Abstract:Objective To investigate the clinical features and treatment experience the H1N1 influenza A cases.Methods We selected the first 81 cases H1N1 influenza in Kunming in 2009,observed and compared their symptoms,signs,blood test,liver function,renal function,myocardial enzymes,C-reactive protein(CRP) and the treatment effects etc.Results The highest average body temperature was(39.09 ± 0.07) ℃ in the early stages among the 81 cases.The percentage patients which had got sore throat stood 100%,cough stood 40%,limb pain,running nose and sneezing accounted for 9.33%,congestive heart angina accounted for 100%,swollen tonsils accounted for 90.67% .Within 3 days after treatment,all the cases recovered to normal body temperature with an average of(36.53 ± 0.04) ℃.Other symptoms disappeared,butthe cough patients got a gradual increase(accounting for 82.67%) which appeared in slightly white phlegm(72%) as well;ofroutine blood test in the early stage,the majority cases’white blood cells were normal,but a few decreased(28.4%) .And the quantity and the percentage neutrophils both reduced(accounting for 43.21% and 50.62%) .Lymphocytes increased(accounting for 85.28%) ,but the quantity and percentage eosinophils dropped(accounting for 50.62% and 61.73%) .Admitted to hospital on the fourth day,the total number and classification white blood cells all returned to normal(P 0.05,significant difference) . Throughout the course the disease,the liver and renal the cases had no abnormal changes.But compared with early stages,the myocardial enzymes in the aspartate aminotransferase(AST) ,lactate dehydrogenase(LDH) ,creatine kinase(CK) all the patients' admission on the fourth day sharply increased(P0.05,significant difference) .However,the creatine kinase isoenzyme(CK-MB) and myoglobin(MYO) still remained the same before and after hospitalization(without significant difference,P0.05) .Those 28 patients that admitted to hospital with a slight increase CRP(5.94 ± 0.88) ,were back to normal on the fourth day treatment(P0.05,significant difference) . The result of Oseltamivir anti-virus treatment,which have made most the patients recovered within a week,was remarkable.Conclusions The clinical features H1N1 influenza A infection among 81 patients in Kunming are:fever,sore throat,and cough as the main performance;physical prevalent angina congestion,swollen tonsils,no obvious abnormalities in peripheral blood in early stage.In addition,the patients' lymphocyte percentage increase meanwhile the percentage neutrophils and eosinophils drop during the early course the disease,but all will recover to normal after treatment without liver damage,nephritis,myocarditis or encephalitis.The result Oseltamivir anti-virus treatment is remarkable.Because the patients with fever may be associated with others diseases,the serious investigation is definitely needed.
