Abstract:Aim: To study the expression of matrix metalloproteinase-8(MMP-8) and its inhibitory factor-1(TIMP-1) in gingival tissue of patients with aggressive periodontitis(AgP).Methods: Immunohistochemistry method was used to detect MMP-8 and TIMP-1 protein in gingival tissue of 15 AgP patients and 20 healthy control.Results: MMP-8 and TIMP-1 were positively expressed in the intraepithelial and subcutaneous connective tissue in both groups. MMP-8 expression level and MMP-8/TIMP-1 of AgP group were significantly higher than those of the control group (Z=2.990 and 4.911,P0.05), but the difference in TIMP-1 expression between the 2 groups was not significant (Z=1.822,P0.05). ROC curve analysis showed that AUC(MMP-8)=0.752(95%CI=0.618-0.885,P=0.003), AUC(MMP-8/TIMP-1)=0.913(95%CI=0.831-0.996,P0.001),AUC(TIMP-1)=0.347(95%CI=0.194-0.499,P=0.068). Conclusion: MMP-8 expression level and MMP-8/TIMP-1 in gingival tissue could be helpful for the diagnosis of AgP.

Abstract:Aim:To analyze the roles of three collagenases (MMP-1,MMP-8 and MMP-13) in the pathogenesis of ag-gressive periodontitis(AgP).Methods:The expressions of MMP-1, MMP-8 and MMP-13 in gingival tissue of 30 patients with AgP and 35 healthy controls were detected by immunohistochemistry method .Results: Low expressions of MMP-1, MMP-8 and MMP-13 were shown in healthy control group .Higher expressions of MMP-1 and MMP-8 were found in the AgP group compared with those of healthy control group (Z=3.980 and 2.990, P<0.05).Conclusion: MMP-1 and MMP-8 may play an important role in the development of AgP .
Correlation between levels of porphyromonas gingivalis outer membrane protein A in subgingival plaques of chronic periodontitis and periodontal lesions

Abstract:Objective To detect porphyromonas gingivalis outer membrane protein A(PgmA) in subgingival plaques from chronic periodontitis patients and to elucidate the correlation between PgmA levels and periodontal lesions.Methods SDS-PAGE was used to monitor the recombined PgmA(rPgmA) expression of prokaryotic expression system and the purity of rPgmA extracted by Ni-NTA affinity chromatography.Rabbits were immunized with rPgmA to obtain antiserum.ELISA method was established to examine PgmA in 209 subgingival plaques from 56 chronic periodontitis patients and to analyse the correlation between PgmA levels and periodontal lesions.Results rPgmA output was approximately 50% of the total bacterial proteins.The extrated rPgmA after SDS-PAGE only showed a single fragment.90.0% of subgingival plaques(188/209)were ELISA PgmA positive.The PgmA levels in the subgingival plaque samples from advanced periodontitis were significantly higher than those in the samples from mild periodontitis(P<0.05).No difference of the PgmA level was found between the samples from mild/moderate periodontitis and the samples from moderate/advanced periodontitis(P>0.05).Conclusion The lesions of chronic periodontitis were associated with the subgingival PgmA level.PgmA might play a role in the pathogenicity of porphyromonas gingivalis.
[Analysis of Serum IgG Titers to Aggregatibacter Actinomycetemcomitans Serotype C in Aggressive Periodontitis Patients].

Abstract:OBJECTIVETo analyze the serum IgG titers to Aggregatibacter actinomycetemcomitans(Aa) and associated factors in patients with aggressive periodontitis (AgP).METHODSVenous blood samples were collected from 62 AgP patients and 45 periodontal healthy controls, unstimulated whole saliva and pooled subgingival plaque samples of AgP patients were also collected for the detection of Aa (PCR method). Serum IgG titers to Aa serotype c were measured by enzyme-linked immunosorbnent assay (ELISA).RESULTSThe detection rates of serum IgG to Aa serotype c in the AgP patients and the healthy controls were both 100%. The AgP patients exhibited significantly higher IgG titers to Aa serotype c than the healthy controls (11.1±1.9 vs. 9.1±1.8, P<0.01). There was no significant difference in serum IgG levels to Aa serotype c and in the prevalence of high-responding patients to Aa serotype c between the incisor-first molar type AgP patients and generalized AgP patients. Serum IgG titers to Aa serotype c in the Aa-positive AgP patients (the patients who were Aa-positive in subgingival plaque or saliva) were significantly higher than those of the Aa-negative patients (11.9±1.3 vs. 10.7±2.1, P<0.05).CONCLUSIONSerotype c was the main serotype of Aa in Chinese patients with AgP. Serum IgG responses in generalized AgP patients were comparable to those in incisor-first molar type AgP patients.
Titer and Avidity of Serum Antibody to Porphyromonas Gingivalis Before and after Periodontal Therapy

