Abstract:OBJECTIVE:To exploit computer-aided design and computer-aided manufactured (CAD/CAM) techniques and application in the reconstruction of mandible large-scale defect with vascularized fibular bone graft.METHODS:Before actually performing surgery, three-dimensional(3D) computed tomography(CT) was performed in 7 patients with mandibular large-scale defects, and 3D CT images were acquired by processing CT data. Then the CT data were transformed into a readable format and transferred to produce facsimile models by means of using rapid prototyping(RP) techniques. When individual mandibular models and enantiomorphous models were produced, evaluation and surgical simulation was performed in model, which included measuring range of mandible lesions, prefabrication of mandibular reconstructive titanium palate, precise position of titanium screws, shaping the free vascularized fibula by mandibular, etc. According to the simulations, the mandible reconstructions were finished in operation.RESULT:CAD/CAM techniques and application can distinctly display the mandibular lesions and ambient relationships, which is very useful for clinical assessment and surgical planning. Particular advantages were the unlimited trials with the imaging method, and the feeling of reality with the model method. The actual operative time was shortened, and surgery results were satisfactory with few complications.CONCLUSION:CAD/CAM techniques are very helpful for simulation of mandible large-scale defect with complicated anatomical and reconstructive problems. By preoperative simulation of procedures, surgeons can improve or refine treatment planning using this method and improve postoperative results.
Technical Modifications of Double Barrel Vascularised Fibula Graft for Mandibular Reconstruction

Abstract:Double barrel vascularised fibula graft for mandibular reconstruction is used to maintain the height of the neomandible. Although the results are favourable to most of the patients, it is not an easy task to perform mandibular reconstruction. The purpose of this paper is to describe some modifications to the original technique. The recommended modifications include taking minimal cuff of flexor hallucis longus muscle attached to posteromedial surface of fibula, vertical osteotomy and subperiosteal dissection without removing the segment at the folding region, making bony contact between two fibula struts fixed one another, using the partial double barrel vascularised fibula graft or condylar prosthesis for reconstruction of large mandibular defect, and choosing the side of lower limb and handling the skin paddle based on the site of the soft tissue defect and the direction of vascular pedicle. These modifications are helpful and easy to follow while performing mandibular reconstruction with double barrel vascularised fibula graft. Good aesthetic and functional results were achieved by using the modified techniques in most cases that we had done with our patients.

Abstract:PURPOSE:To improve the height deficiency resulted from the conventional straight fibular graft,double barrel fibular osteocutaneous graft(DBFOG) and condylar prostheses were used to reconstruct large mandibular defects.METHODS:13 patients underwent reconstruction of large segmental mandibular defects with DBFOG techniques.The reconstructed sites and heights of the fibula were designed on individual models of the mandible which were made preoperatively by CAD/CAM techniques in all patients.DBFOG in the mandibular body or partial DBFOG were performed in different patients according to the location of the defect.DBFOG and condylar prosthesis were used when the defect involved mandibular body,ramus and condylar process.RESULTS:The fibular osteocutaneous grafts in all patients survived without complications in the donor site of the lower extremities.The contours of the face were satisfied and mouth opening ranged from 2.5cm to 3.5cm.The average height of the reconstructed mandibles was lower than that of the normal sides by 0.47cm,ranging from-0.3cm to 1.5 cm.Occlusions were normal and speeches were comprehensible.Panoramic radiograph and the three-dimensional CT scan revealed that the locations and heights of the fibular bones reach as the pre-operative design.The condylar prostheses were located within the glenoid fossa.CONCLUSION:DBFOG might provide a relatively simple and effective technique for reconstructing large mandibular defects and augmenting the height of the mandible.We suggested that DBFOG be used for reconstruction of defects in mandibular body and mental regions where bear major mastication forces,and artificial prostheses be applied for reconstruction of the ramus where bears no-mastication forces.

Abstract:OBJECTIVE:To evaluate the effect of applying the three dimensional craniomaxillofacial model technique in the restoration of mandibular defects with vascularized fibular autograft. METHODS:Three dimensional craniomaxillofacial models of 24 patients with mandibular defects were achieved by CT scan and rapid prototype technique. Surgery simulation and individual prefabricated titanium board were designed on the models. Vascularized fibular autograft was obtained, remodeled with titanium implant based upon preoperative surgery simulation, and implanted into the site with mandibular defect. RESULTS:By using three-dimensional craniomaxillofacial model, all fibular flaps survived without complication in all 24 patients. Bony union was achieved between the implanted fibula and the mandible. First intention of the wound was achieved. The appearance of the mandibular bone was well recovered shown by 3-dimensional CT scan. CONCLUSION:Based upon the preoperative surgery simulation with three dimensional craniomaxillofacial model, vascularized fibular autograft has many advantages for mandible reconstruction and represents the first choice for the head and neck surgeon.

