Risk Factors of Ruptured Hepatocellular Carcinoma Following Transcatheter Arterial Embolization

Abstract:Objective:To study the risk factors and clinical characteristics of ruptured hepatocellular carcinoma (HCC) as a complication of transcatheter arterial embolization(TACE). Methods:A retrospective analysis was performed on 8 patients with HCC who happened tumor rupture after TACE within 1 week among 3527 cases in Liver Cancer Institute,Zhongshan Hospital,Fudan University from Dec. 2003 to Feb. 2008. Results:The incidence of this complication was 0.2% and the tumor rupture occurred between 11 and103 hours after TACE. All patients underwent conservative treatment and the mortality rate was 50%. Tumors of all 8 cases have the common features which adjacent to the surface of the liver or protrude out of the hepatic capsule and the longest diameter was greater than 5 cm. Arterio-portal shunt were found in 2 cases. All patients appeared abdominal pain,a drop in haemoglobin and most of them with haemodynamic changes. Conclusion:Ruptured HCC after TACE is a rare but life-threatening complication. Tumor larger than 5 cm and adjacent to the surface of the liver or protrude out of the hepatic capsule seems to be the relevent risk factors for rupture. Early diagnosis and active treatment are critical to improve the prognosis.
