Abstract:Objective The goal of the current study was to analyze the prospective long-term outcome of patients who received simple open reduction and internal fixation and those who received simultaneous subtalar fusion for Sanders type Ⅳ calcaneal fractures,and to provide a guide to choose proper operation for this kind of fracture.Methods From January 2000 to December 2004,42 cases of Sanders type Ⅳ calcaneal fractures were divided into two groups randomly.The simple internal fixation group was treated by simpe open reduction,autograft and internal fixation with reconstruction plate.the fusin group was treated not only by open reduction,autograft and internal fixation with reconstruction plate,but also by subtalar fusion simultaneously.Bohler's angle and Gissane's angle were measured pre and postoperatively in both groups in order to evaluate the anatomic form of calcis,and Maryland score was also analyzed respectively during 6 months,1 year,2 years and 4 years follow-up postoperatively.Results The significant restoration was observed in both groups according to anatomic form and angle measurement of calcis postoperatively(P<0.01).There was no significant difference between the two groups as to the restoration of Bohler's angle and Gissane's angle(P>0.05).Maryland score was 61.9%,66.6%,71.4%,76.2% in turn in postoperative half year,one year,two years,four years in the simple internal fixation group.Maryland score was 66.6%,71.4%,76.2%,76.2% in turn in postoperative half year,one year,two years,four years in the fusion group.There was no significant difference as to the excellence and good rate between the two groups at the same time during follow-up(P>0.05).The mean time of operation and the amount of hemorrhage in the simpls fixation group was less than those in fusion group(P<0.01).There were 2 and 3 cases with delayed wound healing respectively in both groups.One out of them recovered from regional flap,and the other 4 cases healed after conservative management.There was no infection in all cases.The average time of bone union was 2 to 3 months.There were 7 cases who felt painful in the bone donor site in both groups,which disappeared in 2 or 6 months later.ConclusionsSimultaneous subtalar fusion during open reduction and internal fixation cannot improve the long-term outcome for the patients with Sanders type Ⅳ calcaneal fractures.The main purpose and significance of operation is to reduce and fix the displaced intra-articular fractures and to restore the anatomic form of calcis,and finally to rescue the function of the injured joint.
