Clinical retrospective observation of lung cancer between young and elderly patients

Abstract:Objective To investigate the clinical characteristics among young (≤40ys) and elderly patients (≥70ys) recently diagnosed with lung cancer. Methods The clinical data of the inpatients diagnosed with lung cancer by pathology or cytology were analyzed about age, gender, Eastern Cooperative Oncology Group performance status ( ECOG PS) , symptoms, pathological type, molecular status, TNM stage, metastatic sites and complications from Jan. 2012 to Dec. 2014. Results The average age of the two group (young and elderly) was 34. 88ys and 74. 48ys, male to female ratio was 0. 93 and 4. 64, respectively (P<0. 005). Majority of the patients showed 0-1 in ECOG status performance, and the common symptoms of all the patients were cough, chest distress and pain ( P>0. 1). 78. 57% of the young patients were adenocarcinoma, while 52. 48% (74/141) of the elderly patients were SCLC and squamous cell lung cancer. 31 young cases ( 55. 36%) showed EGFR mutation positive, and 64. 29%(36/56) of the young patients harbored driver-gene. In regard to TNM stage, 25% of the young group showed stage 0-Ⅱ, while 90% of the elderly patients with stageⅢ-Ⅳ(P<0. 01). Lung and pleura were the most frequent meta-static sites, followed by bone, brain, liver and adrenal gland in both groups. A few young patients had complica-tions, while 52. 5% of the elderly patients had co-morbidities. Conclusion The two groups have no specific clinical symptoms, and many of them show advanced stages, but good performance status. Adenocarcinoma and high possibil-ity of target genes are mostly common among young patients, while male, SCLC and squamous cell lung cancer, and more complications are common in the elderly patients. Further study should be focused on its early diagnosis, pre-vention, treatment and prognosis.
