Use of Corticosteroid in Treating Patients with Systemic Lupus Erythematosus in Beijing

Abstract:To investigate the variance in the use of corticosteroid in treating patients with different stages of systemic lupus erythematosus(SLE)by physicians in Beijing.Methods A questionnaire was developed for this survey.SLE was divided into severe,moderate,and mild stages.Physicians who were involved in treating aduh SLE patients were investigated.Data of medication approach,dosage,and duration in treating patients with different stages of SLE were collected.Results For patients with severe SLE, 50% participants chose to use 1 000 mg of prednisone or equivalent dosage,20% chose to use higher than 1 003 mg,but 15% chose 60 mg.Seventy-one percent of participants chose to use the above dosage for 3 days.Ninety-four percent of participants preferred to use steroid intravenously.If the patient's condition became stable,all participants would change to use oral corticosteroid and decrease the dosage to 60 mg prednisone or equivalent dosage.Ninety-seven percent of participants would keep their patients have corticosteroid for more than 6 months.For patients with moderate SLE,40% of participants chose to use 60 mg prednisone or equivalent,while 17% chose to use higher than 500 mg and 22% chose less than 60 rag.Fifty-six percent preferred to choose oral corticosteroid while the rest would use it intravenously.Ninety- four percent of participants chose to have their patients using corticosteroid for more than 6 months.For patients with mild SLE,all participants would use 30-40 mg prednisone or equivalent dosage orally and would discontinue 6 months later.Conclusions The dosage and route of corticosteroid usage in treating patients with different stages of SLE varies between physicians.The variance is more evident for severe and moderate SLE than for mild SLE.Physicians should follow the guideline for SLE treatment in order to standardize the clinical practice.