Abstract:Purpose: The vascularized free fibula flap has become the most popular reconstruction method after mandibular resection because of adequate bone graft length and acceptance of dental implants. However, using 1 fibula bone may produce a height discrepancy between the native mandible and the grafted fibula that results in subsequent difficulty in wearing conventional dentures or osseointegrated implants. Several options can be used to resolve this problem such as delayed onlay bone graft, iliac bone reconstruction, fibula distraction, and double-barrel fibula flap graft. This article describes the reconstruction of segmental mandible defects with the double-barrel fibula flap and denture rehabilitation.Materials and Methods: This procedure was used in 7 patients. A donor site fibula corresponding to at least twice the length of the mandibular defect was harvested. Double-barrel free fibula grafting was performed in 6 patients for primary reconstruction and 1 patient for secondary reconstruction, including 3 cases of osteocutaneous flap with skin islands in the reconstruction of a composite defect from a malignant tumor. Prosthodontic treatment was completed in all 7 patients. Four patients received secondary implant-supported dental reconstruction, and 3 patients who received radiation (6,000 cGy) after graft surgery had conventional removable partial dentures.Results: Microvascular fibula transfers were completely successful, and all skin paddles survived without necrosis. The original mandibular contour was maintained by a reconstruction plate; the reconstruction mandibular length was 6.5 to 10 cm, the reconstruction height of the double-barrel fibula was 3.0 to 3.8 cm, and all patients were satisfied with the postoperative facial esthetics and chewing function from the implant-supported denture and removable partial prostheses.Conclusions: Mandibular segmental defects can be esthetically and functionally reconstructed by a double-barrel vascularized fibula flap that not only matches the height of the native mandible but also allows osteointegrated dental implantation for dental rehabilitation. Abstract:Background: This retrospective study aims to report an algorithm to assist surgeons in selecting different modes of the double-barrel vascularised fibula graft for mandibular reconstruction.Methods: A total of 45 patients who underwent reconstruction of mandibular defects with different modes of the double-barrel vascularised fibula graft were reviewed. Our algorithm for deciding on any one of the different modes for different mandibular defects is influenced by factors including history of radiotherapy, the length of mandibular body defect and the need to preserve the inferior mandibular border. Post-operative functional outcomes included diet type and speech, and aesthetic results gained at post-operative 2 years. Patients with implant-borne prosthetic teeth underwent assessment of their masticatory function.Results: There were four modes of mandibular reconstruction according to our algorithm, which included double-barrel vascularised fibula graft (n = 21), partial double-barrel fibula graft (n = 11), condylar prosthesis in combination with partial/double-barrel fibula graft (n = 11), and double-barrel fibula onlay graft (n = 2). Flap survival in all patients was 97.78%. Good occlusion, bony unions and wound closures were observed in 44 patients. Eleven patients received dental implantation in the transplanted fibula at post-operative 9-18th months. One patient wore removal partial dentures. For 11 patients with implant-borne prosthetic teeth, the average post-operative ipsilateral occlusal force was 41.5 +/- 17.7% of the contralateral force. Good functional and aesthetic results were achieved in 38 patients with more than 2 years of follow-up, including regular diet, normal speech and excellent or good appearance, especially for patients with dental rehabilitation.Conclusion: Good aesthetic and functional results can be achieved after dental rehabilitation by following our algorithm when choosing the different modes of double-barrel vascularised fibula graft for mandibular reconstruction. Because of the height discrepancy between the native mandible and transplanted fibula, it is difficult to wear conventional dentures or receive osseointegrated implants. The purpose of this study was to research if mandibular defect could be reconstructed with fibula flap and nonvascularised free fibula bone graft to get a better contour and high neo-alveolar bone. Methods: 17 patients received mandible reconstruction with fibula flap and free fibula bone graft. In the operation, fibula flap was used to reconstruct the mandible as the inferior portion or superior alveolar portion. The nonvascularised fibula bone was fixed to the fibula flap with titanium plate or screws to augment the height of fibula bone. The periosteum between vascularised and nonvascularised fibula was removed to get a better attachment between vascularised and nonvascularised fibula. Findings: The operation proceeded very smoothly in all 17 cases. The follow up time was 6 months to 24 months. All patients got a good appearance, and the bone height improved 5–15 mm compared with conventional one-strut type technique. Conclusions: It is a good choice to reconstruct mandibular defect with fibula flap and nonvascularised free fibula bone graft to get a better contour and high neo-alveolar bone.
Functional reconstruction of mandibular tumor caused bone defects by vascularized double-barrel free fibular flap

