The effect of low-dose omeprazole on ulcer-like functional dyspepsia and on 24 h intragastric pH

Abstract:Objective: To assess the efficacy of low-dose omeprazole in the treatment of ulcer-like functional dyspepsia(u-FD) and its effect on 24 h intragastric pH. Methods: Eighty-two patients with u-FD were randomly divided into two groups: omeprazole 10 mg once daily for 2 weeks was given to 42 patients in omeprazole group and cimetidine 400 mg two times daily for 2 weeks was given to 40 patients in cimetidine group. The score of dyspepsia symptoms was recorded before and after two weeks of treatment, and the clinical efficacy was compared between the two groups. 24 h-intragastric pH monitoring was performed on first day and eighth day during treatment in 10 patients randomly-selected from omeprazole group and 10 patients randomly-selected from cimetidine group.Results: The total effective rates of symptoms for example epigastric pain, abdominal discomfort, sour regurgitation and heartburn in omeprazole group were significantly higher than those in cimetidine group(P<0.05~P<0.01). The score of total dyspepsia symptoms in both groups significantly decreased after two weeks of treatment(P<0.01). The score of total dyspepsia symptoms in omeprazole group after two weeks of treatment was lower than that in cimetidine group(P<0.05). On the first and eighth day of treatment, the introgastric pH in omeprazole group was 1.6~6.0(median 2.7) and 2.5~6.2(median 4.8) respectively,significantly higher than 1.2~3.1(median 1.5)(P<0.05) and 1.1~4.9(median 2.0) (P<0.01) in cimetidine group respectively. The total time percentage of pH>3 in omeprazole group was 23~81(median 49)% and 46~89(median 65)%, significantly higher than 13~54(median 26)% and 18~78(median 31)% in cimetidine group (P<0.01), respectively. The declined value of total dyspepsia symptoms score was positively correlated with the intragastric media pH(r=0.868,P<0.01) and the total time percentage of pH>3(r=0.928,P<0.01) in omeprazole group on the eighth day of treatment respectively. Conclusion: 10 mg daily omeprazole have a better clinical efficacy on u-FD and a greater inhibitory effect on gastric acid secretion comparing with 800 mg daily cimetidine .
