Study of giant cell tumor of the proximal tibia with preservation of the meniscus after tumor resection

Abstract:Objective To evaluate knee joint reconstruction by preserving the meniscus and reconstructing the tibial plateau with iliac plate autograft after resection of giant cell tumor of the proximal tibia involving single condyle.Methods 8 cases with giant cell tumors of one condyle of proximal tibias(6 lateral and 2 medial condyles) were included in this study.With the normal meniscuses and ligaments of knee joint preserved tumors were en bloc resected,tibial plateaus were reconstructed with iliac plates,and autograft of fibulae and firm internal fixations were performed to restore the normal structure of the knee joints.Results 8 cases' follow-ups overtop 120 months(120~153 months).Among which only one patient had a recurrence who should not have been performed this kind of operation.At last the patient had to be treated with replacement of man-made prothesis.No nonunion of bone happened and no reconstructed tibial plateau collapsed. All patients were comprehensively evaluated with the Enneking's Functional Evaluation System(95%).Conclusion The surgical technology could ensure that the tumors were resected completely and the normal anatomical structures were retained and restored as much as possible.Both complete resection and better reconstruction of joint functions were achieved. Therefore the operation can be used as a new choice in reconstructing knee joint after resecting one condyle of proximal tibia with tumor.