Abstract:Objective To explore the application of vascularized double-barrel fibula flap by microsurgery in the appearance and functional reconstruction of tumor related mandibular defects.Methods Thirteen patients with mandibular defects caused by mandibular tumor expanded excision were enrolled and treated with vascularized double-barrel fibula flap microsurgery,to improve the appearance and chewing function of the mandibular.Results All 13 patients received the surgery successfully.The fibular flaps grew well after transplantation.Patients were satisfied with their facial appearance,articulation capability,as well as the chewing and occlusion function.No surgery complication was observed.Follow-up examination at 6 months after surgery showed good bone union between fibula segments and mandible.Dental implants were placed with great stability.No recurrence was found during the follow up.Conclusion Vascularized double-barrel fibular flap restores mandible morphology,alleviates surgery wound,reduces the failure rate of free graft,facilitates subsequent bone graft,and effectively improves the appearance and chewing function of the patients.

Abstract:PURPOSE:Osseointegrated dental implants in fibular free flaps can be used to reconstruct segmental mandibular defects resulting from head and neck resections. The double-barrel fibular flap (DBFF) has been applied as a modified method to overcome insufficient fibular width. This article describes the use of the DBFF with simultaneous dental implant placement as a superior method for esthetic mandibular reconstruction. MATERIALS AND METHODS:From 2012 to 2015, 26 patients underwent mandibular reconstruction with a free fibular flap and immediate dental implant placement after segmental mandibulectomies. Twelve patients received the DBFF and the other 14 patients received the conventional single-barrel fibular flap (SBFF). Palatal mucosal grafting was performed when necessary. Functional and esthetic outcomes were evaluated after 31 to 45 months of follow-up. RESULTS:All microvascular fibula transplantations were successful. All patients completed prosthodontic rehabilitation. The mean follow-up of patients after reconstruction was 36.3 months (range, 31 to 45 months). The facial esthetic score was significantly higher in patients treated with the DBFF than with the SBFF after 24 and 30 months (P < .05). There was no significant difference in marginal bone resorption between the DBFF and SBFF groups (P > .05). CONCLUSIONS:This 1-stage surgical method is safe and reliable. Use of the DBFF markedly decreased the height discrepancy between the native and new mandible and achieved a better lower face esthetic outcome than the SBFF. Osseointegrated dental implantation was adequate to achieve satisfactory dental rehabilitation in the DBFF group.

Abstract:Purpose: The double-barrel fibula flap and vascularized iliac crest flap are both commonly used for mandibular reconstruction. The present study compared the usage and reconstruction outcomes of transplanted bone with these 2 methods. Patients and Methods: The data from 30 patients who had undergone mandibular osteotomy and reconstruction were retrospectively reviewed. Of the 30 patients, 20 received a vascularized iliac crest flap (group A) and 10 received a double-barrel fibula flap (group B). The following variables were compared between the 2 groups: volume of bone flap (VBF), volume of effective bone flap (VEBF; ie, overlap between the volume of the ideal mandible [VIM] and the VBF), usage of the bone flap (VEBF divided by the VBF), mandibular reconstruction rate (VEBF divided by the VIM), volume of needless bone flap (VNBF; ie, VBF minus VEBF; the VNBF included the volume of needless buccal bone flap [VNBBF] and the volume of needless lingual bone flap [VNLBF]), percentage of alveolar crest restoration (PACR; ie, effective bone flap width divided by ideal alveolar crest width), and height of the bone flap (HBF). The independent-samples t test and the chi(2) test were used to compare the variables between the 2 groups. Statistical significance was at P <= .05. Results: Usage of the bone flap and the length of the mandibular defect were significantly greater in group B than in group A (P =.039 and P <.001, respectively). The VBF, VNBF, and VNLBF were significantly greater in group A than in group B (P <.001 for both). The mandibular reconstruction rate, VNBBF, PACR, HBF, and tooth implantation rate were comparable between the 2 groups. Conclusions: The double-barrel fibula flap can effectively restore the height of the alveolar crest, reconstruct longer mandibular defects, and provide a better buccal and lingual appearance compared with the vascularized iliac crest flap. Although the vascularized iliac crest flap can provide sufficient bone quantity, it must be contoured to the mandible. (C) 2020 American Association of Oral and Maxillofacial Surgeons.

