Abstract:Cleft lip and palate(CLP) patients always have some degree of jaw deformities,which need to be corrected by orthognathic surgery.The surgical techniques include traditional orthognathic surgical approach and maxillary distraction osteogenesis(DO).Le Fort Ⅰosteotomy,the most widely used surgical approach,is applicable to light and moderated maxillary hypoplasia,which often need to have bilateral sagittal split ramus osteotomy(BSSRO) and genioplasty at the same time to improve the aesthetic result and occlusion.The Le Fort Ⅰmaxillary DO or anterior maxillary segmental distraction can be performed with external or internal distraction devices depending on the treatment objective.The traditional orthognathic surgical approach and maxillary DO all have their strong points and limitations.In order to get a good clinical effect,the doctor should have overall consideration of his own clinical experience and the patient's deformity,and thoroughly discuss with the patients to make a reasonable treatment plan.
Internal Midface Distraction In Correction Of Severe Maxillary Hypoplasia Secondary To Cleft Lip And Palate

Abstract:Background: Maxillary hypoplasia is a familiar deformity in patients with cleft lip and palate. A large amount of maxilla advancement is often needed to correct the severe deformity, but local soft-tissue scars around the maxilla restrict maxilla advancement and increase the relapse rate. By gradually lengthening both the bones and the soft tissues, midface distraction can greatly increase postoperative stability and lower the relapse rate.Methods: Ten patients with severe maxillary hypoplasia secondary to cleft lip and palate were treated with midface distraction using three kinds of internal distraction devices. Among them, six patients received an alveolar bone graft from the iliac crest during their Le Fort I osteotomy, and a bilateral sagittal split ramus osteotomy was performed simultaneously to push back the mandible in five patients with prognathia, to obtain a normal soft-tissue profile and occlusal relationship.Results: Successful maxillary advancements ranging from 5 to 15 mm were measured from preoperative and postoperative cephalogranis. Patients' sella-nasion-point A angles increased from an average of 71.25 degrees preoperatively to 79.05 degrees postoperatively. Orthodontic therapies were adopted before and/or after midface distraction. After the consolidation period, dense new bone was found to have formed in the distraction gap. During the follow-up period, the position of the maxilla and the final occlusal relationship were stable and acceptable, and no obvious relapses were seen.Conclusion: Midface distraction is an ideal choice for the correction of severe maxillary hypoplasia secondary to cleft lip and/or palate.
[Transpalatal Modified Le Fort I Osteotomy for Correction of Maxillary Hypoplasia in Cleft Lip and Palate Patients: a Preliminary Clinical Application].

Abstract:OBJECTIVETo evaluate the surgical corrective results of maxillary hypoplasia in patients with cleft lip and palate withtranspalatal modified Le Fort I osteotomy.METHODSIn the study, 11 patients (4 women, and 7 men) with maxillary hypoplasia secondary to cleft lip and palate underwent transpalatal modified Le Fort I osteotomy at Peking University School of Stomatology from Jan. 2012 to Dec. 2013,with the mean age of 21 years ( from 18 to 27 years), Bilateral sagittal split ramus osteotomy (BSSRO)and genioplasty were performed simultaneously in 9 of them for better appearance and functional occlusion. Lateral cephalometric radiographs were taken and traced before surgery, immediately after surgery and 6 months after surgery. The position of subspinale (A) on horizontal direction, the angle of sella-nasion-subsipmale (SNA) and the angle of sella-nasion-supramental (SNB) were collected and analyzed to evaluate the results.RESULTSAll the patients were uneventful with transpalatal modified Le Fort I osteotomy. All of them had a better profile and a satisfactory occlusionafter operation.The position of A was moved forward (6.6±1.1) mm on average in horizontal direction when surgery was completed, and maintained (6.0±1.2) mm on average 6 months after surgery. The average of SNA was 75.9°±2.8° before surgery,81.6°±8.6° immediately after surgery, and maintained 81.0°±2.6° 6 months after surgery. The average of SNB was 82.6°±3.7° before surgery, 78.0°±2.4° immediately after surgery, and maintained 78.5°±2.4° 6 months after surgery.CONCLUSIONThe maxillary hypoplasia in cleft lip and palate patients can be successfully corrected with transpalatal modified Le Fort I osteotomy and the functional occlusion can be achieved simultaneously. The effect of deformity correction was satisfactory. Transpalatal modified Le Fort I osteotomy can move maxilla more sufficiently, especially applicable for the patient with severe palatal scars preoperatively.