Abstract:OBJECTIVE:To observe the changes of titer and avidity of serum antibody to Porphyromonas gingivalis (Pg) before and one month after periodontal therapy, and to observe the correlation of titer and avidity of anti-Pg IgG with clinical indices. METHODS:ELISA and DEA-ELISA were used to determine the titer and avidity of serum IgG antibody to Pg381 before and one month after periodontal treatment in 17 periodontitis patients and 6 healthy control subjects. RESULTS:The titers of anti-Pg IgG antibody were higher in periodontitis patients than in healthy control subjects (P < 0.001). All patients were seropositive (titers to Pg were higher than 2-fold above the median for control subjects). IgG antibody avidity to Pg in periodontitis patients were not found to be significantly lower than that in control group. The titers to Pg decreased significantly at one month after periodontal therapy, but antibody avidity didn't change. Serum anti-Pg antibody titers had a tendency of negative correlation with probing depth. The antibody titers were significant higher in periodontitis patients with < or = 5 mm mean probing depth than in patients with > 5 mm mean probing depth. CONCLUSIONS:It is suggested that the humoral immune response to Pg381 is protective and this protective effect mainly depends on the titer of antibody.

Abstract:AIM: To investigate the titer and avidity of serum immunoglobulin G (IgG) antibody for surface antigens of Porphyromonas gingivalis (P. gingivalis) in sera from patients with early - onset periodontitis (EOP) and evaluate the interrelationship of the antibody titer and avidity. METHODS: 15 patients with early - onset periodontitis, 16 patients with adult periodontitis (AP) and 14 periodontally healthy subjects(HS) participated in this study. Levels and avidities of IgG antibody against P.gingivalis lipopolysaccharide(LPS) were measured. Serum IgG titers against P. gingivalis antigens were measured by enzyme -linked immunosorbent assay (ELISA). IgG avidity was measured by diethylamine dissociation ELISA. RESULTS: Serum IgG antibody levels against P. gingivalis LPS in EOP and AP patients were significantly higher than that of healthy subjects ( P ＜ 0.01 ),but there was no significant difference between early - onset periodontitis and adult periodontitis groups ( P ＞ 0.05). Antibody avidities to P. gingivalis in EOP were significantly higher than that of AP patients and HS group ( P ＜ 0.01). However,there was no significant difference in antibody avidity between AP patients and HS group (P ＞ 0.05). CONCLUSION: These results indicate that there is no correlation between antibody levels and avidities in EOP and AP patients. IgG avidity can be considered as a valuable parameter in the diagnosis of EOP.
Relationship of serum and gingival crevicular fluid antibody levels with the amount of subgingival homologous bacterium in patients with periodontal disease]

Abstract:39 patients were selected in this study, including 12 patients with rapidly progressive periodontitis (RPP), 17 with adult periodontitis (AP) and 10 with marginal gingivitis (MG). The levels of antibody to Porphyromonas gingivalis (Pg) in serum (Sr) and in gingival crevicular fluid (GCF) from their 156 teeth were measured by ELISA. The Pg in subgingival plaque was detected by indirect immunofluorescence technique. Data showed different GCF/Sr ratio of anti--Pg antibody level between periodontitis patients (lower than 1) and MG patients (greater than 1). The amount of subgingival Pg was negatively correlated with clinical periodontal parameters, i.e, probing depth, attachment loss, plaque index and bleeding index. It was suggested that low GCF/Sr ratio of antibody level plus high counts of homologous bacterium might be used as an indicator of periodontal disease activity.

