slice thickness) of the craniofacial skeleton. The data from CT scanning in DICOM (Digital Imaging and Communications in Medicine) format were input into the interactive Simplant CMF software program (Materialise Medical, Leuven, Belgium). Preoperative planning included segmentation and osteotomies. The movements of the jaw bones were simulated by use of Simplant CMF. The affected mandible was reconstructed based on the contralateral side. Then, the titanium plate was shaped on the reconstructed model before surgery. The bone graft was transplanted by the shaped titanium plate during the operation to reconstruct the TMJ. Twenty-four patients who underwent traditional surgery were used as the control group. The operative time of the 2 groups was analyzed with the SPSS software package, version 13.0 (SPSS, Chicago, IL), with the Student t test. The data from CT scanning in the experimental group before and after surgery were compared by paired t test.Results: All the incisions healed primarily without any complications. All patients were satisfied with the operation, because of their symmetric faces and good occlusion. Postoperative magnetic resonance imaging confirmed the position of the transplanted costochondral cartilage in the glenoid fossa. A group t test showed that the operative time was longer in the control group (mean, 7.09 hours) than that in the RP group (mean, 5.67 hours). Three parameters (condyle-incisor, condyle-mental foramen, and condyle-angle) from the postoperative CT scan were analyzed by paired t test, and there was no significant difference between the 2 sides.Conclusion: RP technology provides an advanced method for TMJ reconstruction that can make the TMJ reconstruction more accurate and symmetric, improve the mandible's function, and consequently, enhance the reconstructive effect. Abstract:Objective: To introduce the design, manufacture and clinical application of the custom-made temporomandibular joint (TMJ)-skull base combined prosthesis and evaluate its safety, effectiveness and accuracy. Methods: The patients diagnosed with the TMJ-skull base lesion in Department of Oral Surgery, Shanghai Ninth People's Hospital from October 2016 to November 2020 were recruited in this study. The maxillofacial CT data for all the patients were obtained and transformed into the Mimics 18.0 software preoperatively. The custom-made TMJ-skull base combined prosthesis, included four components, was designed based on the anatomy, stress distribution and movement of the TMJ and skull base, and fabricated by three-dimensional printing and 5-axis milling technologies. The TMJ-skull base lesion was excised completely with the help of digital templates from modified preauricular and/or post and submandibular incisions. The combined prosthesis were implanted and fixed after the lesion resection. The examinations including general situation, cranio-maxillofacial structure and function were taken during and after surgery to assess its using effect. Results: Ten patients [6 females and 4 males, (43.2±13.6) years old] were included and all prostheses were positioned accurately and fixed excellently. After (29.4±17.3) months follow-up, the occlusion relationship was stable and no adverse symptoms such as dizziness, headache, meningeal irritation and permanent facial nerve injury occurred. The pain, diet, mandibular movement function, lateral movement to diseased side and mouth opening had significant improvements. The forward movement and lateral movement to normal side were not improved significantly. There were no prosthesis displacement, loosening and fracture in X-ray and CT postoperatively. With the pre and postoperative craniomaxillofacial model merging, the maximal implanted error was (0.52±0.17) mm for fossa and condyle and (1.62±0.26) mm for skull base and mandibular handle in surface deviation analysis. Conclusions: The custom-made TMJ-skull base combined prosthesis with customized design and 3D printing fabrication is safe, effective and precise in clinical application.

Abstract:Objective: To explore the indication and application of computer-assisted navigation in oral and maxillofacial surgery.Patients and methods: One hundred and four patients including 34 zygomatic-orbital-maxillary fractures, 27 unilateral TMJ ankylosis, 29 craniofacial fibrous dysplasia, 9 mandibular angle hypertrophia, 3 cartilage/bone tumours of jaw and 2 cases with facial foreign bodies were enrolled in this study.CT scans were performed and data was saved in DICOM (digital imaging and communications in medicine) format. The osteotomy lines, amount and range of resection, the reduction position of bony segments and the reconstruction morphology was determined and displayed by preoperative simulation with mirroring and superimposing procedures. All operations were performed under the guidance of navigation system. The accuracy of navigation was evaluated by comparing the postoperative CT 3-D model with preoperative surgical planning.Results: Through registration, an accurate match between the intraoperative anatomy and the Cl' images was achieved. The systematic error checked by computer was within 1 mm. All operations were performed successfully with the guidance of real-time navigation. The mean error between virtual simulation and surgical results was 1.46 +/- 0.24 mm. All patients healed uneventfully and function and profile was improved significantly.Conclusions: With the opportunity to perform preoperative planning, surgical simulation and postoperative prediction, computer-assisted navigation shows great value in improving the accuracy of maxillofacial surgery, reducing operation risk and postsurgical morbidity, and restoring facial symmetry. Abstract:Objective. This article presents our experiences of computer-assisted navigation in mandibular reconstruction and evaluates its effectiveness.Study Design. Eight patients who underwent navigation-guided mandibular reconstruction with a fibula free flap were reviewed. Under intraoperative navigation, the ideal positions of the mandibular angles and condyles were confirmed by the navigation probe. The surgical results were evaluated through postoperative panoramic radiographs, coronal computed tomography scans, and image fusion.Results. Navigation-guided mandibular reconstructions were successfully completed on the basis of preoperative planning and simulation. The surgical discrepancy in the mandibular angle between the actual surgical results and the preoperative designs was 1.92 +/- 0.97 mm. Panoramic radiographs and coronal computed tomography scans illustrated that all the condyles fitted into their glenoid fossae. All patients were satisfied with their functional and aesthetic outcomes.Conclusions. Computer-assisted navigation is a viable technology for improving surgical outcomes in mandibular reconstruction, which can assist the surgeons by providing real-time three-dimensional surgical references during the operation.

Abstract:In this paper, a computer-aided surgical 3D planning and real-time navigation system (Navi-CMFS) was developed to improve the accuracy and reliability of the mandibular angle restoration. This system integrates 3D medical modeling, 3D pre-operative surgical planning and intra-operative real-time navigation, which enabling the surgeon to locate the continually updated position of the instruments and the patient in the operating room, avoiding to hurt the inferior alveolar nerves, minimizing surgical risks and optimizing clinical results. Clinical studies were conducted on four patients with mandibular angles malformation. The average distance deviations between the planning and post-operation model (system precision) were 1.725mm. The results show that this system provided a more accurate and effective method than conventional surgery in mandibular angle surgery. © 2010 IEEE.