Abstract:Cleft lip and palate is a congenital craniofacial anomaly that affects the lip and oral cavity. The management and orthodontic treatment of this anomaly is important but challenging. This article reports the successful treatment of a patient with bilateral cleft lip and palate, Class III malocclusion, bilateral crossbite, crowding and microdontic maxillary lateral incisors. One mandible incisor was extracted, and three miniscrew anchorages were utilized to distalize the maxillary left dental arch and retract the mandibular arch. After treatment, ideal occlusion and a better profile were established, and long-term stability was confirmed by a 4-year follow-up. This article represents a successful attempt of orthodontic camouflage treatment of severe dentofacial discrepancy, as an important part of the series treatment of cleft lip and palate, to provide some insight into the clinical field.

[Application of Distraction Osteogenesis in Severe Maxillary Hypoplasia Secondary to Cleft Palate].

Abstract:OBJECTIVE:To evaluate the effect of distraction osteogenesis in correction of severe undeveloped maxilla secondary to cleft palate and to evaluate the selection of distraction modality.METHODS:Distraction osteogenesis was performed upon 8 patients, 6 males and 2 females, aged 11 to 25 years, one with incomplete cleft palate, 4 with unilateral complete left palate, and 3 with bilateral complete cleft palate, all accompanied with severe maxillary hypoplasia. The 7 adult patients were treated by modified high-stepped Le Fort I osteotomy, the other patient, a child, was treated by Le Fort I osteotomy followed by rigid external distraction. X-ray films of skull in lateral, frontal, or panoramic positions, and X-ray film of temporomandibular joint in Schiller's position were taken pre- and postoperatively. In model surgery the accurate distraction distance was measured and the direction of distraction designed. Orthodontic therapy was started immediately after the distraction was completed and the distractor was removed after a period of 4 months' consolidation.RESULTS:The designed distraction was achieved in a11 8 patients. Except in one case with a bone defect 1 cm x 0.5 cm at the osteotomy line in the lateral wall of the right maxillary sinus, dense new bone was formed in the area of distraction in the other cases. The postoperative occlusal relationship was good and stable. No infection and other complication was found. During an average of 20 months' follow-up, the maxilla and occlusion were stable, and no relapse was found. The distance of forward distraction of the maxilla reached an average of l2 mm (5 approximately 15 mm). The SNA angle increased from 71 degree on average preoperatively to an average of 79 degree postoperatively. Both the facial appearance and occlusal relationship returned to normal in a11 the patients.CONCLUSION:Without need of bone grafting and with a stable effect and little influence on platatopharyngeal competence, distraction osteogenesis is an effective method of correcting maxillary hypoplasia secondary to cleft palate and is worthy of spreading.
[Effects of maxillary distraction osteogenesis on the velopharyngeal configuration of cleft palate patients].

Abstract:OBJECTIVE:To study the effects of internal maxillary distraction osteogenesis(DO) on the velopharyngeal configuration of cleft palate patients. METHODS:Ten patients with severe maxillary hypoplasia secondary to cleft lip and palate patients (7 males and 3 females, average age 20.1 years old) had undertaken high step LeFort I osteotomy, and internal maxillary distraction devices were applied to advance the maxilla. Before surgery, when DO was completed and 6 months after DO was completed, oriented lateral cephalograms at rest position of each patient were taken, and 6 measure indexes of velopharyngeal configuration were collected and analyzed. RESULTS:All patients had successfully accomplished maxillary DO and the maxilla had been averagely advanced 11.3 mm. PNS-PhW, C-PhW, UL and ANS-PNS-T had all significantly increased, and UD had significantly decreased when DO was completed and 6 months after DO was completed as compared with pre-surgery. No significant linear correlation was found between maxilla advancement distance and velopharyngeal configuration changes. CONCLUSION:Correction of maxillary hypoplasia secondary to cleft palate surgery by using internal maxillary DO can increase the velopharyngeal cavity depth, and may impair velopharyngeal competence, but the compensatory changes of velopharyngeal soft tissue can alleviate this impairment to certain extent.

