Abstract:Objective To explore the surgical strategy and effects for treating complicated malignant tumors of the vena cava or/and the tumors extending into right atrium/ventricle under the cardiopulmonary bypass (CPB) and review of literatures.Methods Between Dec.2004 and Jul.2008,surgical procedures for treating 7 patients with tumors of inferior vena cava(IVC)or the tumors extending into right atrium/ventricle under the condition of deep hypothermia with CPB.One patient with recurrence of leiomyosarcoma of the IVC was performed successfully en bloc resection and caval reconstruction.One patient with tumor of mediastinal leaded to compression of the superior vena cava(SVC),resection of SVC and,partial of left and right brachiocephalic vein and right sided of pericardium,as well as resection of the primary carcinoma.The prosthetic graft was used for vena cava reconstruction in 3 patients and vascular patch in other 2 patients.Computed tomography or magnetic resonance imaging was used to exclude the presence of metastatic disease.Results The operative and post-operative course were uneventful and 8 patients were discharged,and one patient was died 2 months later due to functional disorder of liver.During a follow-up with 18(5～45) months,3 patients have been followed up 14～24 months with doing well after the surgical treatment.One patient was died 5 months later due to functional disorder of liver.The recurrence or metastases tumors in four patients after 5,4,11 and 32 months,respectively.One of these patients was unusual primary pleomorphic malignant fibrous histiocytoma (MFH) of IVC which was first reported for our knowledge.And this one recurred at 5-months postoperatively and was performed surgical procedures again 9-month postoperatively,then died of heart failure 2 months later.One patient with recurrence was performed chemiotherapy 7 months later,the metastasis tumor of brain in one patient with the tumor of mediastinal 11 months later and died 15 months later.One patient was lost 45 months later.Conclusions It is technically feasible that active surgical treating of primary malignant tumors of the IVC extending into right atrium without metastases tumors elsewhere.The appropriate planning and experienced surgical skill is very important,as well as multiple disciplines are also required,including vascular surgery,cardiothoracic surgery,and possibly,general surgical oncology.
