Abstract:Population pharmacokinetics is an emerging discipline developed from the combination ofclassical pharmacokinetic compartment model and statistics principles, which has been received more and moreattention in recent years. Population pharmacokinetics plays important roles in all stages of new drug research.In the early preclinical phase, population pharmacokinetic analysis can help to achieve the preliminary predictionof parameters from animal to human, optimize clinical trial designs, and shorten the time required for newdrugs from laboratory to clinical trials. In clinical trials and applications stage, population pharmacokineticresearch can help researchers investigate the related covariates that affecting pharmacokinetic behavior ofpatients comprehensively, and find potential drug-drug interactions in clinical. In addition, populationpharmacokinetics has a unique advantage in pediatric drug development due to its strong analysis ability ofsparse data. Abstract:The application of population approaches to drug development is recommended in several US Food and Drug Administration (Fl)A) guidance documents. Population pharmacokinetic (and pharmacodynamic) techniques enable identification of the sources of inter- and intra-individual variability that impinge. upon drug Safety and efficacy. This article briefly discusses the 2-stage approach to the estimation of population pharmacokinetic parameters, which requires serial multiple measurements on each participant, and comprehensively reviews the nonlinear mixed-effects. modelling approach, which can be applied in situations where extensive sampling is not done on all or any of the participants.Certain preliminary information, such as the compartment model used in describing the pharmacokinetics of the drug, is required for a population pharmacokinetic study. The practical design considerations of the location of sampling times, number of samples/participants and the need to sample an individual more than once should be borne in mind. Simulation may be useful for choosing the study design that will best meet study objectives.The objectives of the population pharmacokinetic study can be secondary to the objectives of the primary clinical study lin which case an add-on population pharmacokinetic protocol may be needed) or primary (when a stand-alone protocol is required). Having protocols for population pharmacokinetic studies is an integral part of 'good pharmacometric practice'.Real-time data assembly and analysis permit an ongoing evaluation of site compliance with the study protocol and provide the opportunity to correct violations of study procedures. Adequate policies and procedures should be in place for study blind maintenance. Real-time data assembly creates the opportunity for detecting and correcting errors in concentration-time data, drug administration history and covariate data.Population pharmacokinetic analyses may be undertaken in 3 interwoven steps: exploratory data analysis, model development and model validation (i.e. predictive performance). Documentation for regulatory purposes should include a complete inventory of key runs in the analyses undertaken (with flow diagrams if possible), accompanied by articulation of objectives, assumptions and hypotheses. Use of diagnostic analyses of goodness of fit as evidence of reliability of results is advised. Abstract:Population pharmacokinetic analysis is a relatively new approach which can be used to obtain important pharmacokinetic and pharmacodynamic information from sparse data sets routinely obtained in phase II and III clinical trials, these studies typically have many patients but few observations per patient. Similarly, this approach is beneficial for studies in which intensive blood sampling is not attainable, such as in children and patients with cancer and AIDS.It was not until the late 1980s and the early 1990s that this approach (which had been introduced by Sheiner and Beal approximately 20 years earlier) gained appreciable momentum. Today many pharmaceutical companies use this approach routinely, to differing extents, during their drug development process. Advocacy by the US Food and Drug Administration for pharmacokinetic screening during phase II and III studies was an important factor in the widespread adoption of this approach. A second factor was the gradual realisation that the approach can be cost effective in revealing clinically important information about the determinants of interpatient pharmacokinetic and pharmacodynamic variability in treated patients.However, several important issues remain to be resolved (such as the optimal study design, quality of the data and user-friendly software) which could determine the future role of the population approach in drug development.

Clinical Pharmacokinetics

Abstract:Currently, there is an increasing focus on the implementation of pharmacokinetic-pharmacodynamic (PK-PD) studies and modelling as essential tools for drug development. Strategies involving specifically the population approach, which are based on relatively recent statistical methodology (e.g. nonlinear mixed effects modelling, NONMEM) have been advocated for investigating pharmacokinetic and pharmacodynamic variability as well as dose-concentration-effect relationships. The present article outlines this approach, and discusses how it can be implemented within the framework of the studies currently performed as part of the clinical phases of new drug development. It also considers study design and performance, based on real-life experiences.Population approaches, if designed carefully and early, as part of the planning of the drug development programme, are expected to play a significant role at every phase of the programme and to contribute to providing information that is valuable for registration purposes. Statistical methodology and software are now widely available. However, practical issues such as integration of the population approach within existing protocols, quality control of the data, timing of laboratory and statistical analyses, as well as resource allocation, remain legitimate concerns to be considered in prospective studies.

Abstract:The core of therapeutic drug monitoring (TDM) is individualization regime, i.e. taking the patients' physiological, pathological, and environmental factors into account to make safe and effective dosage regimes. The key of the problem is to get ideal individual pharmacokinetic parameters. Population pharmacokinetics (PPK) is a new method to combine classical pharmacokinetics with statistical principle, and use sparse data to estimate population pharmacokinetic parameters. With the help of Bayesian method, we can estimate individual parameters relatively accurately and optimize the dosage regime. The principle of population pharmacokinetics and data analysis methods are introduced in this article.

