Abstract:We have read with interest the recent study by Le Nevé et al1Le Neve B. et al.Clin Gastroenterol Hepatol. 2016; 14: 226-233 e3Abstract Full Text Full Text PDF Scopus (34) Google Scholar that assessed visceral sensitivity and symptom severity in patients with irritable bowel syndrome (IBS) following a combined nutrient and lactulose challenge. The authors report that symptom severity assessed by validated questionnaires was significantly higher in patients with “high-intensity gastrointestinal (GI) symptoms” compared with those with “low-intensity GI symptoms” after the test meal. The high-intensity GI symptoms group also had high levels of anxiety and elevated Visceral Sensitivity Index scores (both P < .05). Furthermore, compared with the low-intensity GI symptoms group, patients in the high-intensity GI symptoms group had higher rectal sensitivity to phasic distentions administered by an electronic barostat (P < .05–.001). China is a country with a >95% prevalence of primary, genetic lactase deficiency6Yang J. et al.Clin Gastroenterol Hepatol. 2013; 11: 262-268 e1Google Scholar that has seen a large increase in milk consumption in recent years. This combination of genetic susceptibility and environmental challenge may have contributed to the increasing incidence of IBS documented in this population. The lactose hydrogen breath test is a simple, noninvasive investigation for the assessment of lactose absorption and tolerance. Additionally, the onset of symptoms was closely related to the timing of gas production and high levels of gas production increased the risk of abdominal symptoms (odds ratio, 2.19; P = .028). Also previous exposure to dairy products in the Chinese population may have promoted bacteria that metabolize lactose, whereas lactulose is not part of a normal diet (ie, increased sensitivity of H2 breath testing). Together these findings are consistent with the principle that both the size of the stimulus (eg, gas distention) and sensitivity to the stimulus contribute to symptom generation after FODMAP intake.
