Hip arthroplasty in treatment of femoral neck fractures in 87 senile patients

Abstract:Objective To explore the outcomes of artificial femoral head replacement(AFHR)and total hip replacement(THR)for elderly patients with femoral neck fracture.Methods From September 2004 to July 2007,totally 87 patients with femoral neck fracture,aged over 80,were treated with THR or AFHR.Eighty-five cases were found to have concomitant diseases(hypertension,diabetes,coronary artery diseases,pulmonary infection,brain diseases,etc).The interval between the injury and operation was 1-7 days(4 cases in 1 day,49 in 3 days,34 in 3-7 days).Results Eighty-seven cases were followed up for 12-26 weeks,with an average of 16 weeks.Harris hip score showed excellent outcomes in 65 patients,good outcomes in 19,and moderate in 3,with a total effective rate being 96.6%;there was no death in our group.Seventy-six(87.4%)patients were free of pain and 11(12.6%)has paroxysmal pain or slight pain without influencing daily life.One year after operation,72 cases(82.8%)could move about,54 cases(62.1%)could move without any aid,and one had a limp.Conclusion To avoid complications or aggravations caused by lying in the bed,patients with femoral neck fractures should receive surgical treatment as early as possible.Arthroplasy can reduce blood loss and trauma to tissues,and relieve post-operation pain,accelerate rehabilitation,and therefore greatly improve the survival rate and quality of life in aged patients with femoral neck fractures.(Shanghai Med J,2008,31:799-801)
