Abstract:Hypertension is a very common disease in older population, and the prevalence increases rapidly along with aging. It is estimated that more than 90% of people older than the age of 80 (the very older people) have hypertension. This disease carries a very high risk of stroke and heart diseases. Although many studies have shown that controlling hypertension prevents complications like stroke, heart attack, and heart and kidney failure, many elderly people continue to live with uncontrolled or inadequately controlled hypertension. Recently, a series of research have provided us new evidence on the management of geriatric hypertension, which impels us continuously to control hypertension in the elder patients.
Uncontrolled Hypertension Increases With Age And Associates With All-Cause Mortality In An Elderly Community-Dwelling Chinese Population In Shanghai

Abstract:Objectives: To determine the prevalence of hypertension, medication usage and attainment of blood pressure goals and their association with all-cause mortality in older urban community-dwelling Chinese subjects. Methods: Blood pressure, anthropometric data, blood samples and all-cause mortality were obtained from 3950 subjects (mean age 72 years, 1745 male) including 609 subjects aged ≥ 80 years in the Shanghai Elderly Cardiovascular Health Study (SHECHS). Results: Established cardiovascular disease was present in 7.7% of participants. The prevalence of hypertension was 74.8% overall and it was more than 80% in individuals considered to be in moderate and higher cardiovascular disease risk categories. In hypertensive subjects, 67.1% were on treatment and treatment was more frequent in high and very high cardiovascular risk individuals. Attainment of the systolic blood pressure goal < 150 mmHg was 62.9% and was greater in the elderly than in the very elderly. The most commonly used antihypertensive treatments were calcium channel blockers (54.2%), followed by angiotensin receptor blockers (43.1%). Diuretics were used in 2.6%. Fixed-dose combination antihypertensive tablets were used in some of the elderly (12.4%) and more of the very elderly (18.2%) and 70.9% of the elderly and 80.2% of the very elderly were on monotherapy. The all-cause mortality is reduced by control of hypertension. Conclusion: There were high prevalence and high treatment rates of hypertension, but poor attainment of the systolic blood pressure goal of < 150 mmHg, especially in the very elderly community-dwelling Chinese. Considering that more intensive treatment of hypertension in the elderly and very elderly may be warranted after recent studies, this might be achieved by more frequent use of combinations of effective therapies and diuretics.

Abstract:China has stepped into an aging society. The social service development statistical bulletin 2015 published in July 2016 by Ministry of Civil Affairs showed that till the end of 2015, the amount of people ≥60 years of age had already approached to 222 million, which comprised 16.1% of the Chinese population, while the amount of people ≥65 years of age comprised 10.5% of the Chinese population was 143 million.1 Hypertension is an independent risk factor for cardio-cerebral-vascular diseases and is a primary and contributory cause for death and disability in the elderly. A large number of epidemiological and clinical studies have shown that the risk of target organ damage of hypertension such as ischemic heart disease, cardiac insufficiency, stroke, chronic kidney disease, and aortic and peripheral artery diseases significantly increases with aging. Blood pressure control plays a significant role in reducing cardio-cerebral-vascular events and all-cause mortality.2 Compared to younger patients with similar blood pressure elevation, the risks of cardiovascular and cerebrovascular events significantly increase in the elderly. Because of specialties in the pathogenesis and clinical manifestation in older patients with hypertension, physicians should pay more attention to the population characteristics and individual treatments.

Abstract:There are special pathophysiologic characters in elderly patients with hypertension. Such as the undulation of blood pressure is more obviously and easy to result in orthostatic hypotension, the sensitivity to the nerve reflex reduces, the composition of body fluid and hemodynamics changes, and always complains with other organs incompetence. Isolate systolic hypertension is more general in older. The effect of antihypertension in these patients is poor. In lecture we review the advance of antihypertension in old patients, we suggest that the treatment of elder patients with hypertension should be colligated, not only reduce the blood pressure, but also considering the compliance and the function of other organs. The medication should begin at low dose, and combined treatment could obtain good results.

