Abstract:Objective To follow up the patients with implantable cardioverter defibrillator(ICD) implanted,we aimed to observe the application of anti-tachycardia pacing between primary and secondary prevention of sudden cardiac death.Methods Patients scheduled for ICD implantation were enrolled from January 2005 until June 2009.All ICD interventions were registered and classified as ventricular tachycardia(VT),fast VT(FVT) and ventricular fibrillation(VF).Anti-tachycardia parameters were standardized,antitachycardia pacing(ATP) and shock therapy were provided in VT and VF zone.Follow-up was performed every 3-6 months after implantation,the stored arrhythmic episodes and anti-tachycardia therapy documentation were used to clarify the character of treated tachycardia.Results A total of 38 patients were enrolled in our study,18 in primary prevention group and 20 in secondary prevention group.Patients were followed up for(22.5 ± 17.8 ) months in primary prevention group and (30.2±21.9) months in secondary prevention group.During the follow up period,8 patients(44％ ) in primary prevention group had ventricular arrhythmic episodes which were terminated by ICD,among them,38 VT episodes(73％ ) were terminated by ATP.In secondary prevention group,12 patients (60％) had ventricular arrhythmic episodes which were terminated by ICD,among them,49 VT episodes (64％) were terminated by ATP.No significant difference was found in the incidence of VT and the efficacy of ATP therapy between two groups.However,the VT duration in primary prevention group was markedly shorter than that in secondary prevention group [ (24.6± 16.6) s vs.(78.1 ±58.7 ) s,P＜0.001].Conclusion The incidence of VT and the efficacy of ATP therapy were similar in patients with primary and secondary prevention of sudden cardiac death.ATP therapy,as a painless treatment of VT,is successful in the large majority of VT episodes.
