Abstract:To investigate the feasibility and safety of percutaneous endoscopic gastrostomy (PEG) and percutaneous endoscopic jejunostomy(PEJ).From June 1996 to April 2005, clinical data of 121 patients treated with PEG or PEJ were analyzed retrospectively.A total of 121 patients experienced 134 times of PEG or PEG plus PEJ, including 90 cases (103 times) treated with PEG and 31 patients treated with PEG plus PEJ. Thirteen patients treated with PEG had fistula replacement 6-10 months after PEG. All patients had quick nutrition recovery after fistula tube insert,therefore parenteral nutrition was not required. No severe complications occurred in all patients after 10 months of following-up. Four patients had subcutaneous infection around fistula 4-10 days after PEG.PEG and PEJ are safe and new methods for gastrointestinal decompression and enteral nutrition, which can be substitutes for nasogastric tube.

Abstract:Objective To study the clinical value of percutaneous endoscopic gastrostomy(PEG) and percutaneous endoscopic jejunostomy(PEJ).Methods 97 patients underwent PEG and 34 patients underwent PEG plus PEJ from June 1996 to April 2006.Results All the 131 patients received 146 times of PEG or PEG plus PEJ.The tubes of 15 patients who were treated with PEG were replaced for malfunction.All cases were succeeded.Subcutaneous infection around the tube occurred in 5 patients.Nutritional status were progressively improved and subsequently led to the withdrawal of total parenteral neutrition.Conclusions PEG and PEJ are safe,low-cost new methods for gastrointestinal decompression and enteral nutrition,presenting as a approach to replace the nasogastric tube feeding.

Abstract:Objective:To investingate the usage of percutaneous endoscopic gastrojejunostomy(PEGJ) in critically ill patients.Methods:From July 2002 to October 2004,21 critical ill patients received percutaneous endoscopic gastrojejunostomy,including 14 males and 7 females.The duration,successful rate,and complications of this procedure were recorded.Results:Tubes were successfully placed in all cases within an average time of(14.3±8.6)min.No procedure related complications were observed.One case of tube displacement(4.7%) and another case of leakage around tube site(4.7%) were found. The working time of the tubes were(235.7±209.3)d.Conclusion:Gastrojejunostomy tubes placed via the percutaneous endoscopic route were easily handled,effective and safe.The endoscopic method of placing feeding tubes can be performed at the bedside and allows the gastric decompression and enteral feeding to be rapidly and efficiently achieved,which is particularly useful for intubated patients in the intensive care settings.

Abstract:Objective To discuss the clinical value of percutaneous endoscopic gastrostomy(PEG).Methods 8 patenets were treated with PEG from Jan.to Dec.2006.Results All the 8 cases were successful.All patients recovered successfully after PEG.None servere complications were found during 3 months follow-up.Conclusion PEG is a new method for gastrointestinal decompression which can replace nasogastric tube.It is a convenient and safe method,and can be easily accepted by patients and is worth to be generalized.

Abstract:Objective To summarize the clinical experience of Peng's binding pancreaticojejunostomy,and compare the effect of type Ⅰand type Ⅱ binding pancreaticojejunostomy.Methods From Jan.2000 to Jan.2003,type Ⅱ binding pancreaticojejunostomy was performed on 69 cases and type I binding pancreaticojejunostomy was performed on 57 cases.The cases were all followed up for 1～12 month and ultrasound was used to examine the leakage.Comparison was made between two groups to find out advantages of two types of binding pancreaticojejunostomy.Results The operations of all cases were successful.There was no anastomotic leakage in both two groups. Average time for anastomosis was 16 min and 27 min respectively.Conclusion Type Ⅱ binding pancreaticojejunostomy is as safe as type Ⅱ,but type Ⅱ is simpler and timesaving.Furthermore the stoma can be checked up as soon as anastomosis wasaccomplished in type Ⅱ.
Percutaneous Endoscopic Gastrostomy/jejunostomy: an Effective Method for Home Enteral Nutrition Support