Abstract:Objective:To assess the prevalences of hypertension, rates of medication recommendations and failure rates of blood pressure(BP)control in Chinese elderly patients(≥65 years old).Methods:We used data from the 2011 China Health and Nutrition Survey(CHNS). A total of 2, 391 Chinese adults aged≥65 years with complete information comprising BP measurements repeated three times and antihypertensive medication use were included for analysis.Results:The mean age of subjects was 72.6±6.2 years, and females accounted for 53.1%.The numbers of patients with hypertension were 1784(74.6%)and 1221(51.1%)according to the definitions from 2017 ACC/AHA guideline and the 2010 Chinese guideline, respectively.There was a significant difference in the prevalence of hypertension in the subgroup with a cardiovascular disease(CVD)history according to the definitions from the two guidelines( χ2=23.70, P<0.01). The BP thresholds for drug therapy recommendations were the same as those set in the diagnostic criteria of hypertension based on the two guidelines.Therefore, the rates for medication recommendations were 74.6% and 51.1%, respectively.The numbers of patients with BP above the target levels were 622(88.2%)and 346(49.1%), respectively, according to the definitions from the two guidelines.There was no significant difference in failure rate of BP control due to age( χ2=5.36, P>0.05), gender( χ2=0.12, P>0.05)or a CVD history( χ2=0.07, P>0.05)according to analyses using the definitions from the two guidelines. Conclusions:Compared with the 2010 Chinese hypertension guideline, the prevalence of hypertension and rate of medication recommendations are higher in the Chinese elderly population when the 2017 ACC/AHA hypertension guideline is used.Hypertension management and antihypertensive drug treatment should be reinforced to improve the control rate of hypertension.

Abstract:We studied the prevalence, awareness, treatment and control of hypertension in an elderly Chinese population. The study subjects (age ⩾60 years) were recruited from a suburban town of Shanghai from 2006 to 2008. We administered a standardized questionnaire to collect information on medical history, the use of medications and lifestyle. We measured blood pressure three times consecutively using a validated Omron 7051 oscillometric device (Kyoto, Japan) after the subjects had rested for at least 5 min in the sitting position. We defined hypertension as a blood pressure of at least 140 mm Hg systolic or 90 mm Hg diastolic or as the use of antihypertensive drugs. The 3949 participants (mean age of 68.3 years) included 2185 (55.3%) women, 182 (4.6%) obese subjects (body mass index ⩾30 kg m−2) and 366 (9.3%) diabetic patients. The prevalence of hypertension was 59.4%. In the 2345 hypertensive patients, the awareness, treatment and control (<140/90 mm Hg) rates were 72.5%, 65.8% and 24.4%, respectively. In the 1542 treated hypertensive patients, 1196 (77.6%) used fixed-dose combinations of thiazide and reserpine or clonidine (n=1157, 75.0%) or of an angiotensin receptor blocker and hydrochlorothiazide (n=1) or free combinations (n=38, 2.5%), and 346 (22.4%) used a monotherapy of short-acting calcium channel blockers (n=217, 14.1%) or other classes of antihypertensive drugs (n=129, 8.3%). The corresponding control rates were 37.3% and 36.4%, respectively. In a stepwise logistic regression, the risk of uncontrolled hypertension was higher with older age (+10 years, odds ratio (OR) 1.19, P=0.03), female sex (OR 1.40, P=0.01), obesity (OR 2.35, P=0.0002) and heavy drinking (⩾300 g per week, OR 2.18, P=0.0007). In conclusion, in elderly Chinese, the prevalence of hypertension is high. In spite of reasonably high awareness and treatment rates, the control rate remains low, most likely due to an unhealthy lifestyle and the underuse and/or underdose of antihypertensive drugs.