Abstract:Mandibular and maxillary reconstruction using vascularised bone flap is now commonplace. Sometimes, reconstruction of complex oral–maxilla–facial defect poses a challenge to surgeons because of functional and aesthetic problems. Here we present the first case that received rehabilitation of whole upper alveola, palate and one-side mandibular body defect with single vascularised and non-vascularised fibula after operation of synchronous primary cancers in upper and lower gums and palate.
Mandibular Reconstructions Using Multiple Grafts:Radiologic Follow-Up Observation

Abstract:Objective Multiple grafts had been used to reconstruct mandibular defects. This study was to evaluate the effects of these reconstructions.Methods The radiological characteristics of 67 cases mandible defects (from 1974 to 2004 ) were analyzed retrospectively, compared with effects of all grafts.Results Sixty-seven cases were reconstructed and followed up after average 2 years. Twelve cases used vascularized free fibular flap, 3 out of them received primary insertion of osteointergrated dental implants, all the 5 implants survived. Ilium 15 cases, 1 infected. Rib 28 cases, 7 infected, 3 recurrences, 1 broken and then malunioned. Polymethyl methacrylate plastics 2 cases, both infected. Hydroxyapatite 6 cases,particulate cancellers bone and marrow grafts with titaniumesh 1 case; titanium plate 2 cases; compound transplant 1 case.Conclusion Ribs,iliac bone and fibular flaps are the modern major method for reconstruction of mandibular defects.Vascularized free fibular flap takes its distinct advantages to other autogeneous bone free flaps, and is conformed to be one of the best methods for mandibular reconstruction.
Mandibular Reconstruction With Single Barrel Vascular Free Fibula Flap And Implants-Borne Fixed Prosthesis: Usage Of A Modified Healing Abutment

Abstract:Recent developments in computer-aided surgical planning and manufacturing provide a promising solution to large-scale maxillofacial reconstruction. However, proper dental prosthetic rehabilitation based on osseointegrated implants in cases with single barrel fibula flap is still a challenge. We report one patient with recurrent mandibular ameloblastoma underwent large-scale mandible resection and reconstruction with single vascular free fibula flap. By using computer-aided surgical planning, 6 implants were precisely placed on the fibula graft with limited bone quantity. To address soft tissue hypertrophy and recurrent mucositis, we fabricated a computer-aided design and computer-aided manufactured (CAD/CAM) screw-retained tooth-like healing abutments, which efficiently helped the transepithelial tissue formation of the implants. Finally, the patient was restored using a screw-retained fixed prosthesis with CAD/CAM titanium frame. A 24-month follow-up showed that all implants were in normal function with no obvious peri-implantitis or prosthesis failure. Although further investigation on this technique might be needed, this report illustrated the successful treatment strategy of a computer-aided mandibular reconstruction with single barrel vascular free fibula flap and implants-borne fixed prosthesis.
Comparative Analysis of Dental Implant Treatment Outcomes Following Mandibular Reconstruction with Double-Barrel Fibula Bone Grafting or Vertical Distraction Osteogenesis Fibula: A Retrospective Study

Abstract:PurposeThe purpose of this study was twofold: (i) to compare vertical bone height (VBH) after tumor resection through grafting with either a double-barrel fibula (DBF) technique or vertical distraction osteogenesis of the fibula (VDOF); (ii) to compare the performance of loaded dental implants following either DBF or VDOF with special focus on implant survival, implant success, and bone resorption.Materials and methodsThis retrospective clinical study involved 19 patients who underwent implant placement following DBF (group A, n=9) or VDOF (group B, n=10) for mandibular reconstruction from March 2006 to May 2008. Clinical and radiographic assessments, including VBH, modified Plaque Index (mPI), modified Sulcus Bleeding Index (mSBI), and marginal bone level (MBL), were taken for both groups after delivery of the final prostheses and annually thereafter.ResultsNine patients underwent DBF with 24 implants placed and 10 patients underwent VDOF with 27 implants placed for mandibular reconstruction after tumor resection. Overall, all DBF and VDOF procedures were successful for group A and group B. VBH for group A and group B were 20 and 17mm. There was no statistically significant difference of mSBI scores between group A and group B in the 3-year follow-up (P=0.40). In four cases with eight implants of group A and two cases with three implants of group B, granulomatous soft tissue grew. There was no statistically significant differences of MBL between group A and group B in the 3-year follow-up (p=0.736). The cumulative survival and success rates of implants for group A were 100% and 87.5%, and for group B were 100% and 85.2% in 3-year follow-up, respectively.ConclusionsOn the basis of the study of 19 patients who received a total of 51 implants, reconstruction of the mandible with DBF flap or VDOF flap, combined with dental implant therapy, was considered a predictable option. Compared with implants placed in VDOF bone, implants placed in DBF bone had a relative higher incidence of associated gingival inflammation. The DBF bone seems more resistant to peri-implant resorption processes than VDOF bone during functional loading.
Comparative evaluation of personalized 3D-printed scaffold-driven double-barrel fibula flap for the reconstruction of segmented mandibular defects

