The arthroscopic fixation technique from posterior middle portal for tibial avulsion fracture of posterior cruciate ligament

Abstract:Objective To evaluate the outcome of arthroscopic exchange rod and posterior middle portal technique in the treatment for tibial avulsion fracture of posterior cruciate ligament.Methods From Jan.2008 to Oct.2009,a total of 12 arthroscopically treated cases of PCL tibial avulsion fracture were retrospectively evaluated.After the routine examination of the joint,both the avulsed bone block and the tibia bone bed were refreshed.The additional posterior middle portal was established,and translation of the incision with exchange rod technique was used to fix the avulsed bone vertically by chancellors bone screw from posterior middle portal.The fixation method was the same with the open reduction.Rehabilitation began early after surgery.Results The result showedreduction of the avulsion fragment was obtained in all cases.The fixed screws were all vertical to fracture line.Four cases were out of follow-up.Eight cases were followed up.For these eight followed patients,flexion and extension were back to normal within four weeks,and they returned to normal walk in six weeks.The bone healing was good in three months without any vascular or nerve complications.The Lysholm score was 97.9 in average.IKDC score was normal in seven cases,and one was nearly normal.KT2000 was 0.83 in average.All the patients regained the pre-injury activity level.Conclusions Arthroscopic vertical fixation by transplation and exchange rod technique through posterior middle portal is a simple,reliable,and minimal invasive treatment for PCL tibial avulsion fracture.It is a kind of virtue all arthroscopic technique,no need for bone tunnel.The fixation is direct and stable,compared with open reduction.Moreover,it's good for early rehabilitation.