Serum IgG titers to Aa serotype c in Aa-positive CP patients (Aa-positive in subgingival plaque or saliva, n=7) were significantly higher than those of Aa-negative patients [(12.6+/-1.6) vs (10.4+/-1.7), P=0.005)]. Serum IgG titers to Aa in CP patients tended to be positively related to mean probing depth of whole mouth (r=0.344, P=0.068). CONCLUSION:Serotype c was the main serotype of Aa in CP patients. Serum IgG antibody to Aa serotype c did not seem to have a protective effect against periodontal infection.
Systemic Inflammation Markers in Patients with Aggressive Periodontitis: a Pilot Study.

Abstract:Background: The association between periodontitis and systemic health is evident; however, until recently, there was a lack of scientific evidence to define the relationship between aggressive periodontitis (AgP) and systemic conditions. The aim of this study was to explore the characteristics of peripheral blood cellular and serum protein parameters in patients with AgP.Methods: Patients with AgP (n = 150) and healthy controls (n = 94) were recruited. Clinical parameters, including probing depth (PD), clinical attachment level (CAL), and percentage of severe sites, were examined. Blood cell variables, including leukocyte, neutrophil, and lymphocyte counts, as well as serum protein parameters, including total protein, albumin, globulin, and albumin/globulin ratio, were analyzed.Results: Elevated neutrophil numbers and serum globulin levels were observed in patients with AgP compared to controls (4.22 +/- 1.81 x 10(9)/l versus 3.20 +/- 0.91 x 10(9)/l and 29.20 +/- 3.75 g/l versus 27.17 +/- 3.32 g/l, respectively; P <0.01). Albumin level and albumin/globulin ratio were lower in the AgP group than in the control group (47.65 +/- 2.45 g/l versus 48.88 +/- 2.13g/l and 1.66 +/- 0.24 versus 1.83 +/- 0.24, respectively; P<0.01). In the AgP group, neutrophil counts and serum globulin levels were positively correlated with clinical parameters, including mean values for PD, CAL, and percentage of severe sites (P<0.05); the albumin level and albumin/globulin ratio were inversely correlated with the clinical parameters as described above (P<0.05).Conclusions: Patients with AgP may have elevated peripheral leukocyte numbers and serum globulin levels as well as decreased serum albumin levels and albumin/globulin ratios compared to controls. These changes might be associated with the severity of periodontal destruction. J Periodontol 2008; 79:2340-2346.
Blood Lipids and Glucose Levels in Patients with Periodontitis

Abstract:OBJECTIVE:To determine the serum levels of lipid and glucose in patients with periodontitis.METHODS:The patients with aggressive periodontitis (AgP, n = 117) and chronic periodontitis (CP, n = 40) and healthy controls (n = 37) were recruited. Peripheral blood sample was obtained from each subject by venipuncture. Serum chemistry variables including total cholesterol, triglycerides, and fasting glucose were analyzed by biochemical analyzer.RESULTS:The average serum levels of triglycerides and glucose of the patients with AgP [(1.09 +/- 0.79) mmol/L and (5.07 +/- 0.66) mmol/L] were both significantly higher than those of the control group [(0.94 +/- 0.28) mmol/L and (4.62 +/- 0.64) mmol/L], P < 0.05. Higher serum levels of total cholesterol and glucose were also observed in patients with CP [(4.61 +/- 1.23) mmol/L and (5.40 +/- 1.01) mmol/L] compared with controls [(4.49 +/- 0.78) mmol/L and (4.62 +/- 0.64) mmol/L], P < 0.05. Serum level of total cholesterol in AgP group was positively associated with percentage of severe sites (r = 0.25, P < 0.01).CONCLUSIONS:Patients with periodontitis may have altered serum lipid and glucose levels compared with controls.
[Prevalence of Putative Periodontal Microorganisms in Chinese Patients with Aggressive Periodontitis].

