Repetitive Nerve Stimulation in Amyotrophic Lateral Sclerosis Patients

Abstract:Aim:To evaluate the characteristics of the decrement to repetitive nerve stimulation(RNS)in patients with amyotrophic lateral sclerosis(ALS).Methods:RNS of the median nerve at 3Hz was measured in 49 ALS patients and tetanic contraction was carried out in 16 muscles with decrement.Compound muscle action potential(CMAP)amplitude as well as region of onset,course of disease was also evaluated.Results: Decrement of more than 10%was found in 15 patients and borderline decrement(5%~9%)in another 13 patients.Decrement was found more frequent in patients with upper limb onset compared with the other two groups of lower limb and bulb onset.The correlation between decrement and CMAP size was significant(r= -0.474)and no relationship lied between decrement and course of disease.Post exercise facilitation(PEF)was demonstrated in 4(range from 123%to 200%)and post tetanic facilitation(PTF)in 6 and post tetanic exhaustion(PTE)in 5 of the 16 muscles.Conclusion:Impairment of neuromuscular transmission is not an uncommon feature in ALS.The decrement of CMAP appeared to relate with the severity of axonal damage; patients with upper extremity onset were more likely to show decrement of thenar muscles;pre-synaptic mechanism might occur in some of ALS patients.
