Abstract:1626 Objectives The dysfunction of dopamine system, a neurotransmission mode playing important roles in human brain, is implicated in many disorders in nervous system, such as Parkinson’s disease (PD), Attention Deficit Hyperactivity Disorder (ADHD). 18F-fallypride is a radioactive molecular probe for Positron Emission Tomography (PET) visualizing the Dopamine D2/D3 receptors in striatal and extrastriatal areas1-3. We investigated the visualization of dopamine D2/D3 receptors by 18F-fallypride in normal volunteers by using AMIC Ray-Scan 64 PET/CT (CFDA approved) developed by our group. Methods 15 normal volunteers (8 males and 7 females) were investigated with AMIC Ray-Scan 64 PET/CT. The 18F-fallypride was injected intravenously (2.96MBq/kg) to normal volunteer 1 hour prior to PET/CT brain scanning. Image reconstruction and data analysis was performed by using AMIC Ray-Scan 64 PET/CT software. Results Reconstructed by AMIC Ray-Scan 64 PET/CT software, the image of dopamine system in extrastriatal areas including substantia nigra and red nucleus is visualized clearly, and signal distribution of 18F-fallypride in caudate nucleus and putamen in left and right side is homogeneous and symmetrical in normal volunteers’ brain scanning (result is depicted in supplement Figure). Conclusions 18F-fallypride-PET/CT scanning is a promising method in visualizing of neurotransmitters like dopamine receptors due to the long half-life and its strong signal, comparing to other radiotracers. In this study, we confirmed that the AMIC Ray-Scan 64 PET/CT system, as well as 18F-fallypride, performs excellently in dopamine D2/D3 receptors imaging in normal volunteers. Thus, this Ray-Scan system could be widely used in nervous system disease diagnosis and drug discovery, as well as other clinical applications. Abstract:Brain molecular imaging plays an important role in investigations on the function of brain and mechanism of neural diseases. The dopaminergic receptor system is closely related with emotion, love, addiction, incentives, many diseases and disorders. Hereby, taking 18F-fallypride as an example of novel dopamine receptor D2/D3 probes visualizing the Dopamine D2/D3 receptors in striatal and extra-striatal areas, this paper reviews the trends of R&D(Research and Development) and clinical applications of 18F-fallypride, and investigates 18F-fallypride with 15 healthy volunteers’ brain imaging by using the self-innovated AMIC Ray-Scan 64 PET/CT. The result shows a homogeneous and symmetrical distribution of signal in striatal and extra-striatal areas, indicating that the 18F-fallypride, as well as Ray-Scan 64 PET/CT, has demonstrated an excellent performance in dopamine receptor imaging. Additionally, this paper also reviews the progress of clinical uses of 18F-ML-10, a molecular probe to visualize apoptosis. Besides, 10 patients with brain tumorsare investigated by 18F-ML-10-PET/CT imaging pre-and post-radiation with AMIC Ray-Scan 64 PET/CT, in order to evaluate the effect of treatments.
Synthesis and Evaluation of 18F-Labeled Phenylpiperazine-like Dopamine D3 Receptor Radioligands for Positron Emission Tomography Imaging

Abstract:The dopamine D3 receptor (D3R) is important in the pathophysiology of various neuropsychiatric disorders, such as depression, bipolar disorder, schizophrenia, drug addiction, and Parkinson's disease. Positron emission tomography (PET) with innovative radioligands provides an opportunity to assess D3R in vivo and to elucidate D3R-related disease mechanisms. Herein, we present the synthesis of eight 18F-labeled phenylpiperazine-like D3R-selective radioligands possessing good radiochemical purity (>97%), in vitro stability (>95%), and befitting lipophilicity. Based on in vitro binding assays and static microPET studies, the phenylpiperazine-like radioligands [18F]FBPC01 and [18F]FBPC03 were chosen as lead radioligands targeting D3R. Molecular docking further elucidated their binding mechanism. Radiolabeling conditions were optimized and then applied to an automated radiolabeling process, affording products with high specific activity (>112 GBq/μmol). Dynamic rat PET study demonstrated the specific binding of [18F]FBPC01 and [18F]FBPC03 to D3R in the brain ventricles and the pituitary gland. Validated by dynamic PET data analysis, biodistribution study, and metabolism analysis, [18F]FBPC03 exhibited the highest PET signal-to-noise ratio, good D3R-specific binding in the brain ventricles and pituitary gland of rats with few off-target binding, negligible defluorination, and stable brain metabolism, which indicated that [18F]FBPC03 was a promising D3R radioligand.

