Abstract:AIM: To understand the rehabilitative status and needs of children with intelligent disability, and analyze the rehabilitative characteristics of children from varied household socioeconomic status, so as to provide scientific evidence for the fair distribution of health care resources. METHODS: Totally 558 children were diagnosed as intelligent disability during the sampling survey on disability in 0-6 years-old children in 2001. Father's occupation, mother's education and family average monthly income were scored and made up of the comprehensive index of household socioeconomic status. Combining the disabilities and family background, the study described the current status and demands for the rehabilitation services, and analyzed the effect of household socioeconomic status on them. The severity for the intelligent disability followed the standard of Gesell score: 55-75 for mild; 40-54 for moderate; 25-39 for severe; < 25 for extremely severe. Stratification for the socioeconomic factor: total score of 3-5 for lower class; 6-8 for lower to middle class; 9-11 for middle to upper class; 12-15 for upper class. RESULTS: All the 558 children with intelligent disability screened in the survey were involved in the analysis of results. ① Of the 558 children with intelligent disability, the proportion for the severity of mild, moderate, severe and above was 54.7%, 25.6% and 19.8% respectively. ② The proportion for the socioeconomic factors from low to higher was 45.7%, 38%, 11.6% and 4.7% respectively. There was no age difference but gender difference among children with different household socioeconomic status, the proportion was higher in girls from families of lower class (46.7%), but it was only 19.2% in those from upper ones. ③ All the children with intelligent disability had rehabilitative needs, and 64.5% of them received various rehabilitative services, mainly from family (51.6%), and followed by special institution (25.6%), ordinary institution (16.1%) and hospital (6.6%). ④ The proportion of rehabilitation was the lowest in the children with the family average monthly income of 200-400 yuan (53.8%). According to the order of income from low to high, the proportion of receiving rehabilitation was 70.0% (lower than 200 yuan), 67.7% (401 -600 yuan), 65.2% (601-800 yuan and above 800 yuan as one group), and the was no significant difference. The proportion of rehabilitation was the lowest in the children from families of lower to middle class, and there were no significant differences in the utilization rate of rehabilitative service among children from families of other classes (lower class, middle to upper class and upper class). The children from families of upper class, middle to upper class only demanded for special institution, but those from families of lower class were in great demands for ordinary institution, special institution and hospital, and nearly half of the children who rehabilitated at home were seeking for the rehabilitative services of higher classes. CONCLUSION: The rehabilitative status of the young children with intelligent disability was serious. It is necessary to supply with correct guidance of effective rehabilitative form for the patient and to strengthen the building of special and ordinary institutions. The rehabilitative instruction for the children from families of lower to middle class and the fair utilization of health care resources should be paid more attention.
