Clinicopathologic analysis of meningeal hemangiopericytomas

Abstract:Purpose To study the clinicopathologic features,immunophenotype,differential diagnosis and prognosis of meningeal hemangiopericytoma( M-HPC). Methods The clinical and pathological features were studied with HE and immunohistochemical staining in 29 cases of M-HPC,and the related literature were also reviewed. Results M-HPC was male predominance( 20 males: 9 females),and the age at first operation ranged from 14 to 82 years with an average of 48 years. The most frequent presenting symptoms were headache,limbs and / or head facial numbness and declining eyesight. 17 cases had tumors located in the supratentorial region,4in the infratentoria,5 in the tentorial,and 3 in the spinal cord. Histologically,the tumors were usually consisted of oval to spindle tumor cells with typical"staghorn"vessels and high cellularity. Focal edematous area was also presented in some cases. Immunohistochemically,all of the tumors were positive for vimentin,most of them were positive for CD34 and CD99,some were positive for BCL-2,and only a few cases showed weak expression of SMA,EMA and S-100. Ki-67 prolaferation index from 2% to 45%,with a median of 10%. A network of reticular fibers which surrounded the single tumor cell was detected in M-HPC. Conclusions M-HPC,which classified as a WHO grade Ⅱ / Ⅲ lesion,is a rare tumor which should be differentiated from other tumors,especially accompanied with hemangiopericytoma-like structure,and have a high tendency for local recurrence and metastases outside the central nervous system.