Abstract:Purpose Dopamine transporter (DAT) is a promising target for positron emission tomography (PET) imaging of many neuropsychiatric diseases. 18 F-labelled N-alkyl tropane analogues were reported to be useful PET radioligands for DAT. However, the drawback of 18 F-labelled tropane analogues is that N -alkyl on tropane is easily metabolized in vivo , which interferes with brain imaging. To develop a more in vivo stable DAT-targeted PET radioligand with high DAT affinity and specificity, in this study, we synthesized and compared a series of 18 F-labelled novel deuterated N-fluoropropyl tropane derivatives [ 18 F] 4a-e for DAT tracing. Procedures Five deuterated N-fluoropropyl-d 6 tropane derivatives ( 4a-e ) and corresponding non-deuterated compounds ( 6a-e ) were synthesized, and their semi-inhibitory concentrations (IC 50 ) were measured by competitive binding assay. The radioligands [ 18 F] 4a-e and [ 18 F] 6a-e were obtained by two-step one-pot radio-labelling reactions. The selectivity and specificity of these radioligands were evaluated by cellular uptake and microPET in normal rats. [ 18 F] 4e was selected for further investigation with microPET of the PD model, autoradiography and biodistribution experiments and compared with its non-deuterated [ 18 F] 6e . Finally, in vivo metabolic stability was analyzed by radio-HPLC. Results Ten tropane compounds had high DAT affinity (IC 50 = 2–21 nM), in which FP-CIT-d 6 ( 4e ) had the lowest IC 50 value of 2.7 nM. Radiolabelled [ 18 F] 4a-e and [ 18 F] 6a-e were obtained with radiochemical yields ranging from 10.6 ± 2.8% to 35.1 ± 5.4% with molar activities >20 GBq/μmol and the radiochemical purities >99%. [ 18 F] 4e showed the highest cell uptake (12%) and CFT inhibition efficacy (∼72%) among [ 18 F] 4a-e . MicroPET results showed [ 18 F] 4e has the highest target to non-target ratio (striatum/cerebellum). Therefore, [ 18 F] 4e was then selected for further biological evaluation. Ex vivo autoradiography experiment confirmed high specific binding of [ 18 F] 4e towards DAT. Biodistribution results indicated that [ 18 F] 4e has a higher striatum/cerebellum value than [ 18 F]FP-CIT ([ 18 F] 6e ) at 30–120 min. Furthermore, in vivo metabolism studies in rats revealed improved stability of [ 18 F] 4e as compared with that of [ 18 F] 6e . Conclusions The new probe [ 18 F] 4e is a promising candidate with good DAT affinity, specificity and metabolic stability for PET imaging, and might provide reliable diagnosis, treatment and prognostic detection of DAT-related neuropsychiatric diseases.

Abstract:As an add-on drug approved for Parkinson's disease treatment, safinamide has multiple functions, such as selective and reversible monoamine oxidase-B inhibition, voltage-sensitive sodium/potassium channel blockage, and glutamate release inhibition. Meanwhile, safinamide shows tremendous therapeutic potential in the context of other central nervous system diseases (e. g. ischaemic stroke, amyotrophic lateral sclerosis, depression, etc.). In this work, [F-18]safinamide, which is safinamide labelled by the positron-emitting radionuclide [F-18]fluorine, was synthesized automatically based on iodonium ylide precursors with high radiochemical yield and high molar activity. Density functional theory was applied to calculate the Gibbs free energy change during iodonium ylide-mediated fluorination and to interpret the effect of tetraethylammonium (TEA(+)) as the counter cation in these reactions to improve the nucleophilicity of [F-18/F-19]fluoride. In addition, positron emission tomography studies on Sprague Dawley rats were carried out to determine the imaging characteristics, pharmacokinetics, and metabolism of the [F-18]safinamide radiotracer. The results displayed the complete biodistribution of the radiotracer, especially in rat brains, and revealed that [F-18]safinamide has moderate brain uptake, rapid and reversible binding kinetics, and good stability.