Abstract:The progressive ageing of world population, and the increasing prevalence hypertension in elderly people are leading to the consideration that hypertension in the elderly is one of the main topic in hypertension treatment. Multiple mechanisms, including stiffening of large arteries, endothelial dysfunction, cardiac remodeling, autonomic dysregulation, renal aspects, contribute to the great prevalence of hypertension in the elderly and to increased cardiovascular morbidity and mortality. Treatment of hypertension can hardly put back older patients in a low risk category, especially if target organ damage is present. Nevertheless, blood pressure control can successfully prevent stroke, cognitive decline, coronary heart disease and heart failure, and reduce mortality in the elderly, and even in patients > 80 years, as recently demonstrated. Blood pressure should be lowered below 140/90 mmHg also in older patients. However the HYVET study suggests that a goal of 150/90 mmHg can be reasonable in patients aged 80 years or more. Drug treatment should be titrated with particular caution to adverse responses and excessive blood pressure lowering.

Abstract:BackgroundBoth drug treatment and non-drug treatment were definitely recommended in the 2010 Guidelines for Prevention and Treatment of Hypertension in China. This study aims to provide reliable estimates of both treatments of hypertension and their distribution and related influencing factors in the middle-aged and elderly population in China.MethodsThe China Health and Retirement Longitudinal Study (CHARLS) is a cross-sectional survey of the prevalence, awareness, and treatment of hypertension from 2011∼2012. Eligible individuals (Chinese people aged 45 years or more) were selected through four-stage, stratified, cluster sampling. Hypertension was considered as a mean systolic blood pressure of 140 mm Hg or higher, a mean diastolic blood pressure of 90 mm Hg or higher or was assumed for participants taking anti-hypertensive medication. The primary outcome of this study is the control of hypertension, which was defined as blood pressure less than 140/80 mm Hg. Multivariable logistic regressions were used to estimate odds ratios (ORs). The ethics committee of Peking University Health Science Center approved this study, and all participants gave written informed consent before participation.Findings12 981 people participated in the study, of whom 5371 (33% [95% CI 32·3–33·9%]. 59% of participants with hypertension were aware of the diagnosis. Of those people who were aware of their hypertension, 2440 (77%) were receiving treatment with drugs, and 1838 (58%) were receiving at least one non-drug treatment (of whom 25·4% were receiving health education on weight control, 35% on exercise, 49% on diet, and 22% on smoking control). Patients who received either treatment were more likely to be men living in urban areas with higher social status. 31% of the patients were controlled with an average systolic blood pressure less than 140 mmHg and an average diastolic blood pressure less than 90 mmHg. Compared with the population that received no treatment for hypertension, both drug treatment and non-drug treatment improved the control rate of hypertension (all ORs >7·0) after adjusting for sex, age, gross domestic product, education level, drinking, smoking, body-mass index, diabetes, and cardiovascular diseases.InterpretationThe prevalence of hypertension in the Chinese middle-aged and elderly population is surprisingly high, with disproportionately low awareness and drug treatment. Practical guidelines to non-drug approaches have been provided but are inadequately used. Abstract:Objective: Hypertension is the most challenging public health problem worldwide and seriously affects human health. To date, there are no epidemiological studies on the prevalence of and risk factors for hypertension among older people in mainland China. Methods: We conducted a meta-analysis of the prevalence and risk factors of hypertension among the older population in mainland China. We searched Chinese and English databases for Chinese and English literature on hypertension epidemiology published between 2000 and 2022, and hypertension data among the older population were extracted from the included literature. A meta-analysis was performed using a random-effects model ( I 2 > 50%) with 95% confidence intervals for the forest plots. Data were processed using RevMan 5.3. Forty-nine publications (with data from 84,429 samples) met the evaluation criteria and were included in this study. Results: We found that the total prevalence of hypertension was 47%. The total prevalence rate of the older population in China from 2000 to 2010 was 50%, and the prevalence rate from 2011 to 2021 was 45%, with no significant differences. The total prevalence in Central China was the highest (59%). There was no significant correlation between the prevalence rate of the older population, sex, and urban or rural areas. Conclusion: Hypertension is common among the older population in China, and its control rate is low. Therefore, effective prevention and treatment measures, as well as education, should be formulated to improve the diagnosis and treatment of hypertension in the older population.

