Clinical observation of combined use of naloxone and morphine in different ratio for postoperative patients epidual analgesia

Abstract:Objective To compare the efficacy and the side effects of morphine combined with naloxone in different ratio of the dose,which used as the presciptions of epidural analgesia in patients.Methods Ninety patients,classified as ASA physical status Ⅰ~Ⅲ,who were scheduled to undergo lower abdominal surgery and received a continuous and patient controll epidural analgesia(CEA+PCEA).These patients were randomly divided into three groups by different ratio of doses of naloxone combined with morphine,each group contained thirty patients.There were thirty patients in each group.Group A and Group B were respectively received naloxone and morphine with the ratio of the doses as 1 / 50 and 1 / 100,Group C received with morphine but no naloxone.The same dosage of morphine and Bupivacaine were in all the three groups.Pain,nausea and vomiting,pruritus,sedation,movement of legs and respiratory depression were observed at 0-48hr postoperatively in all patients.Results(1)The incidence and severity of nausea and vomiting in group A was significantly less than that of group C at 3-36hr after operation(P <0.05).(2)The incidence and severity of pruritus in Group C was significantly heavier compared with Group A or B at 6-36hr postoperatively(P<0.05).(P<0.05).(3)There were no significance difference between the groups at mostly observing time in pain,sedation,the movement of legs and respiratory condition(P>0.05).Conclusion The ratio of 1 / 50 of naloxone combined with morphine in epidural analgesia can reduce the side effects(such as pruritus,nausea and vomiting)induced by morphine without affecting the analgesia,and the sedation,the movement of legs and the respiration are normal in all patients.