Abstract:Research on fibroblast activating protein (FAP)-targeting inhibitor (FAPI) has become an important focus for cancer imaging and radiotherapy. Quinoline-based tracers [ 68 Ga]FAPI-04 and [ 18 F]FAPI-42 have been widely used for positron emission tomography (PET) imaging of most tumors. However, there exist some limitations of these tracers with high uptake in biliary duct system and unstable uptake in pancreas, unsuitable for abdominal tumors PET imaging. Here we developed a [ 18 F]-labeled glycopeptide-containing FAPI tracer (named [ 18 F]FAPT) for PET imaging of FAP in cancers.
Radiosynthesis and Preclinical Evaluation of [18F]F-DPA, A Novel Pyrazolo[1,5a]pyrimidine Acetamide TSPO Radioligand, in Healthy Sprague Dawley Rats

Abstract:PurposeMany neurological conditions result in the overexpression of the translocator protein 18 kDa (TSPO), today recognized as a biomarker for microglial activation and neuroinflammation imaging. The pyrazolo[1,5-a]pyrimidine acetamides are a particularly attractive class of TSPO-specific ligands, prompting the development of several positron emission tomography (PET) radiotracers. This includes F-DPA, a recently reported fluorinated ligand (Ki = 1.7 nM), wherein the fluorine atom is directly linked to the phenyl moiety without the presence of an alkyl or alkoxy spacer chain. Reported here is the preparation of [18F]F-DPA using [18F]Selectfluor bis(triflate) and the preliminary evaluation of [18F]F-DPA in healthy rats. Its metabolic profile and biodistribution in rats are compared with that of [18F]DPA-714, a closely related structure.Procedures[18F]F-DPA was synthesized by electrophilic fluorination using [18F]Selectfluor bis(triflate), [18F]DPA-714 was synthesized by conventional nucleophilic fluorination. The biodistribution of both radiotracers was compared in Sprague Dawley rats. Radiometabolites of both radiotracers in plasma and brain homogenates were analyzed by radioTLC.ResultsThe radiochemical yield of [18F]F-DPA was 15 ± 3 % and the specific activity was 7.8 ± 0.4 GBq/μmol. The radiochemical purity exceeded 99 %. The in vivo time activity curves of [18F]F-DPA demonstrate rapid entry into the brain and a concentration equilibrium at 20–30 min after injection. The metabolic profiles at 90 min after radiotracer injection in the plasma show that unchanged [18F]F-DPA and [18F]DPA-714 account for 28.3 ± 6.4 and 11.1 ± 2.6 % of the remaining radioactivity, respectively. In the brain, unchanged [18F]F-DPA accounts for 93.5 ± 2.8 % of the radioactivity; whereas for [18F]DPA-714, this value is 53.6 ± 1.6 %.Conclusions[18F]Selectfluor bis(triflate) was successfully used to label F-DPA with fluorine-18. The labeling position on the aromatic moiety imparts a higher stability compared to [18F]DPA-714 with regard to in vivo metabolism. [18F]F-DPA is a promising new radiotracer and warrants further investigation in animal models of disease.

Interaction of Ligands for PET with the Dopamine D3 Receptor: In Silico and In Vitro Methods

Abstract:[18F]Fallypride and [18F]Fluortriopride (FTP) are two different PET radiotracers that bind with sub-nanomolar affinity to the dopamine D3 receptor (D3R). In spite of their similar D3 affinities, the two PET ligands display very different properties for labeling the D3R in vivo: [18F]Fallypride is capable of binding to D3R under “baseline” conditions, whereas [18F]FTP requires the depletion of synaptic dopamine in order to image the receptor in vivo. These data suggest that [18F]Fallypride is able to compete with synaptic dopamine for binding to the D3R, whereas [18F]FTP is not. The goal of this study was to conduct a series of docking and molecular dynamic simulation studies to identify differences in the ability of each molecule to interact with the D3R that could explain these differences with respect to competition with synaptic dopamine. Competition studies measuring the ability of each ligand to compete with dopamine in the β-arrestin assay were also conducted. The results of the in silico studies indicate that FTP has a weaker interaction with the orthosteric binding site of the D3R versus that of Fallypride. The results of the in silico studies were also consistent with the IC50 values of each compound in the dopamine β-arrestin competition assays. The results of this study indicate that in silico methods may be able to predict the ability of a small molecule to compete with synaptic dopamine for binding to the D3R.