Abstract:OBJECTIVE:To evaluate the prevalence of putative periodontal microorganism in Chinese patients with aggressive periodontitis (AgP).METHODS:A total of 72 subgingival plaque samples were collected from 55 AgP patients and 17 healthy subjects. Seven putative periodontal microorganisms including Actinobacillus actinomycetemcomitans (Aa), Porphyromonas gingivalis (Pg), Tannerella forsythensis (Tf), Treponema denticola (Td), Campylobacter rectus (Cr), Prevotella intermedia (Pi) and Prevotella nigrescens (Pn) were detected by using 16S rRNA based polymerase chain reaction (PCR).RESULTS:The prevalence of Aa in AgP patients was very low (1.8%), while Pg, Tf, Td and Cr were more frequently detected in AgP patients (prevalence: 81.8%, 83.6%, 80% and 81.8%) than in healthy controls (prevalence: 17.6%, 11.8%, 5.9%, 29.4%, P < 0.01).CONCLUSIONS:The prevalence of Pg, Tf, Td and Cr were very high in aggressive periodontitis patients, and the combination of these bacteria may play an important role in AgP.
[Levels of Interleukin-1Beta and Tumor Necrosis Factor-Alpha in Patients with Aggressive Periodontitis].

Abstract:OBJECTIVE:To compare the levels of plasma IL-1beta,TNFalpha in aggressive periodontitis (AgP) patients and periodontally healthy subjects.METHODS:Venous blood samples were collected from 56 AgP patients and 39 periodontally healthy controls. Clinical examination consisted of bleeding index (BI), probing depth(PD)attachment loss measurements (AL) and percentage of severe sites. Plasma levels of IL-1beta and TNF-alpha were determined using a high-sensitivity ELISA assay.RESULTS:The concentrations of IL-1beta in AgP patients were significantly higher than those in healthy controls (7.23 ng/L vs 3.52 ng/L, P=0.003), which were positively correlated with BI, PD, AL and the percentage of severe sites (r=0.170, P=0.038; r=0.175, P=0.033; r=0.168, P=0.040; r=0.236, P=0.004). The difference between the levels of TNF-alpha in AgP patients and healthy controls was not observed (1.07 ng/L vs 1.06 ng/L, P=0.437). No correlations between the plasma level of TNF-alpha and the periodontal parameters were detected (r=0.023, P=0.808; r=0.025, P=0.782; r=0.075, P=0.407; r=0.049, P=0.586).CONCLUSION:AgP may have impact on the elevation of plasma level of IL-1beta, while no association between AgP and plasma level of TNF-alpha was found.

Abstract:Background and Objective: Vitamin D-binding protein (DBP) is a multifunctional and highly expressed plasma protein. Among its diverse roles, including those in the immune and inflammatory responses, it is the primary carrier of vitamin D, which has been implicated in periodontitis. We hypothesized that there is a correlation between systemic DBP levels and generalized aggressive periodontitis (GAgP). Material and Methods: Forty-four patients with GAgP and 32 healthy controls were recruited. Clinical parameters were examined, including the mean bleeding index, probing depth, attachment loss and percentage of severely affected sites. Blood chemistry analyses were performed for each subject. Plasma levels of DBP, interleukin-6 (IL-6) and procalcitonin (PCT) were measured using ELISAs, and plasma levels of 25-hydroxy-vitamin D3 (25(OH)D3) were detected using a radioimmunoassay. Results: Significantly higher levels of plasma DBP, IL-6, PCT and 25(OH)D3, as well as leukocyte counts, neutrophil counts and neutrophil percentages were found in patients with GAgP compared with healthy controls (p < 0.05 for all). Multiple linear regression analysis showed that the plasma DBP levels were significantly correlated with GAgP, plasma PCT levels and smoking status (p < 0.05 for all). In the GAgP group, the plasma DBP levels in smokers were significantly higher than those in nonsmokers (p < 0.001). Conclusion: Elevated plasma vitamin DBP levels are associated with GAgP.