Abstract:Objective:To investigate the use of percutaneous endoscopic gastrostomy / jejunostomy(PEG/J) in patients on home enteral nutrition(HEN).Methods:From January 2004 to March 2007,36 patients received percutaneous endoscopic gastrostomy / jejunostomy for HEN,including 21 males and 15 females.The duration,success rate,complications and effectiveness of this procedure were analyzed.Results:The tubes were successfully placed in 100% of the cases within an average of(6.4±2.1) min in PEG and(15.7±4.2) min in PEJ.No procedure-related complications were observed except some minor ones in 2 cases(5.6%).Significant improvement was noted in the weight,body mass index,plasma protein and quality of life of the patients after 2 month HEN support.Conclusion:Gastrostomy / jejunostomy via the percutaneous endoscopic route,easy to perform and safe in use,are effective methods HEN support.
PEG--A New Way of Enteral Nutrition Feeding

Abstract:Objectives: To evaluate the value of percutaneous endoscopy gastrostomy(PEG) in nutrition support of neurosurgical and neurological patients. Methods: Thirty patients had undergone PEG.HXResults: Nutritional status was improved in the survival patients and the lung infections were controlled.Seven patients recovered with oral intake and 10 patient were discharge from hospital with the feeding tube for further rehabilitation,3 patients died from the deterioration of the original disease.Two cases had the inflammation at the PEG wound but easily treated.No other complications of PEG occured. Conclusions: PEG is safe and effective for enteral nutrition in some neurosurgical and neurological patients.

Abstract:Objective: To investigate the application of percutaneous endoscopic gastrostomy (PEG) to patients with severe craniocerebral injury for the purpose of nutritional support therapy and pulmonary infection prevention.Methods: A total of 43 patients with severe craniocerebral injury admitted to our department from January 2008 to December 2012 received PEG followed by nutritional therapy. There were other 82 patients who were prescribed nasal-feeding nutrition. Nutrition status was evaluated by comparing serum albumin levels, and the incidence of pulmonary infection 1 week before and 2 weeks after operation was identified and compared.Results: Both PEG and nasal-feeding nutrition therapies have significantly elevated serum albumin levels (P<0.05). Serum albumin levels before and after nutritional therapies showed no significant difference between the two groups (P>0.05). The incidence of pulmonary infection in PEG group was significantly decreased compared with that in nasal-feeding nutrition group (P<0.05).Conclusion: PEG is an effective method for severe craniocerebral injury patients. It can not only provide enteral nutrition but also prevent pulmonary infection induced by esophageal reflux.

Abstract:Objective: The purpose of this study was to report the clinical experience of direct percutaneous endoscopic jejunostomy(DPEJ) applied in advanced carcinosis intestinal obstruction patients for decompression and drainage. Methods: Three gastric cancer patients suffered from advanced carcinosis intestinal obstruction after total or subtotal gastrectomy, and received DPEJ for palliative intestinal decompression and drainage. DPEJ were performed by classical pull technique as PEG. Results: DPEJ were all done successfully without serious complication, and can effectively drain the intestinal fluid about (500-)1000 ml/d. The life quality of advanced cancer patients was improved by DPEJ for symptom palliation of intestinal obstruction. Conclusion: DPEJ is more difficult than PEG in technique, but also can be well done by careful and correct perioperative management, and is a safe and effective method for palliative intestinal decompression and drainage.
Direct endoscopic placement of percutaneous endoscopic gastrostomies with jejunal extension tubes (PEG-J) using ultra-thin endoscopes - long-term outcomes from a U.S. tertiary referral center

Abstract:Introduction: Several techniques for PEG-J tube placement have been described, commonly requiring fluoroscopic guidance and/or fixation of the jejunostomy tube (J-tube) into the small intestine. We describe a modified technique for placing jejunostomy tubes under direct visualization through a PEG with the use of ultra-thin endoscopes and steel guidewire. Methods: A retrospective study at a single tertiary academic center evaluating patients who underwent PEG-J placement between 2010 and 2020. All PEG tubes were placed with a pull-through technique. The Olympus GIF-N180 endoscope was advanced through the PEG to the jejunum and a Savary-Gilliard guidewire was used for placement of the J-tube extension. Results: Fifty-eight patients underwent PEG-J placement (median age 61 years; women 52%). Surgically altered gastric anatomy was observed in 11 patients (19%). Median procedure time was 44 min for new PEG-J tube placement (range 26-103) and 20 min for placement of a J-tube extension through an existing PEG tube (range 9-86) or gastrostomy tract. Technical success rate was in 100%. Sixty-two repeat procedures were performed for J-tube exchange in 27 patients (46%, range 1-9 per patient), of which 51 procedures (82%) were done using the same technique. The most common indication for tube replacement was tube dysfunction (63%, n = 39). The median procedure time for tube exchange was 20 min (range 2-62). No major adverse events were encountered. Conclusion: PEG-J tubes can be placed effectively, rapidly, and safely using an ultra-thin caliber endoscope and a stiff steel wire through the PEG tube or mature gastrostomy site, precluding the need for fluoroscopy or oral access. J-tubes can be easily replaced utilizing the same technique.
Clinician's attitude to enteral nutrition with percutaneous endoscopic gastrostomy: a survey in China

