Abstract:Objective To preliminarily study the esophageal pH characteristics of patients with gastroesophageal reflux disease(GERD) who had the extra-esophageal symptoms of pharyngolaryngitis and to evaluate the pharmacal effects of proton pump inhibitor(PPI) and selective 5-HT_4 receptor partial agonist. Methods Seventy-six patients suffering from obstinate chronic pharyngolaryngitis were enrolled in this study. All patients(underwent) the esophageal manometry and dual-probe 24 h esophageal pH-metry,then were randomized to(receive) either omeprazole or tegaserod for 2 weeks. The esophageal pH change and symptom scores were compared before and after treatment. Results Among the 76(patients,)38 patients(50%) had abnormal esophageal reflux.Eighteen patients received omeprazole for 2 weeks, who had marked changes on the esophageal pH-metry(P0.05) and symptom scores(P0.01).Tegaserod also significantly improved the symptoms of other 19 paitents who had abnormal esophageal pH profile(P0.01),but did not significantly change the esophageal pH-metry(P0.05).For those who had normal esophageal pH profile,no marked change of symptom scores was found after 2-week treatment with omeprazole or tegaserod(P0.05).There was no significant difference in the esophageal manometry between patients with normal and abnormal esophageal pH records(P0.05).Conclusions Certain cases of GERD may have the symptoms of pharyngolaryngitis. After being treated with omeprazole or tegaserod, the patients with abnormal acid reflux may get significant symptomatic improvements.
