Abstract:Objective:To explore serial changes of maternal plasma fibronectin in women of hypertensive disorder complicating pregnancy and predictive value for preeclampsia.Methods:Peripheral venous blood samples from 180 pregnant women without hypertension history were obtained at 10~14,20~24 and 30~34 gestational weeks to detect the level of fibronectin by enzyme-linked immunoassay technique,and 50 non-pregnant and healthy women were selected as control group.The outcomes of pregnancy were followed up.Results:①12 women were found gestational hypertension(gestational hypertension group),10 women were found preeclampsia(6 severe preeclampsia and 4 mild preeclampsia,preeclampsia group) and 158 women were found normal pregnant outcome(normal pregnancy group) in 180 women.②The mean maternal plasma fibronectin levels at 10~14 and 20~24 gestational weeks remained normal,but increased significantly at 30~34 gestational weeks(P0.01).There was no significant difference in the levels of maternal plasma fibronectin between preeclampsia group,gestational hypertension group and normal pregnancy group at 10~14 gestational weeks;there was significant difference in the levels of maternal plasma fibronectin between preeclampsia group and the other two groups at 20~24 and 30~34 gestational weeks(P=0.000),but there was no significant difference in the levels of maternal plasma fibronectin between gestational hypertension group and normal pregnancy group(P0.05).③Maternal plasma fibronectin levels had predictive value at 20~24 and 30~34 gestational weeks by ROC curve,the area under ROC curve were 0.858 and 0.846,respectively(P=0.000),the best cut-off values of fibronectin for preeclampsia at 20~24 gestational weeks and 30~34 gestational weeks were 388.65mg/L and 416.95mg/L,and the sensitivity,specificity,positive predictive value,negative predictive value and likelihood radio were 100%,78.24%,21.28%,100% and 4.85 at 20~24 gestational weeks and 100%,77.06%,20.00%,100%,4.25 at 30~34 gestational weeks,respectively.Conclusion:Maternal plasma level of fibronectin in preeclampsia is higher than those in gestational hypertension and healthy normal pregnancy women at 20~24 and 30~34 gestational weeks.Fibronectin is potentially useful as a tool for early identification of patients at risk for developing preeclampsia,and the better stages of prediction for preeclampsia are 20~24 gestational weeks.
