Functional MRI in the Diagnosis of Breast Carcinoma and Monitoring Early Response to Neoadjuvant Chemotherapy

Abstract:Objective:To assess the value of proton MR spectroscopy(1H-MRS),diffusion-weighted imaging(DWI) and MR first pass perfusion imaging(PWI) in the diagnosis of breast carcinoma and monitoring early response to neoadjuvant chemotherapy.Methods:Prospective study on the curing efficacy of pre-operative neoadjuvant chemotherapy treatment in a group of 19 patients with surgically and pathologically proven breast carcinoma was performed.The results of 1H-MRS,DWI and PWI were measured and comparatively evaluated before and after two cycles of relative treatments.Results:On DWI maps(b=1000s/mm2),the carcinoma lesions were detected as high signal intensity foci with low values of ADC [(1.12±0.03)×10-3mm2/s].After chemotherapy treatment,the mean ADC values of the 17 clinically relieved cases increased remarkably [(1.29±0.05)×10-3mm2/s] with a tendency of reduction in lesion size.The choline-containing compounds(Cho) might be detected and measured by 1H-MRS.After two cycles of relative treatments,the peak amplitude of cho in 10 of the 11 clinically relieved cases were shown decreased or disappeared.Time-signal intensity curve(TIC) of the breast carcinoma showed a negative perfusion one,in which the MR signal intensity decreased abruptly with scarce or without signal intensity recovery after a short and temporary platform in the perfusion phase.After the relative treatment,the TIC changed into a smooth and gradual sloping one.Conclusion:1H-MRS,DWI and PWI were capable of detecting and monitoring the curing efficacy of breast carcinoma after pre-operative neoadjuvant chemotherapy treatment.