Abstract:Systemic levels of C-reactive protein and interleukin-6 have been reported to be elevated in patients with periodontitis compared with periodontally healthy individuals. Most studies included patients with chronic periodontitis and comprised predominantly Caucasians. The aim of this study was to determine the relative levels of C-reactive protein and interleukin-6 in plasma of patients with aggressive periodontitis in China and to examine the relationships between these two inflammatory mediators and clinical parameters, peripheral blood cells and protein variables. Plasma samples were collected from 84 patients with aggressive periodontitis and from 65 control subjects. Periodontal examination consisted of taking probing depth and attachment loss measurements. The levels of plasma C-reactive protein and interleukin-6 were determined using enzyme-linked immunosorbent assays. The levels of plasma C-reactive protein in patients with aggressive periodontitis were significantly higher than those in control subjects (1.87 vs. 0.52 mg/L). The level of plasma interleukin-6 in patients with aggressive periodontitis was 1.20 pg/mL, higher than that in control subjects (0.08 pg/mL). Multiple linear regression analysis showed that log C-reactive protein was significantly related to severe sites percentage and albumin following correction for age, gender, body mass index and smoking (p = 0.000, p = 0.008, respectively). Log interleukin-6 was found to be significantly correlated with periodontal diagnosis, leukocyte count and level of fasting blood glucose after adjusting for the confounders (p = 0.000, p = 0.009 and p = 0.013, respectively). Patients with aggressive periodontitis have significantly elevated levels of plasma C-reactive protein and interleukin-6. These elevated inflammatory factors might potentially increase the risk for cardiovascular events and glucose dysregulation in relatively young individuals.

Abstract:Background Increasing evidence indicates that periodontitis contributes to systemic low‐grade inflammation. Porphyromonas gingivalis is strongly associated with periodontitis, and antibodies against the bacterium may be used as a serological proxy to account for periodontal status, when studying diseases associated with periodontitis. The aim of the present study is to identify an easily accessible and reliable serological biomarker for determination of periodontal status and oral carriage of the bacterium. Methods Saliva and serum samples were collected from periodontally healthy controls (n = 27), and patients with periodontitis stage II (n = 12) or stages III or IV (n = 44). Serum levels of immunoglobulin G (IgG) antibodies against intact and fragmented P. gingivalis, recombinant gingipains (RgpA and RgpB), and the bacteria Escherichia coli and Capnocytophaga ochracea as controls were quantified with a multiplex bead‐based assay. P. gingivalis was identified in saliva using quantitative polymerase chain reaction (qPCR). Results Serum IgG antibodies against P. gingivalis whole bacteria were good indicators of periodontitis (area under the curve [AUC]: 0.75, 95% confidence interval [CI]: 0.64–0.85). The same was observed for levels of antibodies against P. gingivalis fragments (AUC: 0.78, 95% CI: 0.68–0.88). Likewise, levels of antibodies against P. gingivalis whole bacteria or P. gingivalis fragments were good indicators of oral carriage of P. gingivalis (AUC: 0.92, 95% CI: 0.86–0.98 and AUC: 0.96, 95% CI: 0.92–1, respectively). Conversely, antibodies against recombinant RgpA and RgpB were not good indicators of periodontitis or oral carriage of the bacterium. None of the antibody levels differed significantly between stage II and stage III or IV periodontitis. Conclusion Serum IgG antibody levels against heat‐inactivated whole P. gingivalis proved to be the preferable biomarker for periodontitis and oral carriage of the bacterium.

Abstract:AIM: To evaluate the effect of mechanical periodontal treatment combined with tetracycline on per iodontal attachment level (AL) and the avidity of serum antibody against porphyromonas gingivalis( P. gingivalis) in patients with aggressive periodontitis(AgP). METHODS: Twenty- five patients with AgP and twenty periodontally healthy controls were studied (HS). Clinical examination and recordings of AL were performed before and 3,6 and 12 months after the periodontal treatment. The avidity of IgG antibody against P. gingivalis lipopolysaccharide(LPS) was measured by diethylamine dissociation ELISA. RESULTS: A significant improvement in AL was observed following treatment ( P ＜ 0. 01 ). The avidity of serum IgG antibody against P. gingivalis increased compared with controls, and was decreased significantly after mechanical periodontal treatment combined with tetracycline (P ＜ 0. 01 ). CONCLUSION: Ourresults demonstrate that mechanical periodontal treatment combined with tetracycline provides clinically favorable results in patients with AgP.

Abstract:In the present study, serum anti-Bacteroides gingivalis (Bg) and anti-Actinobacillus actinomycetemcomitans (Aa) antibody titers in RPP, AP patients and healthy subjects were determined by ELISA. Discriminant analysis for differential diagnosis of RPP, AP and healthy subjects was performed using clinical measurements (PD, AL, PII, BI) and serum antibody levels as combined parameters. The results showed significantly different serum antibody levels among the three examined groups. The results also suggested that the combined use of clinical parameters and serum specific antibody titers may be a valuable measure in differential diagnosis of periodontal diseases.

