CT and MR Imaging Findings of Renal Angiomyolipomas with Minimal Fat

Abstract:Objective To evaluate CT and MRI findings in the diagnosis of the renal angiomyolipoma(AML) with minimal fat. Methods CT and MRI findings in 15 cases with renal AML with minimal fat histopathologically confirmed were analysed retrospectively. Results Eighteen well-defined masses were found in 15 cases with minimal fat renal AML ranging from 1.5 cm×2.3 cm to 5.0 cm×7.0 cm in diameter. Sixteen masses were round whereas 2 masses appeared irregular. On precontrast CT, 12 masses showed homogeneously hyperdense, 2 masses were isodense, and 4 masses demonstrated small amount of fat in the mass. On dynamic-enhanced CT, marked enhancement in the cortical phase was seen in all 18 masses whereas decreased attenuation relative to renal parenchyma in the parenchymal phase was observed in all 18 masses with no enhancement of small amount of fat in 4 tumors. All tumors showed low intensity signal on T2-weighted MRI with fat suppression and most tumors demonstrated isointensity signal on T1-weighted imaging. Surprisely, small amount of fat failed to show high intensity signal on T1-weighted MRI in 4 lesions. The manifestations of dynamic-enhanced MRI were similar to those of dynamic-enhanced CT. Conclusions The renal AML with minimal fat could be confused with the renal cellular carcinoma, but characteristic CT features may distinguish between them. MRI would be indicated in some cases which could not be diagnosed differentially on CT.