Intensive blood pressure control in older patients with hypertension-a STEP in the right direction?

Abstract:In November 2022, the Clinical Practice Guidelines for the Management of Hypertension in China were updated; the definition of hypertension and the target of treatment was changed from 140/90 mm Hg to 130/80 mm Hg. This was prompted by the results of the Strategy of Blood Pressure Intervention in the Elderly Hypertensive Patients (STEP) study, a large multicenter randomized controlled trial conducted in China. STEP echoed the results of the American Systolic Blood Pressure Intervention Trial (SPRINT), and confirmed that intense lowering of blood pressure is on the whole beneficial. This confirmation is important for the Chinese population, in which strokes outnumber myocardial infarctions. It is also reassuring to know that treating adults aged 60 years or older to achieve a systolic blood pressure of <130 mm Hg is safe, while reducing cardiovascular events, stroke and all-cause mortality. Nevertheless, further studies are needed to delineate the risks and benefits in subgroups, such as the elderly with diabetes or a history of stroke.

Abstract:To achieve a consensus on optimal blood pressure (BP) targets for older adults remains challenging, necessitating a trade-off between cardiovascular benefits and the risk of impaired organ perfusion. Evidence suggests that age and frailty have a minimal influence on the cardiovascular benefits of intensive BP control in community-dwelling elderly. Nonetheless, an increased incidence of acute kidney injury with intensive BP control has been observed in octogenarians. Therefore, it is recommended to maintain systolic BP below 130 mmHg for hypertensive patients aged 65-80 years. If well-tolerated, a systolic BP target below 120 mmHg can be recommended for patients with chronic kidney disease (CKD). However, no conclusive evidence supports a stringent BP target for patients aged 80 years and older. The selection of antihypertensive medications for elderly patients requires consideration of their cardiovascular condition and potential contraindications. Combination therapy may be necessary to achieve the desired BP target. Angiotensin-converting enzyme inhibitors or angiotensin receptor blockers are the primary choices for patients with CKD. Newer generation mineralocorticoid receptor antagonists may further reduce the risk of cardiovascular or renal events in this population. In conclusion, managing hypertension in elderly patients requires a personalized approach that balances cardiovascular benefits with potential risks, considering individual health profiles and tolerability.

Abstract:BACKGROUND:Hypertension is considered a major public health challenge. It is the most important risk factor for cardiovascular disease and is a prominent risk for China's elderly population. However, few studies have addressed the effect of blood pressure control on elderly hypertension patients in China. In response, this study aimed to investigate the prevalence and control of hypertension in the elderly population in Shanghai's communities. METHODS:A secondary data analysis based on a government-financed health check-up program for an elderly population aged 65 and older from 2012 to 2014 was conducted in a central district of Shanghai. RESULTS:Of the 44,978 study participants, 20,305 (45.1%) were males and 24,673 (54.9%) were females. The participants' median age was 72. Half of the participants were overweight or obese (BMI ≥ 24.0 kg/m2). The prevalence of hypertension was 59.9%. In the 18,032 participants without prior diagnosis of hypertension, 5530 (30.7%) had increased blood pressure. Among the 26,946 confirmed hypertension patients, the proportions of treatment and blood pressure control were 32.8% and 43.4% respectively. Multivariate analysis showed that the uncontrolled hypertension was significantly associated with older age, being overweight or obese, a lower level of education, an unbalanced dietary pattern, regular drinking and non-comorbidities. CONCLUSIONS:The prevalence of hypertension was high in China's elderly population. The proportion of individuals who received treatment remained low, and blood pressure control was poor among hypertension patients. These results indicate that improvement of the ability to manage and control hypertension among urban elderly residents is urgently needed.

