Surgery on Cervical Esophagus Carcinoma

Abstract:OBJECTIVE To study the feasibility and effect of substituting esophagus with stomach or colon wit hout thor acoto my on the trea tment of cervi cal esophagus carcinoma. METHODS Twenty-nine patients with cervical esophageal carcinoma were retrospectively reviewed, including 3 patients in T2N0, 3 in T2N1, 6 in T3N0, 7 in T3N1, 5 in T4N0, 5 in T4N1. The esophagus was removed and reconstructed by gastric pull up in 22 patients and by colon interposition in 7 patients. All patients received postoperative radiotherapy. The con- trol group of 23 cases received simple radiotherapy. RESULTS Twenty-five patients of combined therapy group were follow up over 3 years. The 3 and 5 year survival rates were 51.85 %(14/27) and 37.50 %(3/8), respectively. The laryngeal function preservation rate was 72.41 % (21/29) and the decannulation rate was 76.19 %(16/21). The complication rate was 27.59 %(8/ 29). While the 3 and 5 year survival rates of the simple radiotherapy group were 23.81 %(5/21) and 20 %(1/5) respectively. The 3year survival rate of combined therapy group were higher than that of the simple radiotherapy group, the diff eren ces was sign ific ant (P 0 .05). CONCLUSION Surgical resection of cervical esoph- ageal carcinoma with removal of the extraesophageal invaded tissues and preserving the laryngeal function is possible. Combined with radiotherapy, the survival rate and life quality of the patient might be improved.