Abstract:Dopamine (DA) is involved in drug reinforcement, but its role in drug addiction remains unclear. Positron emission tomography (PET) is the first technology used for the direct measurement of components of the dopaminergic system in the living human brain. In this article, we reviewed the major findings of PET imaging studies on the involvement of DA in drug addiction, especially in heroin addiction. Furthermore, we summarized PET radiotracers that have been used to study the role of DA in drug addiction. To investigate presynaptic function in drug addiction, PET tracers have been developed to measure DA synthesis and transport. For the investigation of postsynaptic function, several radioligands targeting dopamine one (D1) receptor and dopamine two (D2) receptor are extensively used in PET imaging studies. Moreover, we also summarized the PET imaging findings of heroin addiction studies, including heroin-induced DA increases and the reinforcement, role of DA in the long-term effects of heroin abuse, DA and vulnerability to heroin abuse and the treatment implications. [18F]F13640: a selective agonist PET radiopharmaceutical for imaging functional 5-HT1A receptors in humans

Abstract:Abstract Purpose F13640 (a.k.a. befiradol, NLX-112) is a highly selective 5-HT 1A receptor ligand that was selected as a PET radiopharmaceutical-candidate based on animal studies. Due to its high efficacy agonist properties, [ 18 F]F13640 binds preferentially to functional 5-HT 1A receptors, which are coupled to intracellular G-proteins. Here, we characterize brain labeling of 5-HT 1A receptors by [ 18 F]F13640 in humans and describe a simplified model for its quantification. Methods PET/CT and PET-MRI scans were conducted in a total of 13 healthy male volunteers (29 ± 9 years old), with arterial input functions (AIF) ( n = 9) and test–retest protocol ( n = 8). Several kinetic models were compared (one tissue compartment model, two-tissue compartment model, and Logan); two models with reference region were also evaluated: simplified reference tissue model (SRTM) and the logan reference model (LREF). Results [ 18 F]F13640 showed high uptake values in raphe nuclei and cortical regions. SRTM and LREF models showed a very high correlation with kinetic models using AIF. As concerns test–retest parameters and the prolonged binding kinetics of [ 18 F]F13640, better reproducibility, and reliability were found with the LREF method. Cerebellum white matter and frontal lobe white matter stand out as suitable reference regions. Conclusion The favorable brain labeling and kinetic profile of [ 18 F]F13640, its high receptor specificity and its high efficacy agonist properties open new perspectives for studying functionally active 5-HT 1A receptors, unlike previous radiopharmaceuticals that act as antagonists. [ 18 F]F13640’s kinetic properties allow injection outside of the PET scanner with delayed acquisitions, facilitating the design of innovative longitudinal protocols in neurology and psychiatry. Synthesis and biological evaluation of [18F]C for in vivo studies of dopamine D4 receptor

Abstract:AIM: The 7-azaindole derivative(L-750, 667) is a novel, high-affinity (Ki = 0.43 nmol · L-1) and selective dopamine D4 R ligand. We synthesized and evaluated 3-{[-4-(4-[18F] fluorophenyl) methyl]piperazin-1-yl}-methyl-1H-pyrrolo [2,3-b] pyridine([18F] C), which is an analog of L-745, 870 binding D4 receptor in vitro. METHODS: The [18F]C was prepared by reductive alkylation of 3-(piperazin-1-yl)-methyl-1H-pyrrolo [2, 3-b] pyridine with 4-[18F] fluorobenzaldehyde in radiochemical yield (decay-corrected, 9.0% ∼ 12.0%) and with high specific activity (>37 GBq · μmol-1). RESULTS: A higher uptake w0as observed in the frontal cortex and hippocampus (0.43 % ID/g, 0.35% KID/g) compared to the striatum, The radioactivity in the medulla displayed higher uptakes than into others regional at all time points. CONCLUSION: Tissue distribution studies in rats demonstrated specific distribution of the [18 F]C in brain regions, suggesting that this radioligand may be a suitalble agent for in vivo studies of dopamine D4 receptor.
Synthesis and Fluoro-Radiolabelling of Dopamine D_2 Receptor Imaging Agent ~(18)F-FSABZM

