Abstract:Editor, O ver the last three decades, there was a spectacular rise in the prevalence of type 2 diabetes mellitus (T2DM) in nearly every nation around the world (Chen et al. 2012). Among the diabetic population, visual impairment was not rare. In 2001, a population-based study found a 2.84% prevalence of significant visual impairment (<6/18) in 6482 diabetic residents in the North West of England, and the predominant (39.8%) cause of visual impairment in T2DM population was age-related macular degeneration (Prasad et al. 2001). In the Asian T2DM population, the prevalence rate of visual impairment was much higher. For example, a study conducted on the Hong Kong population found that 11.3% of 2301 T2DM subjects were significantly visually impaired (<6/18), and nearly 70% of the visual impairment can be corrected by prescription spectacles (Fung et al. 2010). Up till now, there have been no published data on the prevalence and causes of visual impairment and blindness associated with T2DM in mainland China. Therefore, from October 2014 to January 2015, we carried out a population-based, crosssectional survey among 2216 adult T2DM residents living in the Beixinjing community in Shanghai, China. The sampling frame was constructed using geographically defined clusters based on residential registration data, and according to the local Chronic Diseases Prevention and Control System, there were 2582 T2DM residents living in the 18 randomly selected geographical clusters. Fieldwork was carried out by two ophthalmological doctors, ten optometrists and four general health practitioners. Best-corrected distance VAwasmeasuredmonocularly using a retro-illuminated EDTRS logarithm of the minimum angle of resolution tumbling E chart at a distance of 4 m and at 1 m for those failing to read the top line (20/200). Blindness and visual impairment were defined based on World Health Organization criteria (International Classification of Diseases-10, Update and Revision 2006; World Health Organization 2014). Eyes presenting with BCVA <20/63 were assigned a principal cause of visual impairment/blindness using a 12-item list. If two or more diseases were present, the disease with the most significant and curable or reversible influence was assigned as the principal cause. A total of 2216 participants were finally examined, with an inclusion ratio of 85.8% (2216/2582). The mean (range) age of the examined population was 67.9 (30–97) years. The mean (range) duration of diabetes was 7.2 (0–40) years. The overall prevalence of moderate/severe visual impairment and blindness (BCVA <20/63 in the better eye) was 5.9% (132/2216). The lenses status of the 2216 participants was shown in Table 1. Among those with moderate/severe visual impairment and blindness, 75.8% (83/ 132) had cataract diagnosed as the principal cause of visual impairment. We calculated surgical coverage rate, which is defined as the ratio of number of cataract-afflicted people who have been operated on to the total visual impairment resident (<20/63) with cataract in one or both eyes (including those already had operation plus those not yet had operation) (Zhao et al. 1998). All bilaterally operated-on individuals (104 persons) plus all unilaterally operated-on individuals who were visually impaired in the fellow eye (46 persons) were included in the total of 186 operated-on cataract visual impairment. Of the visually impaired individuals, 95 had cataract as the principal cause in at least one eye. Surgical coverage rate among the cataract visual impairment was then calculated as low as 66.2% [(186/(186 + 95)) 9 100%]. We also calculated the cataract blindness burden and found the cataract blindness burden in the population observed was 12.7 persons per 100 subjects examined [((186 + 95)/2216) 9 100]. We had expected T2DM residents in Shanghai, an internationally renowned metropolis, would use eye-care services, such as cataract surgery services, more frequently than the general population. However, there was a surprisingly large proportion of our subjects with impaired vision caused by cataract who do not take effective treatments. Special attention should be paid to ophthalmic examination, particularly for cataracts in patients with T2DM.
