The Relationship Between HBW Genotype and HBV-associated HCC

Abstract:Objective To investigate the molecular basis of the relationship between HBV genotype,and the HBV genotype of patients in Fujian province.Methods One hundred and twenty-four HCC patients and 124 controlled chronic HBV carriers were involved in the study.HBV genotypes were determined by restriction fragment length polymorphism analysis or sequencing.Clinical data of HCC patients infected by different HBV genotypes were also compared.X gene(HBx),enhancer II(Enh2) and basic core promoter(Bcp) of HBV in sera from HCC patients were amplified by nest PCR,then sequenced and compared with the sequences in Genbank.Logistic regression analyses was used to identify the variables of hepatitis B e antigen(HBeAg) and the amino acid changes in HBx,Enh2 and Bcp impacting on the relationship between HBV genotypes with HCC.Results The genotypes of HBV in all patients were genotype B and C.Genotype B and C in chronic HBV carriers were of 62.1% and 37.9%,while in HCC patients were of 41.5% and 58.5%.Average age of HCC patients infected by genotype B was about 50 years old which is similar to that in genotype C.The HBV genotype in HCC patients younger than 35 was mainly genotype B,whereas genotype C appeared more often in the group from 36 to 50 years old,and no significant difference in the infected HBV genotypes in the group of HCC patients elder than 50.Clinically,the serum total bilirubin level in HCC patients infected by HBV genotype B were higher than those in genotype C [(56.3±79.5)μmol/L vs.(23.7±18.3) μmol/L,P=0.015;and the rate of extrahepatic metastases of HCC(20.7% vs.0%,P=0.01].In the virology of hepatitis B,there was no difference in the level of HBV DNA between genotype B and C in HCC patients,however the positive rate of HBeAg in HCC patients infected by HBV genotype C was higher than that in genotype B(34.1%vs.9.74%,P=0.024).In HBx,Enh2 and Bcp,the nt1762/1764 double mutations were found in both the genotype B and C in HCC patients,while changes at amino acid 32,36,38,42,87,94,143 had significant differences.Conclusion The major genotype of HBV in HCC patients in Fujian Province was type C.E antigen,amino acid mutations in HBx,Enh2 and Bcp might not be the cause that genotype C of HBV leading to HCC.HCC patients infected by genotype B of HBV appeared easier to develop extrahepatic metastases,which is worthy to be further studied.
