Treatment of infected nonunion of the tibia with the Ilizarov technique

Abstract:Objective To evaluate the Ilizarov technique in the management of infected nonunion of the tibia.Methods Twenty-two patients with infected nonunion of the tibial shaft were included.The eradication of infection was done by resection of massive segment infected tibial shaft followed by metaphyseal osteotomy and limb reconstruction using segment transport technique with Ilizarov technique within 8 ～ 25 weeks.Gradual distraction at the metaphyseal osteotomy site to reconstruct and replace the removed bone started 10 days postoperatively.The distraction rate was quarter millimeter every 6 hours.When the transported segment reached the docking site.Compression was maintained between the docking sites until bone union.Results All of the cases were followed up for 12 ～ 24 months.There was no recurrence of osteomyelitis at the nonunion site.An average duration of external fixation was 6.5 ～ 13 months.19 patients reached full bone healing with no bone graft in 6 months when the bone transport were finished.Three patients who didn't have a bony union were performed autograft at the docking site.One of them was performed plate internal fixation and autograft after removing external fixator.All of them achieved full bone union in 4 ～ 6 months.Conclusions Ilizarov technique is an effective method in treating infected nonunion of the tibial shaft,which can achieve complete healing.