Abstract:Vitamin D-binding protein (DBP) is the main transport protein of vitamin D and plays an important role in the immune system and host defenses. The purpose of this study was to measure DBP levels in plasma and gingival crevicular fluid (GCF) of patients with generalized aggressive periodontitis (GAgP), in comparison to healthy controls, with the goal of elucidating the relationship between DBP and GAgP. Fifty-nine GAgP patients and 58 healthy controls were recruited for the study; clinical parameters of probing depths (PD), bleeding index, and attachment loss (AL) were recorded. DBP levels were measured by enzyme-linked immunosorbent assay. From the results, GAgP patients had higher plasma DBP concentrations (P < 0.001) but lower GCF DBP concentrations (P < 0.001) than healthy controls. In GAgP group, after controlling the potential confounders of age, gender, smoking status, and BMI index, GCF DBP concentrations correlated negatively with PD (P < 0.001) and AL (P = 0.009). Within the limits of the study, we concluded that decreased GCF DBP level and increased plasma DBP level are associated with periodontitis.

Abstract:Background The association between periodontitis and allergic symptoms has been investigated. However, the difference in immune signatures between them remains poorly understood. This cross-sectional study assessed the relationship between serum immunoglobulin G (IgG) antibodies to periodontal pathogens and allergic symptoms in a nationwide population cohort. Methods Two phases of the Third National Health and Nutrition Examination Survey (NHANES III) were used as discovery dataset (n = 3700) and validation dataset (n = 4453), respectively. Based on the antibodies against 19 periodontal pathogens, we performed an unsupervised hierarchical clustering to categorize the population into three clusters. In the discovery dataset, cluster 1 (n = 2847) had the highest level of IgG antibodies, followed by clusters 2 (n = 588) and 3 (n = 265). Data on allergic symptoms (asthma, hay fever, and wheezing) were obtained using a self-reported questionnaire. Survey-weighted multivariable logistic regression evaluated the association between these clusters and allergic symptoms. Results In the discovery dataset, the participants with lower levels of antibodies to periodontal pathogens exhibited a higher risk of asthma (odds ratio [OR](cluster 3 vs. cluster 1) = 1.820, 95% confidence interval [CI]: 1.153-2.873) and wheezing (ORcluster 3 vs. cluster 1 = 1.550, 95% CI: 1.095-2.194) compared to those with higher periodontal antibodies, but the non-significant association with hay fever. Consistent results were found in the validation dataset. Conclusions Serum IgG titers to periodontal pathogens were inversely associated with the risk of asthma and wheezing, suggesting the potentially protective role against allergic conditions.

Abstract:Background: Epidermal growth factor (EGF) is a pro-inflammatory small peptide that stimulates cell growth, proliferation and differentiation through binding to its receptor. EGF rs2237051 and serum EGF levels have been demonstrated to be related with a variety of diseases, including several tumors and inflammatory diseases. Therefore, this study aims to investigate the association of the EGF rs2237051 variant and serum EGF levels in Chinese patients with generalized aggressive periodontitis (GAgP). Material and Methods: A case-control study was conducted among 216 patients with GAgP and 138 healthy controls. The clinical parameters of plaque index, probing depth, attachment loss and bleeding index were recorded. The EGF rs2237051 polymorphism was genotyped using time-of-flight mass spectrometry, and serum EGF levels were determined. Logistic and linear regression models were used to investigate the association between the genotypes of EGF rs2237051, serum EGF levels and GAgP risk. Results: The AA genotype of EGF rs2237051 showed higher risk for GAgP than the combined genotypes GG and AG (adjusted OR = 1.65, 95% CI [1.06-2.57]). Increased serum EGF levels were associated with GAgP (adjusted OR = 1.18, 95% CI [1.14-1.22]). Moreover, the serum EGF level for the AA genotype was significantly higher than that for the AG/GG genotypes in patients with GAgP (adjusted beta = 4.70, 95% CI [2.09-7.31]). Conclusion: We demonstrated that EGF rs2237051 variant and the increased level of serum EGF were associated with the risk of GAgP, the serum EGF was up-regulated in patients with GAgP. It was indicated that serum EGF might be a biomarker of GAgP and EGF rs2237051 may be related to the genetic background of GAgP.