Abstract:The preparation of a 18F-labeled imaging agent with higher affinity to dopamine D2 receptor is reported. The ligand of (S)-N-[(1-ethyl-2-pyrrolidinyl)methyl]-5- [N-(2-[18F]fluoroethyl)-N-methylsulfonyl]amino-2- methoxybenzamide (18F-FSABZM) was synthesized by nitryl reduction, N-dihydroxyethylation, and methylsulfon- ylation. Finally, 18F-FSABZM was prepared by nucleophilic fluorination of the mesylate with K18F/K222(Kryptofix). The chemical structure of the fluoro-labeling precursor SABZM,the labeling product with non-radioactive fluorine and all other intermediates were validated by 1HNMR and MS. Determined by HPLC, the labeling yield was 12%— 15%. After purifying, the resultant 18F-FSABZM, with a radiochemical purity of over 97%, was stable for more than 4 hours at room temperature. The whole process (including synthesis and analysis) took only 80—90 minutes.

Abstract:Mutations that increase leucine-rich repeat kinase 2 (LRRK2) activity in the brain are associated with Parkinson's disease. Here, we synthesized a novel compound 4-(6-fluoro-4-(5-isopropoxy-1H-indazol-3-yl)pyridin-2-yl)morpholine (FIPM) and labeled it with fluorine-18 (18F), to develop a positron emission tomography (PET) tracer for in vivo visualization of LRRK2 in the brain. FIPM showed high in vitro binding affinity for LRRK2 (IC50 = 8.0 nM). [18F]FIPM was prepared in 5% radiochemical yield (n = 5), by inserting 18F into a pyridine ring, followed by removal of the protecting group. After HPLC separation and formulation, [18F]FIPM was acquired with >97% radiochemical purity and 103-300 GBq μmol-1 of molar activity at the end of radiosynthesis. Biodistribution and small-animal PET studies in mice indicated a low in vivo specific binding of [18F]FIPM. While [18F]FIPM presented limited potential as an in vivo PET tracer for LRRK2, we suggested that it can be used as a lead compound for developing new radiotracers with improved in vivo brain properties.

Abstract:We synthesized a novel 18F-labeled dopamine D4 receptor antagonist (Ki=4.3nM), 3-(4-[18F]fluorobenzyl)-8-methoxy-1,2,3,4-tetrahydrochromeno[3,4-c]pyridin-5-one ([18F]FMTP), which has exhibited high affinity and selectivity. Radiosyntheses were accomplished by the reaction of fluorine-18-labeled intermediate with 8-methoxy-1,2,3,4-tetrahydrochromeno[3,4-c]pyridin-5-one (1) followed by HPLC purification. The overall radiochemical yield of the radiosynthesis was 19.5% (decay corrected), the specific radioactivity was about 110GBq/μmol and the radiochemical purity was greater than 99%, the time of synthesis and purification was approximately 110min. Tissue distribution studies of the [18F]FMTP in rats showed that the radioactivity in the brain was concentrated in frontal cortex and medulla, the region that has a high density of D4 receptors. Pre-treatment with nonradioactive FMTP (1.0mg/kg) produced a significant reduction of radioactivity in all the regions. About 40% of total radioactivity in plasma and 100% in rat brain extract represented unchanged radioligand at 60min after injection as determined by HPLC. These results indicate that [18F]FMTP have some specific binding to the D4 receptor.
DRD2 expression based on 18F-fallypride PET/MR predicts the dopamine agonist resistance of prolactinomas: a pilot study

