Abstract:Objective To explore the effectiveness of preoperative combination of D-dimer (DD) and intestinal fatty acid-binding protein (i-FABP) detection in early predication of strangulated intestinal obstruction (SIO) in children .Methods Sixty-five children with acute intestinal obstruction were recruited prospectively between May 2014 and April 2017 .For experimental group ,36 cases of SIO were confirmed intraoperatively and further divided into bowel necrosis (BN) group (n= 11) and non-bowel necrosis (non- BN) group (n = 25 ) based upon intraoperative bowel vitality . Twenty-nine non-SIO patients served as control group .The clinical data of demographic profile ,clinical manifestations and laboratory tests prior to surgical interventions were recorded and statistically analyzed . The serum levels of DD and i-FABP were retrospectively compared between those of SIO and non-SIO cases . The cut-off values for predicating strangulation were calculated with a receiver operating characteristic curve . Meanwhile , the sensitivity , specificity ,positive likelihood rate (+ RL) ,negative likelihood rate (-RL) and area under the curve of ROC were determined .Results Among 36 SIO children ,mesenteric hernia (n = 4 ) ,intussusception (n = 4 ) , intestinal volvulus (n= 2) and incarcerated hernia (n= 1) were diagnosed while incarcerated hernia (n= 8) , intussusception (n= 9) ,volvulus of intestinal adhesion (n= 4) ,volvulus of intestinal malrotation (n= 2) , greater omentum hernia (n = 1) and mesenteric band (n = 1) were confirmed intraoperatively .The serum levels of DD and i-FABP were significantly higher in SIO patients with BN than those with non-BN [(1115 . 55 ± 469 .39) vs .(576 .20 ± 195 .22) μg/L] ,[(63 .75 ± 20 .72) vs .(21 .90 ± 10 .76) ng/ml , P< 0 .05] . The serum levels of DD and i-FABPwere significantly higher in SIO patients with non-BN than those with non-SIO [(576 .20 ± 195 .22) vs .(338 .07 ± 179 .53) μg/L ,(21 .90 ± 10 .76) vs .(13 .83 ± 9 .09) ng/ml , P< 0 .05] .On the other hand ,serum levels of DD and i-FABP were significantly higher in SBO children with SIO than those without [(741 .00 ± 390 .61) vs .(338 .07 ± 179 .53) μg/L ,(34 .69 ± 24 .17) vs . (13 .83 ± 9 .09) ng/ml ,P< 0 .05] .Using the DD cut-off value of 551 .50 μg/L ,the sensitivity ,specificity and AUC of ROC were 0 .69 ,0 .86 and 0 .86 respectively .Using the i-FABP cut-off value of 24 .01 ng/ml , the sensitivity ,specificity and AUC of ROC were 0 .58 ,0 .90 and 0 .81 respectively .When DD and i-FABP were employed for performing parallel combined diagnostic tests ,the sensitivity and specificity were 0 .87 and 0 .77 and + RL and -RL 3 .78 and 0 .17 respectively .Conclusions Preoperative serum levels of DD and i- FABP are feasible and useful biochemical markers for diagnosing strangulated bowel obstruction and gauging the severity of SIO in children .And a co-detection of DD and i-FABP improves the sensitivity and accuracy of diagnosing SIO .