Abstract:Physiologically based pharmacokinetic (PBPK) modeling and simulation can be used to predict the pharmacokinetic behavior of drugs in humans using preclinical data. It can also explore the effects of various physiologic parameters such as age, ethnicity, or disease status on human pharmacokinetics, as well as guide dose and dose regiment selection and aid drug-drug interaction risk assessment. PBPK modeling has developed rapidly in the last decade within both the field of academia and the pharmaceutical industry, and has become an integral tool in drug discovery and development. In this mini-review, the concept and methodology of PBPK modeling are briefly introduced. Several case studies were discussed on how PBPK modeling and simulation can be utilized through various stages of drug discovery and development. These case studies are from our own work and the literature for better understanding of the absorption, distribution, metabolism and excretion (ADME) of a drug candidate, and the applications to increase efficiency, reduce the need for animal studies, and perhaps to replace clinical trials. The regulatory acceptance and industrial practices around PBPK modeling and simulation is also discussed.

Abstract:Interindividual variability in anatomical and physiological properties results in significant differences in drug pharmacokinetics. The consideration of such pharmacokinetic variability supports optimal drug efficacy and safety for each single individual, e.g. by identification of individual-specific dosings. One clear objective in clinical drug development is therefore a thorough characterization of the physiological sources of interindividual variability. In this work, we present a Bayesian population physiologically-based pharmacokinetic (PBPK) approach for the mechanistically and physiologically realistic identification of interindividual variability. The consideration of a generic and highly detailed mechanistic PBPK model structure enables the integration of large amounts of prior physiological knowledge, which is then updated with new experimental data in a Bayesian framework. A covariate model integrates known relationships of physiological parameters to age, gender and body height. We further provide a framework for estimation of the a posteriori parameter dependency structure at the population level. The approach is demonstrated considering a cohort of healthy individuals and theophylline as an application example. The variability and co-variability of physiological parameters are specified within the population; respectively. Significant correlations are identified between population parameters and are applied for individual- and population-specific visual predictive checks of the pharmacokinetic behavior, which leads to improved results compared to present population approaches. In the future, the integration of a generic PBPK model into an hierarchical approach allows for extrapolations to other populations or drugs, while the Bayesian paradigm allows for an iterative application of the approach and thereby a continuous updating of physiological knowledge with new data. This will facilitate decision making e.g. from preclinical to clinical development or extrapolation of PK behavior from healthy to clinically significant populations.

Abstract:Although many population pharmacokinetics (PPK) researches have been conducted on chemical drugs, few have been in the field of Chinese medicine (CM). Each ingredient in CMs possesses different pharmacokinetic characteristics, therefore, it is important to develop methods of PPK studies on them to identify the differences in CM drug safety and efficacy among the population subgroups and to conduct quantitative studies on the determinants of CM drug concentrations. To develop an expert consensus on study design and implementation for PPK of CM, in August 2013, 6 experts in the field of PPK, CMs pharmacology, and statistics discussed problems on the PPK research protocol of CMs, and a consensus was reached. The medicines with toxicity and narrow therapeutic windows and with wide range of target population or with frequent adverse reactions were selected. The compositions with definite therapeutic effects were selected as indices, and specific time points and sample sizes were designed according to standard PPK design methods. Target components were tested through various chromatography methods. Total quantity statistical moment analysis was used to estimate PPK parameters of each component and PPK models reflecting the trend of CMs (which assists in reasonable adjustments on clinical dosage). This consensus specifies the study design and implementation process of PPK. It provides guidance for the following: post-marketing clinical studies, in vivo investigations related to the metabolism in different populations, and development and clinical adjustment of dosages of CMs.

Abstract:Mechanism-based pharmacokinetic/pharmacodynamic models study the time course of pharmacological effects of drugs,and are widely used in the field of drugs research & development and clinical application.The purpose of this article is to describe some commonly used mechanism-based pharmacokinetic/pharmacodynamic models,their features,and relevant equations.According to the pharmacological response,the models are divided into reversible,irreversible and tolerance models.They reflect the basic mechanism of action,and future pharmacodynamic models will most likely reflect an assembly of the basic components outlined in this review.

Abstract:OBJECTIVE: To introduce the application of population pharmacokinetic model to dose-adjustment of antibiotics.METHODS: To analyze basic concepts of population pharmacokinetic model and pharmacokinetic(PK)-pharmacodynamic(PD) model,set up the procedure and study applicational characters and development status at home and abroad.two examples were given for explaining dose-adjustment method.RESULTS&CONCLUSION: Close combination of population pharmacokinetics and therapeutic drug monitoring can be applied to anti-infective therapy in the clinic.The use of PK-PD model will be beneficial to dose design and adjustment in clinical pharmacy,it will also make most rational explanations for different patient population in anti-infective therapy.

