Comparison of postoperative tear film and eye surface changes between 23 Gauge and 20 Gauge pars plana vitrectomy

Abstract:Objective To evaluate and compare the changes of postoperative tear film and eye surface between 23 Gauge and 20 Gauge pars plana vitrectomy. Methods Forty-two patients(46 eyes) underwent vitrectomy in our hospital from May to September 2011 were involved in this research,including 22 cases (24 eyes) who underwent 20 Gauge pars plana vitrectomy (20GPPV group) and 20 cases (22 eyes) underwent 23 Gauge pars plana vitrectomy (23GPPV group),and the chief complaint of dry eye symptom (DES),Schimer I test (SIT),tear break-up time(BUT),corneal fluorescent staining(CFS) scores were analyzed before operation and 3 days,7 days,14 days and 30 days after operation. Results The DES,SIT,BUT and CFS scores at postoperative 3 days in 20GPPV group were 2.95±0.94,(25.75±4.32)mm,(2.75±4.32)s and 6.43±2.38,respectively,which in 23GPPV group were 2.03±0.78,(17.67±5.24)mm,(5.89±5.76)s and 3.28±2.16,respectively;Compared 20GPPV group,the patients in 23GPPV group suffered less DES,less CFS,shorter BUT,and produced more secretion of tears at postoperative 3 days,7 days and 14 days,there were statistical differences(both P<0.05).There was no statistical difference in SIT and CFS scores at postoperative 30 days between two groups(both P>0.05),however,there were statistical differences in DES and BUT between two groups(both P<0.05).Patients in 20GPPV group suffered more severe damage with tear film and surface of eyes than 23GPPV group,and the number of patients who accorded with dry eye diagnosis standard were more than 23GPPV group at postoperative each time point. Conclusion Compared with 20 Gauge pars plana vitrectomy,23 Gauge causes significantly less postoperative effect on tear film and eye surface.
