Anesthetic Management for Repair of Ebstein's Anomaly

Abstract:Objective Twenty-two patients with Ebstein's anomaly (11 males, 11 females) aged 3 months-52 yrs weighing 5-80 kg received surgical repair from March 2004 to December 2005. The children were premedicated with intramuscular ketamine 6-7 mg·kg-1 and scopolamine 0.01 mg·kg-1 . Anesthesia was induced with sufentanil 1μg·kg-1 and pancuronium 0.1-0.2 mg·kg-1 . The adults were premedicated with morphine 10 mg and scopolamine 0.3 mg IM and oral diazepam 10 mg. Anesthesia was induced with midazolam 0.03-0.05 mg·kg-1 , etomidate 0.2-0.3 mg·kg-1 , sufentanil 1-2μg·kg-1 and pipecuronium 0.08-0.1 mg·kg-1 . The patients were intubated and mechanically ventilated. PETCO2 was maintained at 30-35 mm Hg. Anesthesia was maintained with 1.0%-1.5% isoflurane and intermittentⅣboluses of sufentanil and pipecuronium. ECG, direct BP, SpO2, PETCO2, CVP, body temperature and TEE were monitored during anesthesia. Measures were taken during operation to reduce preload and afterload of right ventricle, avoid increasing pulmonary vascular resistance, support cardiac function and maintain hemodynamic stability. The symptoms were greatly relieved after operation in all 22 patients. Ultrasound examination showed that the internal diameter of right atrium and ventricle were significantly decreased as compared to that before operation. No patient died from operation.