Abstract:PurposeThe dopamine agonists (DA) have been used widely to treat prolactinomas. However, it is difficult to predict whether the patient will be responsive to DA treatment.MethodsWe aimed to investigate whether the in vivo expression of DRD2 based on F-18-fallypride PET/MR could predict the therapeutic effect of DA on prolactinomas. Seven patients with prolactinomas completed F-18-fallypride PET/MR. Among them, three patients underwent surgery and further tumor immunohistochemistry. Imaging findings and immunohistochemical staining were compared with treatment outcomes.Results(18)F-fallypride PET/MR was visually positive in 7 of 7 patients, and DRD2 target specificity could be confirmed by immunohistochemical staining. A significantly lower tracer standard uptake value (SUV) could be detected in the resistant patients (n = 3) than in the sensitive patients (n = 4; SUVmean, 4.67 +/- 1.32 vs. 13.57 +/- 2.42, p < 0.05). DRD2 expression determined by F-18-fallypride PET/MR corresponded with the DA treatment response.Conclusion(18)F-fallypride PET/MR may be a promising technique for predicting DA response in patients with prolactinoma.
3-(2'-[18F]fluoroethyl)spiperone: in vivo biochemical and kinetic characterization in rodents, nonhuman primates, and humans

Abstract:3-(2'-[18F]fluoroethyl)spiperone (FESP), a recently developed dopamine D2-receptor binding radiopharmaceutical, was used for dynamic characterization of dopamine-receptor binding in Macaca nemestrina monkeys and humans with positron emission tomography (PET). FESP in vitro binding properties to the dopamine receptor (IC50 = 1.5 nM) are similar to those of spiperone. Serial PET scans in monkeys after intravenous bolus injection of FESP revealed specific radioactivity accumulation in striatum (rich in dopamine D2-receptors), whereas radioactivity concentration declined after 20 min in frontal cortex (serotonin receptors) and more rapidly in cerebellum (nonspecific binding). Specific dopamine D2-receptor binding was saturated with increasing concentrations of radioligand (specific activity range: 1-10,000 Ci/mmol), was stereospecifically blocked with (+)butaclamol (0.5 mg/kg), and showed only partial displacement with spiperone (200 micrograms/kg, i.v. administration 90 min after FESP injection). From PET experiments with FESP in humans, it is possible to visualize accumulation of radioactivity in striatum in a manner similar to that observed in monkeys and, ex vivo, in rodents (adult male Sprague-Dawley rats). Biochemical analyses in rat brain revealed that the activity (approximately 90%) in striatum was unmodified FESP up to 4 h after injection. On the other hand, FESP was metabolized peripherally (rat greater than monkey greater than human), with only 11% of plasma radioactivity remaining as intact FESP in rodents and 54% in humans after 2 h. Based on these interspecies scaling pharmacokinetic data, it is unequivocal that FESP peripheral metabolites do not significantly contribute to the accumulated radioactivity in striatal tissue. Therefore, it is concluded that FESP is suitable for the quantitative estimation of dopamine D2-receptor sites using PET.
Evaluation of the early response and mechanism of treatment of Parkinson's disease with L-dopa using 18F-fallypride micro-positron emission tomography scanning

Abstract:The aim of the present study was to investigate the use of 18F-fallypride micro-positron emission tomography (micro-PET) imaging in the evaluation of the early therapeutic efficacy of L-dopa in the treatment of Parkinson's disease (PD) and the underlying mechanism. 18F-fallypride was synthesized and its specific binding with dopamine (DA) receptors in normal mouse brain was studied. Following the establishment of a mouse model of PD, the animals were divided into normal control, PD model and L-dopa treatment groups. General behavior, swimming test, locomotor activity counts, transmission electron microscopy, immunohistochemical analysis, high performance liquid chromatography-electrochemical detection and 18F-fallypride micro-PET imaging were used to study intergroup differences and the correlation between the changes of striatal uptake of 18F-fallypride and the therapeutic efficacy. The general behavioral features of PD model mice were similar to the clinical symptoms of PD patients and were alleviated after treatment. The swimming time, locomotor activity and frequency of standing posture of PD model mice were lower than those of the control mice, but had no difference from those of the control mice after L-dopa treatment. The number of tyrosine hydroxylase-positive neurons and the striatal contents of glutathione peroxidase, superoxide dismutase, DA and its metabolites 3,5-dihydroxyphenylacetic acid and homovanillic acid in the PD group were lower than those in the control group, but were significantly improved following the treatment; the significant reduction in DOPAC/DA and HVA/DA ratios post treatment suggested that the rate of DA metabolism decreased significantly. The striatal malondialdehyde content in the PD group increased compared with that in the control group, but was reduced after L-dopa treatment. Micro-PET imaging indicated that the uptake of 18F-fallypride in the mouse striatum of the PD group was lower than that of the control group and was significantly increased after the treatment. The mechanism of treatment of PD with L-dopa in mice may involve increasing the number of TH-positive cells and DA receptor levels, as well as reducing the rate of DA metabolism; such changes can be noninvasively observed in vitro by 18F-fallypride imaging.

