Left Cardiac Sympathetic Denervation Surgery for Treating Long QT Syndrome

Abstract:Objective Long QT syndrome (LQTS) is a cardiovascular disorder characterized by prolongation of the QT interval on ECG and presence of syncope, seizures, and sudden cardiac death. The therapy for LQTS is mainly dependent on medicines. The aim of the present study was to investigate the effect of a surgical method of left cardiac sympathectomy for LQTS patients. Methods Left cardiac sympathetic denervation (LCSD) was performed in 5 LQTS patients. Patients' ECG and clinical symptoms were assessed before and after the surgery. Results LCSD was successful in all patients. QTc was decreased from (0. 59 ±0. 05 ) s to (0. 48 ±0. 04)s (P =0. 006,n =5) after the procedure. Meanwhile,following QT shortening U wave was gradually become prominent The 24 hours Holter monitoring ECG showed that the mean value of QTc within 24 hours was decreased in all 5 patients to various degrees. The maximum HR decreased slightly from (103 ± 17) bpm to (96±15) bpm ,and the minimum HR increased slightly from (42 ±7) bpm to (46±5) bpm. The exercise test showed that there was a significant change in the T wave morphology on some leads immediately after the test, and these changes were abolished by LCSD. After surgery combined with increased dosage of pro-panalol, the maximum heart rate obtained during exercise can no longer reach the level before surgery for all patients,it decreased from (162±4) bpm to (129±10) bpm (P = 0.006). The 5 patients were followed for 15 months. All patients were symptom free except one ( case2) who had 2 syncopes during morning sleep at that time she had teken propranolol only 20 mg once a day (0. 3 mg/kg). No propranolol was taken for case 4 and 5, and propranolol dosage for case 1 and 3 are 10 mg three times a day and 20 mg twice a day. Conclusions Our direct observations are consistent with the safety and efficacy of LCSD for LQTS patients observed by other investigators. The low-cost and simplicity of LCSD may have significant implications in patients of developing countries like China,where expensive therapies such as ICD are unlikely to be widely applied due to financial constraints.
