Clinical comparison of postoperative analgesia effect between PCSA and PCEA

Abstract:Objective To observe the effect of postoperative intrathecal patient-controlled analgesia(PCA) after hip or lower limbs operation using continuous spinal anesthesia.Methods(Forty) patients ASA Ⅰ-Ⅲ aged 20-55 y,scheduled for hip or lower limbs surgery were included in this(study.) When the operations finished,the patients were randomly devided into groupⅠ(group PCSA,n=20) and groupⅡ(group PCEA,n=20).In groupⅠ,the operation was performed under CSA with 0.5 % hyperbaric bupivacaine solutions using a 24G Spinocatheter(and) the patients received(PCSA) with 0.075% bupivacaine and 0.0002% fentanyl afte operation Intrathecal PCA included a(bolus) of 1ml on demand with a 25-minutes lockout and one hour dose limit of 3ml.Backgroud infusion(0.5ml/h) was used in groupⅠ.In groupⅡ the operation was finished under combined spinal-(epidural)(anesthesia)(CESA) and the patients received patient-controlled epidural analgesia(PCEA) with 0.1875% bupivacaine and 0.001% fentanyl after operation.PCEA included loading dose 2ml,Backgroud infusion 2.0ml/h,PCEA bolus 1ml on demand with a 15-minutes lockout and one hour dose limit 5ml.The patients were observed for 48 hours after operation and the pain(VAS 0-10),motor blockade(Bromage Score,BMS 0-3) and adverse effects were(recorded.)Results There were no statisticolly difference among the basic conditions(P>0.05).VAS scoreless than 3 accounted for 95%(groupⅠ) and 75%(groupⅡ) respectively.The percentage of excellent(analgesia) was higher in groupⅠthan that in group Ⅱ(P<0.05).The consumption of bupivacaine and fentanyl was smaller in groupⅠ(P<0.01).The incidence of patients with postoperative nausea and vomiting in group Ⅱwas higher than that in group Ⅰ(P<0.05).Conclusion(Patient-)controlled spinal analgesia after operation has the advantage of better analgesia,lower adverse effects,less anesthetic drugs.
