Relationship between lipid metabolism and pregnant outcomes in women with abnormal gestational glucose metabolism

Abstract:Objective To investigate the serum lipid levels and the effect of lipid metabolism on pregnant outcome in pregnant women with glucose metabolism disorders.Methods Base on the result of a 3 hour oral glucose tolerance test(OGTT),406 pregnant women of 24—28 gestational week,whose 50 glucose challenge test ≥7.8 mmol/L,were divided into gestational diabetes mellitus (GDM) and gestational impaired glucose tolerant (GIGT) group (n=175),and glucose challenge test (GCT,+) group (n=231). The serum levels of total cholesterol (TC),triglyceride (TG),high density lipoprotein cholesterol (HDL-C),low density lipoprotein cholesterol (LDL-C),apolipoprotein A1 (ApoA1) and apolipoprotein B (ApoB) were determined at the same time. The age,times of pregnancy,and complications were also recorded.Results The TG and LDL-C levels in GIGT+GDM group were significantly higher than in GCT(+) group (P<0.05); the levels of HDL-c and ApoB in GIGT+GDM group were lower than in GCT(+) group (P<0.05). The incidences of preterm (18.3% vs. 9.5%) and pre-eclampsia (12.6% vs. 6.1%) in GIGT+GDM group were higher than in GCT(+) group (P<0.05).There was significant difference in the incidences of macrosomia (25.0% vs. 11.2%) and pre-eclampsia (20.5% vs. 2.7%) between patients with different TG levels (≥3.5,<3.5 mmol/L)in GCT (+) group. The incidences of preterm (26.2% vs. 13.6%) and pre-eclampsia (30.8% vs. 1.8%) in patients with high TG (≥3.5 mmol/L) in GIGT+GDM group were higher than in those with normal TG (<3.5 mmol/L,P<0.05). Logistic regression analysis showed that increased TG level was an independent risk factor of pre-eclampsia. Conclusion Women with gestational abnormal glucose metabolism have abnormal lipid metabolism,which is associated with poor outcomes of pregnancy.
