Laparoscopic assisted total colorectal resection for familial adenomatous polyposis: with a report of 9 cases

Abstract:Objective: To evaluate the safety,feasibility and value of treating familial adenomatous polyposis(FAP) with laparoscopic assisted total colorectal resection. Methods:The clinical data,including short term follow-up outcomes,safety,and recovery of 9 FAP patients who underwent laparoscopic assisted total colorectal resection were analyzed retrospectively.The data were compared with those of 5 patients who underwent conventional open surgery. Results:Laparoscopic assisted total colorectal resection were performed successfully on 9 patients without severe complications.The mean operation time of the laparoscopic group was 195.56(160-250) min,slightly longer than that of the conventional group;the mean operative blood loss [138.89(50-300) ml] and hospital stay [12.11(7-20) d] in the laparoscopic group were not significantly different when compared with those in the conventional group.Anal exsufflation time [2.78(2-4) d] and incision length [5.11(4-6) cm ] of the laparoscopic group were significantly shorter than those of the conventional group.No severe complications had taken place,except one patient suffered from anastomotic leak.No local recurrence,distant metastases or death were found during the follow-up of 7-40 months. Conclusions:Laparoscopic assisted total colorectal resection for FAP can be performed safely and effectively with the advantages of minimal invasion,quick recovery and good short term effect by the experienced surgeons,but further accumulation of cases and randomized controlled trials are required.