Abstract:Background: Percutaneous endoscopic gastrostomy (PEG) is recommended for long-term enteral nutrition. However, long-term nasogastric (NGT) feeding is still commonplace in China. We surveyed Chinese clinicians' opinions toward PEG feeding in order to identify the potential barriers to acceptancy of PEG feeding. Methods: A self-reported questionnaire was developed and distributed to 600 doctors. Five-point Likert scales were used for most responses. Results: Of 525 respondents, the mainly nutritional support method was NGT while PEG was less used. Doctors working in the tertiary class A hospitals and radiotherapy department were more likely to choose PEG feeding (p = 0.000). Overall, 241 (46%) participants did not know PEG and 284 (54%) have different understanding degree of PEG. Age (p = 0.002), working life (p = 0.044) and professionalism (p = 0.005) were significantly related to the understanding of PEG. Levels of agreement was high (score of 3.47) for using PEG in patients with prolonged stroke-associated dysphagia. There was high agreement level in the statement that PEG was unnecessary when NGT could sustain the basic needs of patients, though better outcome can be predicted with PEG feeding. The highest scoring factor (score of 3.91) that influenced clinicians' choice of PEG was resistance from patients and families and the second one was the poor cooperation among departments (score of 3.80). Conclusions: Doctors' insufficient knowledge of PEG feeding, resistance from patients and families, poor cooperation among departments, all these factors leading physicians to prefer more conservative treatment to avoid disputes rather than better ones.
Percutaneous endoscopic gastrostomy in a general hospital: prospective evaluation of indications, outcome, and randomised comparison of two tube designs

Abstract:The indications for percutaneous endoscopic gastrostomy (PEG) and patient outcome, were examined prospectively in the setting of a general hospital. In the course of 26 months, 76 patients underwent PEG (median age 62 years (range 18-99)) and were followed up for 6887 patient days. The median (range) duration of PEG feeding was 93 (3-785) days. The procedure was carried out for neurological indications in 76% of cases (stroke 51%) and 53% of patients were severely malnourished (body mass index < 17 kg/m2) at the time of referral. In 12 (16%) patients swallowing recovered and the PEG was removed after a median (range) of 55 days (20-150). Three (4%) deaths were related to PEG (one oesophageal perforation, one haemorrhage, and one aspiration pneumonia). One patient developed peritonism and ileus, which resolved with conservative treatment. Minor complications included local sepsis 3%, tube blockage 12%, and tube connector leak 5%. During seven days of observation, demands on nursing time for routine care of the PEG were the same as for nasogastric tube feeding, median (range) 21 (4-42) v 16 (4-40) min/day respectively, but in about half the latter cases the tube had to be replaced at least once. Over 15 months, 29 patients were randomised to receive a 1.9 mm inner, 2.9 mm (9F) outer diameter Fresenius and 27 a 3.0 mm inner, 4.0 mm (12F) outer diameter Bower polyurethane tube and were followed for 2920 and 2388 patient days respectively. There was no difference in the insertion time (median (range) 20 (10-45) v 24 (10-45) min respectively) or number of patients with complications (three v eight patients NS), although there were more minor mechanical problems (three v 12, p < 0.01) with the 12F tube. The internal anchoring device of the 12F tube allowed its non-endoscopic removal, a method applicable too 16% of cases. No tubes were removed because of blockage.
Acceptability and outcomes of the Percutaneous Endoscopic Gastrostomy (PEG) tube placement--patients' and care givers' perspectives