Abstract:OBJECTIVE:To develop a new method to correct secondary nasanl deformities of cleft lip and palate.METHODS:Establishing the balance of maxilla on both sides, followed by varied procedures to correct anormaly of osseous-cartilagious framework of nose, and taking some of nasal-labial muscle to restore the continuity and balance of anterior facial expressive muscles.RESULTS:78 cases were treated by this way and the result was satisfying.CONCLUSIONS:Imbalance of maxilla and anterior facial expressive muscle and anormaly of nasal framework should be repaired for correcting the nasal deformity of cleft lip and palate effectively.

Abstract:Cleft lip and palate is a common congenital developmental defect, which is mainly manifested as congenital oral and maxillofacial malformations. In patients with cleft lip and palate, jaw deformity often involves three-dimensional orientation, among which the correction of sagittal dysmorphism is the key point and difficulty in its diagnosis and treatment. In this article, we will elaborate on the indications and corrective methods for the treatment of maxillary retraction of cleft lip and palate, and introduce our experience in the sequential treatment of cleft lip and palate maxillary retraction in order to promote the standardization of its diagnosis and treatment.
[Management of Secondary Nasolabial Deformities after Bilateral Cleft Lip Repair].

Abstract:OBJECTIVE:To evaluate the method to correct the seconary nasolabial deformities after the surgical treatment in the patients with bilateral cleft lip.METHODS:From January 2000 to June 2003, forty patients with secondary deformities following repair of bilateral cleft lip were treated with a combined treatment procedures. AU of the forty cases underwent the following preoperative treatments: alveolar bone graft in 28 cases, preoperative orthodontics in 22 cases, prosthodontics in 20 cases and orthognathic surgery or distraction osteogenesis treatment in 20 cases, respectively. In order to improve the enlongation of nasal column, reconstruction of Cupid's bow, philtrum and correction procedures, continuous incision was made from the vermilion in median of the upper lip, the scar edge, the bilateral sides of the nasal column to the inner side of the nose, even extending the bilateral incision to nasolabial groove and nostril fundus.RESULTS:Forty patients were got the follow-ups for 3 months to 3.5 years and the satisfactory rate reached 95%.CONCLUSION:It is natural to emphasize the setting up of odontomaxillary frame and then utilize the surgical procedure to correct the secondary nasolabial parenchyma deformities. The method could be feasible and reliable for the correction of the secondary nasolabial parenchyma deformities after bilateral cleft lip repair.

Abstract:OBJECTIVE:Complex secondary deformities of cleft lip include nasal and labial deformities, alveolar cleft, oronasal fistula, and so on. In this report, the authors present a method to simultaneously repair the above complex deformities with autogenous costal cartilage and rib grafts.METHODS:The surgical procedure of simultaneous correction includes RESULTS:: Satisfactory outcomes were achieved in all 22 patients. Fifteen patients were followed up for 6 to 24 months and exhibited good nasal shape, repaired alveolar cleft, improved labial shape, and closed oronasal fistula. Three patients presented with red nasal tips. One patient had local infection, which was cured by antibiotics.CONCLUSION:This methodAs this method has led to positive outcomes, it has the potential to be widely used in clinical practice.