Abstract:Objectives To investigate the epidemiological status quo of hypertension in elderly population in Changchun, China, and provide a reference for the prevention and control strategies of hypertension of elderly population in this region. Design A cross-sectional study, as a part of a comprehensive project in Northeast China, was designed to perform in 10 districts in Changchun. Participants and setting A total of 6846 participants who were ≥60 years old were selected using a random sampling method. Main outcome measures The epidemiological status quo of hypertension. Results The prevalence of hypertension in Changchun was 52.6%. Among participants with hypertension enrolled in this study, 87.6% of the participants had been diagnosed with hypertension before the study, 69.1% was taking antihypertensive medications and 66.9% had effective blood pressure control. Obesity, widower/widow, history of diseases and family history of hypertension were risk factors of hypertension (all p<0.05). Participants with obesity, a personal history of heart coronary disease, or a family history of hypertension were susceptible to realising risks of hypertension (all p<0.05). However, participants with diabetes, hyperlipidaemia, or a family history of hypertension were difficult to control blood pressure within the normal range (all p<0.05). In addition, 92.6% participants taking antihypertensive medications used a single medication, and calcium channel blockers was the most commonly used antihypertensive medications in monotherapy. Conclusion The rates of awareness, treatment and control of hypertension are greater in Changchun than those in China, indicating that the prevention and control of hypertension in Changchun are effective. However, the prevalence of hypertension in the elderly population in China is lower than that in Changchun, also rendering Changchun a substantial challenge for the supervision of hypertension.

Abstract:Objective To explore the current status of the prevalence, awareness, medical treatment, and control rates of hypertension among the elderly residents from rural areas in Rongcheng area. Method Cross-sectional study was conducted among 2125 adults ( ≥ 50 years) randomly selected from 10 spots of Rongcheng by cluster sampling method and a questionnaire survey was carried out of hypertension. Datas were statistically analyzed. Result 1309 out of 2125 adults suffered from hypertension with the prevalence of 61.6% and 516 (24.3%) were newly diagnosed. The awareness, treatment and control rates of hypertension were 60.6%, 40.7% and 12.9%, respectively, and varied significantly among the elderly with different age and gender (P < 0.05). Conclusion The awareness, treatment and control rate of hypertension among the elderly from rural areas in Rongcheng have increased, but still relatively low especially the treatment and control rates. The intervention should be used to improve the awareness, treatment and control rates of hypertension in Rongcheng area.

Abstract:Abstract With a rapidly aging population, adequate blood pressure (BP) control is critical for hypertension management and prevention of cardiovascular events. Impressive cardiovascular benefits have been observed with intensive BP control (SBP target, <120 mmHg) in the SPRINT (Systolic Blood Pressure Intervention Trial) study, even in patients 75 years of age or older. A most recent meta-analysis including 51 randomized trials with over 350,000 participants from the BPLTTC (The Blood Pressure Lowering Treatment Trialists’ Collaboration) showed that BP lowering is effective in older people for reducing major cardiovascular events. The STEP (Strategy of Blood Pressure Intervention in the Elderly Hypertensive Patients) study—a multicenter, randomized, controlled trial conducted in China, provided important evidence that intensive BP treatment (SBP target, 110 mmHg to <130 mmHg) benefits older hypertensive patients (aged 60–80 years) and reduced the incidence of cardiovascular events than standard treatment (target 130 mmHg to <150 mmHg). Because Asian people have a higher burden of hypertension and stroke than Caucasian people, intensive BP treatment has more advantages in reducing the risk of cardiovascular events including stroke in Asian hypertensive patients than in Caucasian people. Home BP monitoring is helpful to facilitate hypertension management for older patients. It should also be noted that clinical decision-making should be on a patient basis, such as fragility, diabetes, stroke, and other comorbidities, with tailored BP targets. Here we review the important clinical trials of BP control in elderly hypertension, interpretate the main findings of STEP, and also discuss the perspectives of managing hypertension in Asia.
