Imaging Features of Encapsulated Papillary Carcinoma of the Breast

Abstract:Objective:To study the imaging features and pathological features of encapsulated papillary carcinoma of the breast.Methods:Seven cases of encapsulated papillary carcinoma of the breast who underwent operation and confirmed by the postoperative pathology from July,2012 to December,2012 were enrolled.Their ultrasonic and mammographic images,as well as pathological features were analyzed.Results: The ages of the patients ranged from 57 to 77 years with an average of 70 years.All of them visited hospital after they noticed breast masses.The ultrasonic findings indicated that the masses were in irregular shape with unclear boundaryies;internal echo was mixed echo;in case of no echo,low echo was found with papillae and rear echo was enhanced;calcification has not been found;blood flow signals were visible around the tumor;all tumors were BI-RADS IV.The mammographic findings indicated that the masses were in irregular shape with superficial lobular boundaries,high density and calcification;all tumors were BI-RADS IV.The average diameter of the tumor was 20mm(12~40mm) in pathological examination.Fine fibrovascular cores were found in pathological examination;neoplastic epithelial cells were of low or intermediate nuclear grades surrounded by a fibrous capsule.ER,PR were strongly positive in neoplastic epithelial cells;but p53,Ki67,cerbB-2 were low or negative.P63 expression in myoepithelial cell layer within the papillae or at the periphery of the lesion was negative.Conclusions: The unique features of ultrasonic and mammographic images and pathological features were helpful in identification and diagnosis of encapsulated papillary carcionma.