Abstract:Physiologically based pharmacokinetic/toxicokinetic (PBPK/PBTK) models are designed to elucidate the mechanism of chemical compound action in organisms based on the physiological, biochemical, anatomical, and thermodynamic properties of organisms. After nearly a century of research and practice, good results have been achieved in the fields of medicine, environmental science, and ecology. However, there is currently a lack of a more systematic review of progress in the main research directions of PBPK models, especially a more comprehensive understanding of the application in aquatic environmental research. In this review, a total of 3974 articles related to PBPK models from 1996 to 24 March 2024 were collected. Then, the main research areas of the PBPK model were categorized based on the keyword co-occurrence maps and cluster maps obtained by CiteSpace. The results showed that research related to medicine is the main application area of PBPK. Four major research directions included in the medical field were "drug assessment", "cross-species prediction", "drug–drug interactions", and "pediatrics and pregnancy drug development", in which "drug assessment" accounted for 55% of the total publication volume. In addition, bibliometric analyses indicated a rapid growth trend in the application in the field of environmental research, especially in predicting the residual levels in organisms and revealing the relationship between internal and external exposure. Despite facing the limitation of insufficient species-specific parameters, the PBPK model is still an effective tool for improving the understanding of chemical–biological effectiveness and will provide a theoretical basis for accurately assessing potential risks to ecosystems and human health. The combination with the quantitative structure–activity relationship model, Bayesian method, and machine learning technology are potential solutions to the previous research gaps.

Abstract:Natural medicine has been widely used for clinical treatment and health care in many countries and regions. Additionally, extracting active ingredients from traditional Chinese medicine and other natural plants, defining their chemical structure and pharmacological effects, and screening potential druggable candidates are also uprising directions in new drug research and development. Physiologically based pharmacokinetic (PBPK) modeling is a mathematical modeling technique that simulates the absorption, distribution, metabolism, and elimination of drugs in various tissues and organs in vivo based on physiological and anatomical characteristics and physicochemical properties. PBPK modeling in drug research and development has gradually been recognized by regulatory authorities in recent years, including the U.S. Food and Drug Administration. This review summarizes the general situation and shortcomings of the current research on the pharmacokinetics of natural medicine and introduces the concept and the advantages of the PBPK model in the study of pharmacokinetics of natural medicine. Finally, the pharmacokinetic studies of natural medicine using the PBPK models are summed up, followed by discussions on the applications of PBPK modeling to the enzyme-mediated pharmacokinetic changes, special populations, new drug research and development, and new indication adding for natural medicine. This paper aims to provide a novel strategy for the preclinical research and clinical use of natural medicine.

Abstract:Population pharmacokinetics (POP PK) is a pharmacokinetic tool, which measures and explains the variability in drug exposure and drug effect between individuals. For prescribers, POP PK model will allow to estimate an individual Withdrawal Time for a given horse. For scientists, POP PK model will allow meta‐analysis of existing data and to generate new drug knowledge. For organisations, POP PK model will allow to objectively and impartially summarize the data available to experts, facilitating consensus when establishing international screening limits. Population pharmacokinetics (POP PK) is a powerful pharmacokinetic tool, which measures quantitatively, and explains the variability in drug exposure and drug effect between individuals. POP PK uses an observational (nonexperimental) approach; it is conducted in the target population living in its normal environment (e.g., farm and race‐track). The strength of the POP PK approach lies in its greater relevance for the population studied in its different natural environments than experimental studies carried out in more or less biased laboratory conditions. In clinical settings, it is commonly necessary to restrict the number of samples per subject collected for analysis and the derived data cannot be analyzed using traditional individual data analytical methods; rather data are merged and analyzed with an appropriate statistical tool: the nonlinear mixed effect model (NLMEM). POP PK modeling is frequently used with the objective of adjusting drug dosage, and hence drug exposure, not only for the whole population but also for subgroups of animals (e.g., for a given breed, sex, and age). It can also have application at the individual subject level, in the context of precision medicine. For horses, the use of the POP PK/PD model will allow prescribers to estimate an individual Withdrawal Time for a given horse whose treatment they are supervising. Another potential field of application will be meta‐analysis of existing data to generate new knowledge on a drug or to collate and synthesize, in an objective and transparent manner, existing data; this will facilitate harmonization of screening limits at an international level.

Abstract:In population pharmacokinetic (PPK) research of Chinese medicines with narrow therapeutic windows of toxicity, or when the target population is not homogeneous, or when there are frequent adverse reactions to parenterally administered Chinese medicine, select those that have a definite therapeutic effect, and in which the compositions of the toxic substances compositions are known, for study, and use complete PPK sampling design to take samples at specific time points. Use gas chromatography, HPLC, and LC-MS methods for the detection of target components. Finally, use total quantity statistical moment analysis, to account for each component of the PPK parameters. Thus, PPK model can reflect the overall trend of Chinese medicine, to provide the basis for reasonable clinical dosage adjustments.