Abstract:Maxillary hypoplasia is a craniofacial characteristic in patients with isolated cleft palate (ICP). For maxillary hypoplasia with dental arch length deformity and severe crowding, premaxillary distraction osteogenesis can correct maxillary hypoplasia and increase the dental arch length to eliminate crowding. This case report describes the interdisciplinary treatment of a 20-year-old female with ICP with severe maxillary hypoplasia, mandibular prognathism and asymmetry, severe maxillary crowding, missing teeth, midline discrepancy, and anterior crossbite. An interdisciplinary treatment plan that included comprehensive orthodontic treatment, premaxillary distraction osteogenesis, mandibular orthognathic surgery, implant surgery, and endodontic and periodontal treatment was implemented to correct craniofacial deformity and malocclusion. A harmonious facial profile with satisfactory occlusion was obtained at the end of the interdisciplinary treatment. At 1-year follow-up, the treatment outcomes were stable. This case report aimed to demonstrate that interdisciplinary collaboration and excellent patient compliance are essential for patients with ICP to achieve functional occlusion and harmonious facial esthetics.
[Early Face Mask Treatment for Maxillary Hyperplasia in Patients with Cleft Lip and Palate]

Abstract:OBJECTIVE: Early interfering orthodontics were used to treat maxillary hyperplasia by face mask and extra-oral bow technique in teenages.The position changes of maxilla after orthodontic treatment were evaluated.METHODS:The cephlometric variables before and after treatment were measured.Variables after treatment were compared with normal values in children with same ages,and changes of the maxillary position was determind.RESULTS:The position of the maxilla after orthopedics was significantly advanced (P<0.001),but there was still difference from normal values.CONCLUSION:In the peroid of mixed dentition of patients with cleft lip and palate,early orthodontics on maxillary hyperplasia could stimulate maxillary development and growth,remove or improve crossbite,extra-oral bow and face mask were efficient tools for this kind of patients.

Abstract:Objective To prevent and treat secondary dento -maxillofacial deformities via the joint orthodontics and orthognathic surgery following the repair cleft lip and palate and to evaluate their outcomes. Methods 56 cases with secondary dento-maxillofacial deformities following the repair cleft lip and palate were prevented and corrected by the joint orthodontics and orthognathic surgery in our hospital. There were 30 males and 26 females, ranging from 8 to 38 years in age. 25 teenage cases were performed maxillofacial growth by orthodontics and alveolar bone graft. 31 adult cases were undergone preoperative orthodontics, their surgical procedures included: ① alveolar bone graft before orthoganathic procedure; ② Le Fort Ⅰ osteotomy; ③ multisegment Le Fort Ⅰ osteotomy; ④Le Fort Ⅰ osteotomy and bilateral sagittal split ramus osteotomy (BSSRO); ⑤BSSRO and genioplasty. Rigid internal fixation with Titanium palates and screws was applied and elastic traction of 2～3 weeks intermaxillary was essential in each case. All patients were undergone postoperative orthodontics and followed -up and ranged from 12 to 49 months. Results The teenage patients had a much-improved facial profile and occlusion. The adult cases had a satisfactory facial proportion with a good occlusion relationship and facial figure. Conclusion The joint orthodontics and orthognathic surgery could be performed satisfactorily to prevent and correct secondary dento-maxillofacial deformities following the repair cleft lip and palate. The authors recommend the joint orthodontics and orthognathic surgery should have individuation basing on patients' occlusion relationship and their requirements.

Abstract:Clinically, patients with operated unilateral cleft lip and palate always present with a concave profile, depressed midface, maxillary hypoplasia, narrow upper dental arch, and class III malocclusion. In this clinical report, the authors describe the successful orthodontic treatment of a patient with unilateral cleft lip and palate. A boy, 7 years 11 months of age, with a history of unilateral cleft lip and cleft palate presented with a Class I malocclusion on Skeletal Class III base. A satisfactory occlusion and a favorable lateral profile were achieved after maxillary protraction (face mask) combined with fixed appliance treatment, including alveolar bone grafting surgery. An acceptable occlusion and facial proportion were maintained after a 3-year retention period. These results suggest orthodontic treatment with growth interference is an effective option for a patient with cleft lip and palate.
[Correction of the Secondary Nasal Deformity in Bilateral Cleft Lip with a New Technique].