Abstract:Background: Percutaneous endoscopic gastrostomy tube has now become a preferred option for the long-term nutritional support device for patients with dysphagia. There is a considerable debate about the health issues related to the quality of life of these patients. Our aim of the study was to assess the outcome and perspectives of patients/care givers, about the acceptability of percutaneous endoscopic gastrostomy tube placement. Methods: This descriptive analytic study conducted in patients, who have undergone percutaneous endoscopic gastrostomy tube placement during January 1998 till December 2004. Medical records of these patients were evaluated for their demographic characteristics, underlying diagnosis, indications and complications. Telephonic interviews were conducted till March 2005, on a pre-tested questionnaire to address psychological, social and physical performance status, of the health related quality of life issues. Results: A total of 191 patients' medical records were reviewed, 120 (63%) were males, and mean age was 63 years. Early complication was infection at PEG tube site in 6 (3%) patients. In follow up over 365 +/- 149 days, late complications (occurring 72 hours later) were infection at PEG tube site in 29 (15 %) patient and dislodgment/blockage of the tube in 26 (13.6%). Interviews were possible with 126 patients/caretakers. Karnofsky Performance Score of 0, 1, 2, 3 and 4 was found in 13(10%), 18(14%), 21(17%), 29(23%) and 45(36%) with p-value < 0.001. Regarding the social and psychological aspects; 76(60%) would like to have the PEG tube again if required, 105(83 %) felt ease in feeding, and 76(60%) felt that PEG-tube helped in prolonging the survival. Regarding negative opinions; 49(39 %) felt that the feeding was too frequent, 45(36 %) felt apprehensive about dependency for feeding and 62(49%) were concerned about an increase in the cost of care. Conclusion: PEG-tube placement was found to be relatively free from serious immediate and long- term complications. Majority of caregivers and patient felt that PEG-tube helped in feeding and prolonging the survival. Studies are needed to assess the real benefit in terms of actual nutritional gain and quality of life in such patients.

Abstract:Objective: The purpose of this study was to report perioperative application of percutaneous endoscopic gastrostomy(PEG) in 2 radiative enteritis patients. Methods: Two radiative enteritis patients were resected main part of small intestine due to intestinal adhesion. They suffered from intestinal obstruction before operation and short bowel syndrome aftero peration. PEGs were performed for two patients at admission. PEG were used for gastro-intestinal decompression before operation and for long-term enteral nutrition after operation. Results: By PEGs gastro-intestinal fluid about 500-1 000 ml/d could be drained before operation and enteral nutrition about 2 092-5 020.8 kJ/d energy could be infused to gastro-intestinal tract after operation. Conclusions: During perioperation period, PEG could be used in some special patients for gastro-intestinal depression and long-term enteral nutrition.
[Percutaneous endoscopic gastrostomy (PEG) in pediatric patients: our experience and proposal of follow-up methodology]

Abstract:Objective: To evaluate effectiveness and acceptability of percutaneous endoscopic gastrostomy (PEG) in pediatric patients and to propose a data collection and follow-up methodology. Materials and methods: Observational retrospective and prospective study on 33 pediatric patients and 5 adults with PEG, placed using Gauderer "push" technique, between 2000 and 2007. By means of an appropriate questionnaire, the following parameters were evaluated: complications, factors of further risk, nutritional status, management and acceptability of PEG. Results: No problems occurred during placement. Complications were few and easy to resolve. In 3 patients a stomal dehiscence occurred, strongly related to the tube gauge. During replacement, in 4 patients, bumper was not taken away because of difficult removal. 8 patients had pre-PEG Gastroesophageal reflux: In 2 of them, during the PEG placement, fundoplication was realized. Subsequently PEG procedure, only 1 patient needed fundoplication for worsening of GER. All of them continued gastroprotective treatment. Respiratory tract infections decreased in our 13 patients carries of tracheostomy. Conclusions: To prefer smaller gauge reduces risk of dehiscence. If the bumper's removal is hard, to leave it inside is acceptable and quite safe, on condition of a careful surveillance of gastrointestinal obstruction signs. GER is not a contraindication of PEG. A careful follow-up is important, by recording all the evaluated parameters and by questionnaire to the family, during every hospital admission. This study, even if on few patients, confi rms PEG as the technique of choice for long-term enteral feeding, also in children. Training of family and caregivers is important to care.
Percutaneous endoscopic gastrostomy for critically ill patients in a general intensive care unit

