Abstract:Natural orifice translumenal endoscopic surgery (NOTES) emerged with the concept of managing surgical conditions using a flexible endoscope as a novel progress within the narrow submucosal or "third space" combining minimal invasive surgery and endoscopy along with the development of submucosal tunneling endoscopic resection (STER) in the past decade. We hereby present the successful treatment of a rare case regarding transesophageal NOTES for treatment of schwannoma in the mediastinum. A 66-year-old male was admitted for esophageal 'SMT' found on routine check-up by regular esophagogastroduodenoscopy (EGD) examination for 1 month. Contrast-enhanced computed tomography (CT) of the chest showed a clear boundary mass, 23×19 mm in size, in the left superior mediastinum with internal heterogeneity. An endoscopic exploration was done and a transesophageal NOTES was accomplished with an operative time of 50 minutes for this patient. When a smooth elevated lesion was identified 23cm from the incisor teeth, submucosal injection was performed followed by a 2-cm longitudinal mucosal incision made at 5 cm above the tumor site. A submucosal tunnel was created toward the tumor location followed by full-thickness myotomy above the estimated location of the extraluminal tumor. Once the tumor is located, the surrounding tissues were dissected, and during this process, the tumor was found to originate from a strange cord-like structure. And to have an en bloc resection, the cord-like structure was sacrificed. The tumor was extracted by using a snare followed by careful inspection of the tunnel for any evidence of bleeding sites. The closure of the mucosal incision was achieved by clips. The tumor is 4×2×1.5cm in size with necrotic tissues inside and diagnosed as benign schwannoma by pathological examination. CT images are transfered to pattern diagrams to have a better understanding of its location. And this patient complaints of hoarseness on D1, an endoscopy inspection was done immediately which showed left side of the vocal cord fixed and the glottis opening which bears out earlier suspicion that it is a schwannoma in the mediastinum that originates from recurrent laryngeal nerve. Schwannoma in the mediastinum is extremely rare and difficult to differentiate from other submucosal tumors before surgery. Next time when you see an esophageal 'SMT', you should think of the possibility of schwannoma in the mediastinum. Traditional surgical resection via thoracotomy or video-assisted thoracic surgery (VATS) is definitely curative but may carry a significant risk of morbidity and mortality. STER makes NOTES revive. NOTES emerged as a novel development within the mediastinal cavity that appears to be an effective and minimally invasive endoscopic technique to avoid surgery. But it's highly demanding and only those who are very experienced endoscopists are able to do it.