Based on the thoughts that secondary deformities of the nose and lip should be treated as a whole, the authors introduced a new technique, in which five aspects of the deformities were considered and treated at the same time. Skeletal Stability after Maxillary Advancement with Orthognatic Surgery in Patients with Cleft Lip and Palate

Abstract:Objective To investigate the skeletal stability after maxillary advancement with orthognatic surgery in patients with cleft lip and palate and the factors related to relapse.Methods 34 cleft lip and palate patients underwent modifiedLeFort I maxillary advancement osteotomy,29 of them also had BSSRO and/or genioplasty.The clinical follow-ups were no shorter than 12 months.Lateral cephalograms were taken at presurgery,immediately postsurgery and at least 1 year postsurgery.The positional change of point A and PNS and angular change of SNA were measured by serial cephalometric analysis.Results Horizontal advancement averaged (4.38±1.88) mm,and the mean relapse was (20.10±18.39) 0％.Downward surgical movement averaged (3.46±2.15) mm,and the vertical relapse was (34.78±32.89)％.∠SNA was 77.9° presurgicaly,82.3° immediately after surgery and 81.4°at least 1 year postoperatively.There was no significant correlation between surgical movement and postoperative relapse in the horizontal plane(P＞0.05),but had a significant positive correlation in the vertical plane (P＜0.05).Variance analysis of continuous follow-ups of 15 patients showed most of the relapse occurred in the first 3 months after surgery.Conclusions Maxillary surgical advancement in patients with CLP showed relapse to a certain extent.Most of the relapse occurred in the first 3 months after surgery.The vertical relapse was positively correlated with inferior surgical movement of the mandible.

Abstract:Maxillectomy in childhood not only causes composite primary defects but also secondary malformation of the middle and lower face. In the case presented, we introduced computer-assisted planning and simulation of orthognathic surgery combined with fibular osteomyocutaneous flap reconstruction to correct complex craniofacial deformities. Virtual orthognathic surgery and maxillary reconstruction surgery were undertaken preoperatively. LeFort I osteotomy, with bilateral sagittal split ramus osteotomy and lower border ostectomy, was performed to correct malocclusion and facial asymmetry. Maxillary reconstruction was accomplished using a fibular osteomyocutaneous flap. The patient recovered uneventfully with an adequate aesthetic appearance on 3D computed tomography. Our experience indicates that orthognathic surgery combined with fibular osteomyocutaneous flap reconstruction can used to correct complex facial asymmetry and maxillary defects secondary to maxillectomy. Computer-assisted simulation enables precise execution of the reconstruction. It shortens the free flap ischemia time and reduces the risks associated with microsurgery.

Abstract:Conventional maxillary distraction osteogenesis and anterior maxillary segmental distraction were applied in the treatment of severe maxillary hypoplasia secondary to cleft clip and palate. The aim of the present study was to compare the difference between these 2 osteotomy modalities used for rigid external distraction.Ten patients with severe maxillary hypoplasia secondary to CLP were enrolled in our study. They were randomly divided into 2 groups. Conventional maxillary distraction osteogenesis was performed in 5 patients and anterior maxillary segmental distraction in 5 patients. The preoperative and postoperative lateral cephalograms were compared, and cephalometric analysis was performed. The independent sample t test was used to evaluate the differences between the 2 groups.All patients healed uneventfully, and the maxillae moved forward satisfactorily. The sella-nasion-point A angles, nasion-point A-Frankfort horizontal plane angles, overjets, and 0-meridian to subnasale distances had increased significantly after distraction osteogenesis. Significant differences were found in the changes in palatal length between the 2 groups (P < .05). A mean increase of 7.50 mm in palatal length was found in the anterior maxillary segmental distraction group. No significant difference in the changes in palatopharyngeal depth or soft palatal length was found.With the ability of increasing the palatal and arch length, avoiding changes in palatopharyngeal depth, and preserving palatopharyngeal closure function, anterior maxillary segmental distraction has great value in the treatment of maxillary hypoplasia secondary to CLP. It is a promising and valuable technique in this potentially complicated procedure.