Abstract:Background and aim: percutaneous endoscopic gastrostomy (PEG) is an effective way of providing enteral feeding to patients with functionally normal gastrointestinal tract who cannot meet their nutritional needs because of inadequate oral intake. This retrospective study evaluated the clinical outcome of critically ill patients with high assistance level undergoing PEG in a general ICU over a 12 year period. Patients and methods: we studied a cohort of 82 patients who underwent PEG over a 12-year period between 1 January 1999 and 31 December 2010. Patients were followed-up for 1 year after PEG placement. Results: There were no complications related either to the procedure or to the management of PEG, even in house nursing.In one patient, PEG with a collapsible bumper was successfully removed because the patient fully recovered from his neurological problem. Catheter substitution was necessary in three patients during the first year, because of stoma inflammation due to enteric reflux between the stoma and the catheter. One year after PEG, 66 patients were still alive while 16 patients died from the underlying disease during hospitalization. None of the patients with PEG had aspiration pneumonia. Conclusions: PEG, in expert hands, is a safe and effective procedure for enteral nutrition. Moreover, catheters should be chosen in relation to the duration of enteral feeding and as to whether the patient is likely to recover from his underlying disease.
Percutaneous endoscopic gastrostomy (PEG): retrospective analysis of a 7-year clinical experience

Abstract:Aims: Since its description in 1980, percutaneous endoscopic gastrostomy has become the modality of choice for providing enteral access to patients who require long-term enteral nutrition. This study aimed to evaluate current indications and complications associated with PEG feeding. Methods: We conducted a retrospective analysis of all patients who referred to our endoscopic unit of the Department of Gastroenterology and Hepatology of the Medical Center University of Sarajevo for PEG tube placement over a period of 7 years. Medical records of 359 patients dealing with PEG tube placement were reviewed to assess indications, technical success, complications and the need for repeat procedures. Results: The indications for enteral feeding tube placement were malignancy in 44% (n=158), of which 61% (n=97) patients were suffering of head and neck cancer and 39% (n=61) of other malignancy. Central nervous disease was the indication in 48.7 % (n=175) of patients. Cerebrovascular accidents (CVA) accounted for 20% (n=73), head injury for 16% (n=59) and cerebral palsy for 11% (n=38). In 6.13% (n=22) of patients minor complications occur which included wound infection (0.8%), inadvertent PEG removal (2.5%) and tube blockage (1.1%). 11 patients experienced major complications including hemorrhage, tube migration and perforation. There were no deaths related to PEG procedure placement and the overall 30-day mortality rate due to primary disease was 15.8%. Oral feeding was resumed in 23% of the patients and the tube was removed subsequently after 6 -12 months. Conclusions: Percutaneous endoscopic gastrostomy is a save and minimally invasive endoscopic procedure associated with a low morbidity (9.2%) rate, easy to follow-up and to replace when blockage occurs. Over a seven-year period we noticed an increase of 63% in PEG placement at our department.

Abstract:Percutaneous endoscopic gastrostomy (PEG) tubes offer alternative enteral nutrition routes for patients unable to orally ingest adequate nutrition. There are 2 primary types of PEG tubes: standard, which provide better drainage, and low-profile, favored for patient comfort. Internal anchoring mechanisms include balloon, capsule domes, and capsule monarchs. In cases of gastric dysfunction, PEG tubes modified with jejunostomy attachments (PEG-J tubes) are used. If inadequate, percutaneous endoscopic jejunostomy (PEJ) tubes are considered. Adaptation of PEG tubes for jejunal placement is common practice, with careful consideration of luminal diameter differences between the stomach and small bowel essential to minimize complications.

Abstract:Direct percutaneous endoscopic jejunostomy (DPEJ) provides post-pyloric enteral access in patients unable to meet long-term nutritional needs per os in situations where gastric feeding is neither tolerated nor feasible. Specific conditions associated with feeding intolerance due to due to nausea, vomiting, or ileus include gastric outlet obstruction, gastroparesis, or complications of acute or chronic pancreatitis; infeasibility may be due to high aspiration risk or prior gastric surgery. Since performing DPEJ is not an ACGME requirement for GI fellows or early career gastroenterologists, not all trainees are taught this technique. Hence, provider expertise for teaching and performing this technique varies widely across centers. In this article, we provide top tips for successful performance of DPEJ.