Abstract:The challenge of reconstructing large mandibular bone defects with double-barrel fibula flap (DBFF) is limited by the availability of autogenous bone, which remains a significant issue in maxillofacial surgery. This study aims to compare the novel scaffold-driven double-barrel approach with the DBFF method, and investigate the biomechanical behavior of 3D-printed porous titanium scaffolds with varying design parameters. The mandibular reconstruction model underwent FEA in two surgery phases, under two experimental loads and two design parameters. The experimental scaffold design was categorized into 9 groups based on the porous diameters (D P =4mm, 5mm, 6mm) and strut diameters (D S =0.4mm,0.5mm,0.6mm). The results illustrated that the stress in fibula, cortical and trabecular bones for all groups were within the ranges of normal physiological levels. The porous scaffolds effectively transmit mechanical stimulation, resulting in bone graft materials achieving up to 99.9% strain, with low stiffness to avoid stress shielding. The study revealed that the design parameters of Group B and Group F were more conducive to meeting biomechanical demands and promoting bone regeneration. This study presents scientifically proven and effective clinical solution for restoring both function and aesthetics of mandibular defects, while also providing valuable insights for future design criteria in bone regeneration scaffolds.

Abstract:Fibula transplantation plays an irreplaceable role in restoring the function and morphology of the defected mandible. However, the complex load-bearing environment of the mandible makes it urgent to accurately reconstruct the mandible, ensure the position of the condyle after surgery, and restore the patient's occlusal function and contour. The intervention of digital design and three-dimensional (3D) printed titanium mesh provides a more efficient method and idea to solve this problem. Digital design guides the accurate positioning, osteotomy, and simultaneous implant placement during surgery, and 3D printed titanium mesh ensures stable condyle position after surgery, restoring good mandibular function. The double-layer folded fibula maintains the vertical height of the mandible and a good facial contour, and simultaneous implant placement can establish a good occlusal relationship. This study conducted a retrospective analysis of five patients with jaw defects who underwent digital fibula reconstruction over the past 3 years. It was found that the surgical protocol combining digital design, 3D printed intraoperative guides, 3D printed titanium mesh, free fibula flap, immediate implant, and occlusal reconstruction to repair jaw defects had more ideal facial appearance and biological function. It will provide a more reliable surgical protocol for clinical management of large mandibular defects.

Biomechanical behavior of mandibles reconstructed with fibular grafts at different vertical positions using finite element method

Abstract:Background: For large mandibular defects, surgical reconstruction using microvascular fibular grafts has advantages over other alternatives in terms of blood supply and good quality of grafted bone. However, the fibular segment is usually lower in height than that of the original mandible, meaning that the vertical positioning of the fibular graft is variable, with different biomechanical consequences on the reconstructed mandible. Objectives: To use finite element method (FEM) to evaluate stress distribution and displacement of a reconstructed mandible versus an intact mandible under occlusal loads. Methods: A three-dimensional intact edentulous mandibular bone (Model I) and a reconstructed mandible bone with fibular graft were created from CBCT images. Calculation models were generated with fibular bone graft extracted from the reconstructed mandible of identical length placed into a mimicked defect area on the right-hand side of the mandible at three different vertical positions: superior (Model II), intermediate (Model III), and inferior (Model IV). Forces were applied at lower left first molar region and lower left central incisor area. Von Mises stresses and mandibular displacement were calculated as outcome measurements during loadings. Results: Maximum stress and strain within the reconstructed mandible were identified at the posterior border of the graft and the contralateral condyle. Maximum displacement occurred near the interface of fibular graft and anterior segment of the mandible. Stress distribution in the graft under functional loads is much higher than that in the residual mandibular segments from Models II to IV. The combined average maximum stress from anterior and posterior loads is 10.66 times higher in the mandible with inferiorly positioned graft (Model IV), 8.72 times for superior graft (Model II), and 3.68 times for intermediate graft (Model III) than that in the control group (Model I). The worst displacement result during functional loadings was in the group with fibular graft located at the inferior border of the mandible. Conclusions: The position of fibular graft placed in the surgical resection site has significant effects on the mechanical behavior of the reconstructed mandible. The fibular graft aligned with the inferior border of the mandible, the most common site designated location by clinicians, has the worst effects on the stress distribution and displacement to the mandibular under functional loads. The fibular graft placed at the intermediate location has the best biomechanics and provides favorable condition for subsequent prosthetic reconstruction.