Abstract:PurposeWe present the findings of our final prospective study submitted to the U.S. Food and Drug Administration (FDA) for New Drug Application (NDA) approval for the use of 3,4-dihydroxy-6-[18F]fluoro-l-phenylalanine (F-18 FDOPA) positron emission tomography (PET) imaging for Parkinson's disease (PD). The primary aim was to determine the sensitivity, specificity, and predictive values of F-18 FDOPA PET in parkinsonian patients with respect to clinical standard-of-truth (SOT). Secondary outcomes included the inter-rater reliability, and correlation of quantitative measures for PET with dopaminergic status.MethodsIn 68 parkinsonian subjects, F-18 FDOPA PET scan from 80 to 100 min was acquired following a CT scan. Scan images were presented to one expert in F-18 FDOPA image interpretation and two physicians with prior experience in I-123 FPCIT single-photon emission computed tomography image interpretation. Fifty-six subjects completed the study with a follow-up for SOT determination. Image readers were blind to the clinical/quantitative data; SOT clinician was blind to the image data.ResultsFor 47 of the 56 patients, SOT was in agreement with the PET scan results. For nine patients, SOT suggested dopaminergic deficit, whereas the imaging showed normal uptake. The specificity and positive predictive values are 91% and 92%, respectively, suggesting high probability that those who test positive by the PET scan truly have dopaminergic degeneration. The sensitivity was 73%. Inter-rater agreement was 0.6–0.8 between the different readers.ConclusionOur prospective study demonstrates high specificity and moderate sensitivity of F-18 FDOPA PET for PD. We received NDA approval in October 2019.

Abstract:By modifying the structures of targeted A2AR antagonists and tracers, novel compounds 3, 7a, 9, 12c, and BIBD-399 were designed and synthesized. In vitro inhibition experiments demonstrated that 3, 12c, and BIBD-399 have high affinity for A2AR. [18F]3 and [18F]BIBD-399 were successfully synthesized. In terms of biological distribution, the brain uptake of [18F]MNI-444 exhibits greater than that of [18F]3 and [18F]BIBD-399. PET imaging shows that [18F]3 is off-target in the brain, while [18F]BIBD-399 and [18F]MNI-444 can be specifically imaged in regions with high A2AR expression. Differently, [18F]BIBD-399 could quickly reach equilibrium in the targeted region within 10 min after administration, while [18F]MNI-444 shows a slowly increasing trend within 2 h of administration. [18F]BIBD-399 is mainly metabolized by the liver and kidney, and there is no obvious defluorination in vivo. Additional in vitro autoradiography showed that the striatal signals of [18F]BIBD-399 and [18F]MNI-444 were inhibited by the A2AR antagonist SCH442416 but not by the A1R antagonist DPCPX, demonstrating the high A2AR binding specificity of [18F]BIBD-399. Molecular docking further confirms the high affinity of MNI-444 and BIBD-399 for A2AR. Further tMCAo imaging showed that [18F]BIBD-399 can sensitively distinguish between infarcted and noninfarcted sides, a capability not observed with [18F]MNI-444. Given its pharmacokinetic properties and the ability to identify lesion regions, [18F]BIBD-399 has potential advantages in monitoring A2AR changes, meriting further clinical investigation.
