The treatment of femoral bone defect and selection of femoral prosthesis in the revision of total hip arthroplasty

Abstract:Objective To investigate the treatment of femoral bone defect and selection of femoral prosthesis in revision total hip arthroplasty. Methods Fifty five cases, including 16 males and 39 females, were studied from January 2002 to June 2007. The age was 30 to 89 years old, average 73 years old. Revision reasons: prosthetic loosening was 35 cases; femoral prosthesis subsidence was 4 cases; femoral prosthesis stem broken was 1 case; acetabular grinded breakthrough was 2 cases; periprosthetic femoral fracture was 3 cases; bone cortex perforated by femoral prosthesis stem was 3 cases; acetabulum no match with femoral head was 2 case; repeated relapse dislocation of hip prosthesis was 2 cases: local acetabular osteolysis was 3 case. femoral bone defect was found in 21 cases. According to the separate criteria of AAOS, type Ⅰ(segment bone defect) was 5 cases; type Ⅱ (lacouna bone defect) was 10 cases; type Ⅲ(mixed pattern bone defect) was 6 cases. Results Revision totalhip arthroplasty was done in 48 cases; primary femoral prosthesis were kept in 7 cases. Bone graft was done in 21 cases, in which, massive autogenous iliac bone graft+lengthen distal stable uncement prosthesis were done in 2 cases, massive allograft and autograft+lengthen distal stable uncement prosthesis were done in 1 case, and massive autogenous iliac bonegraft+lengthen cement prosthesis were done in 2 cases for segmental proximal femoral bone defect; mixed autograft and allograft+lengthen distal stable uncement prosthesis were done in 8 cases and lengthen cement prosthesis were done in 2 cases for lacunar proximal femoral defect; mixed autograft and allograft+lengthen distal stable uncement prosthesis were done in 4 cases and lengthen cement prosthesis were done in 2 cases for mixed pattern bone defect. All cases were followed up with 6 to 62 months, average 38 months. Compared with preoperative, the average Harris score increased from 49 to 88 points. ConclusionTo dislodge the cement prosthesis and repair the bone defect were the diffical work. Osteotomy was a effective method for dislodging cement prosthesis. Lengthen distal stable uncement prosthesis was suitable for most revision of femoral prosthesis.
